Dr. Miyai has been setting the highest standards of excellence in orthodontics for over 5 years. Dr. Miyai earned his undergraduate degree from the University of Hawaii at Manoa and his dental degree from the University of Nevada, Las Vegas, where he was honored with the title of valedictorian, as well as several other academic honors. Dr. Miyai then went on to complete a specialized post-graduate residency in orthodontics from Louisiana State University in New Orleans. After several years as an associate orthodontist, Dr. Miyai returned home to Hawaii to open his own private practice on Maui, Kauai, and the Big Island.
Dr. Miyai enjoys giving back to his community through charitable work via the Shriners Hospital for Children. In his spare time, he likes to read, work out, fish, hike, cook, and spend time with his family and friends.
A native Floridian, Dr. Alex earned his Bachelor of Science in biology from the University of Central Florida in 2012. He attended Nova Southeastern University, where he received his Doctor of Dental Medicine in 2016. Committed to representing his colleagues, he served as the president of his class and executive president of student government for the College of Dental Medicine. As a result of his leadership, he was honored to receive numerous awards, including the prestigious College of Dental Medicine Student of the Year award. Following dental school, Dr. Alex attended a three-year postgraduate residency in orthodontics and dentofacial orthopedics at the Georgia School of Orthodontics.
Dr. Alex is a lifelong learner and stays up to date on progressive orthodontic techniques and procedures. Dedicated to the highest standards of care for his patients, he completed the optional board certification process and is an active Diplomate of the American Board of Orthodontics. In his free time, Dr. Alex enjoys photography, aviation, hiking, fishing, fitness, playing a variety of instruments and spending time with family and friends.

Amazing staff and Doctors. We were traveling from Missouri when on day 2 in Maui, my daughter told me a wire from her braces was digging into her cheek which could have ruined our whole trip. I called Hawaii Smile Designs...even though they had a full schedule they were able to get my daughter in and repair a slipped wire. If I lived in Maui, this is where I would be taking my children for orthodontics. Their main concern was that my daughter was in pain. Thank you so much for helping us!!!!

During our Maui vacation my daughter's brace wire came off of it's bracket. We called various Orthodontic businesses on the island and luckily Hawaii Smile Designs was able to help us. It was a quick fix and we were back to enjoying our vacation. The staff at Hawaii Smile Designs was so kind and welcoming they have so much aloha spirit. We felt so lucky to find them and if we lived on the islands (we wish) Hawaii Smile Designs would be our go to for any Ortho needs.

Took us in last minute due to a loose bracket my daughter had on her braces- so nice, friendly and really took care of my daughter. Lifesaver for our trip in Maui!

On our visit to Maui, my sons retainer was becoming unbearable and Hawaii Smiles Designs saved our vacation. They squeezed my son in the next morning and were able to help him. He was so relieved and happy. We can't thank all the staff enough. You all were so kind, and what a super cute office! Wish you were in our home town.

She usually adjust my braces and then another orthodontists comes in. He usually checks my teeth progress and tells the other orthodontist what to put on my braces. They even gave me a birthday present . In conclusion I think they are very professional and all very kind, and if you're thinking about getting braces or Invisalign make sure to go to them!!
