Your tooth consists of two main parts: the crown, which is that part of the tooth above the gum and visible in your mouth; and the root or roots, which is that part of the tooth that lies beneath the gum and is surrounded by bone. Inside each root is a channel that runs the length of the tooth. This channel is the root canal and contains the pulp (nerves, blood vessels, and soft tissue), which is often referred to as the nerve of the tooth. The pulp may become irreversibly damaged by bacteria associated with decay, very deep restorations, fractures, trauma, or periodontal disease.

In order to preserve a tooth in which this has occurred, it is necessary to remove the diseased pulp tissue. This procedure is known as endodontic therapy. Since endodontic therapy is concerned with removing only the pulp from the root canal, the hard/supportive portion of the root will continue to function normally to keep the tooth firmly functioning within the bone. It is always advisable to remove an injured pulp because it may become infected, cause varying degrees of pain, and/or act as an irritant to the surrounding bone and nerve tissues.

The perception of root canals being painful began decades ago but with modern technologies and anesthetics, root canal treatment today is no more uncomfortable than having a filling placed. In fact, a recent survey showed that patients who have experienced root canal treatment are six times more likely to describe it as painless than patients who have not had root canal treatment.

Most patients see their dentist or endodontist when they have a severe toothache. The toothache can be caused by damaged tissues in the tooth. Root canal treatment removes this damaged tissue from the tooth, thereby relieving the pain you feel.

The myth: Patients searching the Internet for information on root canals may find sites claiming that teeth receiving root canal (endodontic) treatment contribute to the occurrence of illness and disease in the body. This false claim is based on long-debunked and poorly designed research performed nearly a century ago by Dr. Weston A. Price.

In the 1920s, Dr. Price advocated tooth extractionthe most traumatic dental procedureover endodontic treatment. This resulted in a frightening era of tooth extraction both for treatment of systemic disease and as a prophylactic measure against future illness.

The truth: There is no valid, scientific evidence linking root canal-treated teeth and disease elsewhere in the body. A root canal is a safe and effective procedure. When a severe infection in a tooth requires endodontic treatment, that treatment is designed to eliminate bacteria from the infected root canal, prevent reinfection of the tooth and save the natural tooth.

  • The presence of bacteria in teeth and the mouth has been an accepted fact for many years. But the presence of bacteria does not constitute infection and is not necessarily a threat to a persons health. Bacteria are present in the mouth and around teeth at all times, even around teeth that have never had a cavity or other trauma. Research shows that the healthy immune system takes care of bacteria in a matter of minutes. Tooth extraction is a traumatic procedure and is known to cause a significantly higher incidence of bacteria entering the bloodstream; endodontic treatment confined to the root canal system produces much less trauma and a much lower incidence and magnitude of bacteria entering the blood stream. There is no adequate replacement for the natural tooth it should be saved whenever possible. Root canal treatment, along with appropriate restoration, is a cost effective way to treat infected teeth because it is usually less expensive than extraction and placement of an implant. In most cases, endodontic treatment allows patients to keep their natural teeth for a lifetime.

  • But what about Dr. Price? This is a good example of how the Internet can give new life to long-dispelled theories. Believe it or not, the misinformation about roots canals that is found on the Internet is still based on Dr. Prices century-old, discredited research. Dr. Prices research techniques were criticized at the time they were published, and by the early 1930s, a number of well-designed studies using more modern research techniques discredited his findings. In 1951, the Journal of the American Dental Association took the extraordinary step of publishing a special edition reviewing the scientific literature and shifted the standard of practice back to endodontic treatment for teeth with non-vital pulp in instances where the tooth could be saved. The JADA reviewed Dr. Prices research techniques from the 1920s and noted that they lacked many aspects of modern scientific research, including absence of proper control groups and induction of excessive doses of bacteria.

    Nothing can completely replace your natural tooth. An artificial tooth can sometimes cause you to avoid certain foods. Keeping your own teeth is important so that you can continue to enjoy the wide variety of foods necessary to maintain the proper nutrient balance in your diet. If your dentist recommends extraction, ask whether root canal treatment is an option.

    Endodontic treatment, along with appropriate restoration, is a cost-effective way to treat teeth with damaged pulp and is usually less expensive than extraction and placement of a bridge or an implant.

    Millions of healthy endodontically treated teeth serve patients all over the world for years and years after treatment. Those healthy teeth are helping patients chew efficiently, maintain the natural appearance of their smiles, and enhance their enjoyment of life. Through endodontic treatment, endodontists and dentists worldwide enable patients to keep their natural teeth for a lifetime.

    Autogenous bone grafts, also known as autografts, are made from your own bone, taken from somewhere else in your body. The bone is typically harvested from the chin, jaw, lower leg bone, hip, or the skull. Autogenous bone grafts are advantageous in that the graft material is live bone, meaning it contains living cellular elements that enhance bone growth.

    However, one downside to the autograft is that it requires a second procedure to harvest bone from elsewhere in the body. Depending on your condition, a second procedure may not be in your best interest.

    Allogenic bone, or allograft, is dead bone harvested from a cadaver, then processed using a freeze-dry method to extract the water via a vacuum. Unlike autogenous bone, allogenic bone cannot produce new bone on its own. Rather, it serves as a framework or scaffold over which your own bone from the surrounding bony walls can grow to fill in the defect or void.

    Xenogenic bone is derived from non-living bone of another species, usually a cow. The bone is processed at very high temperatures to avoid the potential for immune rejection and contamination. Like allogenic grafts, xenogenic grafts serve as a framework for bone from the surrounding area to grow and fill the void.

