Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.
If you are looking for a highly trained and experienced dentist in Sandy, you have come to the right place. At our practice, you will receive the highest quality dental treatment. Our dental office uses the latest state-of-the-art equipment and cutting edge technology. We are extremely concerned about contagious diseases and take every precaution to make sure that there is no contamination occurring in our office. We meet all guidelines set forth by the ADA, OSHA and the Center for Disease Control. From the moment you walk into our office, you will receive a warm and friendly greeting from our staff. We treat all of our patients like family.
We participate with most insurance groups, including Aetna, Altius, Ameritas, Blue Cross Blue Shield, Cigna, Connection Dental, Delta Dental Direct, Dental Select Gold, Dental Select Platinum, DMBA, EMI, Guardian, Humana, Lincoln, MetLife, PEHP, Principal, TDA, and United Concordia. We will need all of your insurance information before treatment. We will verify your coverage. Please anticipate to receive statements during the time your insurance is pending. This will give you the opportunity to stay abreast of your balance. In the event that your insurance company pays more than expected and you have a credit on your account, you will be issued a refund or the credit can be used toward future treatment. Please understand your insurance policy is a contract between you and the carrier and it is your responsibility to know your insurance maximum and benefits. If you have treatment at another office, it is your responsibility to know what insurance is being used at that office. It is important for you to be involved to help assure timely payments on your account. To find out if we accept your plan, please call our office.

I've had four fillings break in my life. The first was from when I was a child and was over 20 years old. The other three happened within a few years of getting them done here. I really like the dentist, but those numbers are just not ok.

Happy with my dental experience. Dr. Ellison is professional and friendly, and explained various treatment options for condition. Staff is kind and helpful. Wait times are minimal- nothing of which to speak. We pay out of pocket, and the cost ended up being less than the initial estimate given, even after upgrading the crown material. I also appreciated we were given an estimate up front, and the self pay package offering a discount with its purchase.

I am so glad I found Dr Ellingson at Granite View Dental! The last time I went to the dentist was pre-covid and that dentist game me a treatment plan with 4 teeth needing treatment. I have never had that many cavities, so I wanted a second opinion. Due to covid l, it was 2 years later that I got that second opinion. Dr Ellington explained the treatment plan I received was all cosmetic (which the other dentist didn't) and unnecessary. I am thrilled I found Dr Ellington! And a bonus with his beautiful new office and wonderful staff.

The entire staff was professional and thorough. The hygienist did an excellent job cleaning my teeth. The dentist is very practical, establishing a baseline and a plan to watch specific areas discovered through the evaluation and cleaning process. I highly recommend them based on this first experience.

I loved the staff and their attention to detail, kindness. They are so friendly!!!! The new office is beautiful and inviting. Dr. Ellingson is amazing and thoughtful. The sweet hygienist did a fabulous job at the cleaning, I have receding gums which cause my teeth to be super sensitive, I did not get a single zing. She was super gentle and thorough. I loved the experience!!!!!

For me it is difficult to trust a dentist, but I have been very pleased with everyone at Granite View Dental. They do a wonderful job. I feel very comfortable and respected there.

I have always enjoyed going to the dentist. When I moved to Sandy I needed an new dentist and I was glad to find Dr Ellingson. I have nothing but positives to say about him and his staff. The front desk is always able to work with my schedule and get me in and the dental hygienists are the best I have ever had. I'm very lucky to only see them every 6 month.

Dr. Ellingson and his staff are excellent. Very thorough and a very nice atmosphere in their office. It's an older building but I actually like that. You know you're not wasting a ton of money on their expensive office. They're just getting your teeth done right. We moved our whole family over to their office I'm glad we did. Also they can get you in a lot faster than our last dentist, which is a big plus.