    Both allogenic and xenogenic bone grafting are advantageous in that they do not require a second procedure to harvest your own bone, as with autografts. However, because these options lack autografts bone-forming properties, bone regeneration may take longer than with autografts, with a less predictable outcome.

    This product is processed allograft bone, containing collagen, proteins, and growth factors that are extracted from the allograft bone. It is available in the form of powder, putty, chips, or as a gel that can be injected through a syringe.

    Graft composites consist of other bone graft materials and growth factors to achieve the benefits of a variety of substances. Some combinations may include: collagen/ceramic composite, which closely resembles the composition of natural bone, DBM combined with bone marrow cells, which aid in the growth of new bone, or a collagen/ceramic/autograft composite.

    Synthetic materials also have the advantage of not requiring a second procedure to harvest bone, reducing risk and pain. Each bone grafting option has its own risks and benefits. Dr. Steven Williams or Dr. Harley Williams will determine which type of bone graft material is right for you.

    Dr. Steve added me on to his very full day and was able to effectively diagnose the problem with my tooth and pinpoint where my pain was coming from. He is very thorough and gentle. He took the time to explain what the problem was and and how best to fix it. He is very professional. The front desk staff was very helpful. The office is spotless. I highly recommend this team!

    An infection of my old root canal caused excruciating pain and I needed to see a Periodontist fast. I was referred to one Periodontist by my dentist 2 weeks ago. However I could not even get a consultation appointment almost two months later. I began to research Periodontists nearby and I am so fortunate to find the office of Dr. Harley Williams and Dr. Steven Williams. I contacted the office right away and Sandi arranged the tooth extraction operation for me within days after Dr Harley carefully evaluated my X-Rays sent from my dental office. Everyone (Sandi, Jill, Liz and others) here is very friendly, professional and compassionate. During the operation, the projector on the ceiling was displaying all kinds of blooming beautiful flowers and the music was my favorite "Stand by me". I felt very relaxed and very comfortable. I also learned a fun fact: Karen worked for Dr. Harley Williams for more than 20 years! Dr. Harley Williams and her awesome assistant Karen were very gentle when preparing for the extraction. I hardly felt any pain and the tooth was pulled out with just one decisive pull by Dr Harley Williams. The post surgery care instructions are very clear. I am doing great so far with Advil, Tylenol and antibiotics. This is the best dental experience I have had since we moved to the bay area 25 years ago! Thank you Dr. Harley Williams, Karen, Jill, Liz! Special thanks to Sandi for meticulously arranging the surgery date!

    I got an emergent issue and could not find a dental clinic in Dublin and Pleasanton to walk in. I found them through Google and tried to call them at ~2:00Pm. The front desk was very nice. Though this was the first time I called them, they still managed to make me an appointment at 4:00Pm this afternoon to treat my issue even they were very busy. Dr. Harley Williams solved the issue very quickly. They really saved my day! Otherwise, I could not even dare to close my month. Thanks a lot!

    I have been a patient for several years. Both Drs. Harley and Steven Williams take the time to understand the problem and spend as much time as needed to ensure the best outcome possible. As a physician, it's always a compliment to be "the doctor's doctor" and both Drs. Harley and Steven Williams earn--and exceed--that trust over and over. For example, they recently evaluated a tooth that had a questionable 'area' that may need intervention. We looked at the films and discussed--and then they called me back (on a weekend!) to discuss further plans for future care and preventative care, as well. It's clear that they care deeply for their patients--as people, not just a number. Truly amazing doctors and people who care and give so much to our community. Proud to be their patient!

They took the time to listen and create an experience from Paperwork, X-ray to treatment plan that addressed my concerns and my needs.     Transitioning from Michelle, after so many years, was not something I looked forward to. My experience with Amy was great. She took time to explain procedures and made recommendations which makes for a good experience. Of course, front office staff always make you feel welcome and comfortable.

    The BEST place I have gone to in my life!!! I had a root canal done someplace else before coming here where I felt absolutely everything. My nerves would not numb. I had to go back after the first attempt because they had failed on numbing me. I was feeling so much pain! I tried to tell them to stop and they yelled at me. Not only that, but they also broke a file off in one of the canals of my tooth. After much research, I decided to come here! Let me tell you... you will not regret it! I was born and raised in Livermore. I have been to several dentist around town... no one compares to this one!!!! They helped me so much with my extreme anxiety especially after previous dental trauma! They explain everything so well, the front desk is so welcoming and kind, the Dr is amazing and explains everything so well!!! I felt no pain!!! None!!! No anxiety and no stress!!! I had a tooth extraction and I took a nap!!! What felt like 15-30 minutes was actually 4 hours!!! Look no further, this is the place you want!!! Trust me!!!

    I have nothing but good comments thus far. I was referred to Dr. Harley Williams by my dentist and was quite anxious about the visit. I found the staff to be professional, yet friendly. Dr. Harley took her time during the evaluation and patiently answered questions and explained options and procedures available. I discovered that she's an experienced periodontist. I like her philosophy around patient care, involvement, and patient comfort. I feel confident in my decision to have her perform the work needed . Knowledge and experience are a must... a good 'bed side manner' is a plus; all of these is what you get with Dr. Harley.

    I couldn't have been happier about my recent visit, unless of course there hadn't been a root canal involved. Dr S Williams is great about answering the questions I have about new techniques, equipment changes, and what ever comes up. During procedures, he has shown the skill and patience to do what's best for the particular tooth being worked on when met with difficulties. I go in having complete confidence that when, after getting a better look, and he tells me what the plan is, is that'll it work. The staff is great as well, they keep me informed about insurances, appointments, and my pre-meds I need to take, all while being friendly snd efficient! Very Happy!
