A Rogue Valley native, Dr. Jake is happy to have returned to Medford with his family to practice after dental school. He graduated with a Bachelors Degree from Southern Oregon University, and then attended Oregon Health and Sciences University School of Dentistry to receive his DMD in 2015.He has also participated in a Sleep Dentistry Residency at UCLA Dental School. On a mission to Chile, Dr. Jake spent two years serving local people, learning their culture, and becoming fluent in Spanish.
In our office, you will often find fresh-baked bread, with homemade honey-butter, or some other baked goodie Dr. Jake has made for our patents. Outside the office, he enjoys exploring the outdoors on his mountain bike, rafting, snow skiing, and traveling with his wife and three children.
Dr. Foutz believes in helping people improve the quality of their life through dentistry. Passionate about dispelling the common anxieties around visiting the dentist, he is especially interested in helping people with sleep-related breathing disorders, eliminating dental pain, and helping create a more beautiful smile.
Dr. Foutz began college studying to become an engineer. After realizing modern engineering is mostly done on computers, he changed directions and earned an undergraduate degree in biology with a minor in chemistry. He likes to work with his hands and enjoys building things. His manual dexterity, coupled with an engineering mindset, translates into comprehensive, whole-mouth dental care where all underlying problems are addressed and resolved.
Always looking for ways to expand his learning and knowledge and stay on the cutting edge of technology, Dr. Foutz is a member of numerous dental and medical associations, including the American Dental Association, Oregon Dental Association, and Southern Oregon Dental Society. His goals are to always provide the highest standard of care, and build lasting relationships with his patients.
In Oregon, every dentist needs to take 40 hours of continuing education every two years in order to keep their license active. Dental hygienists need 24 hours every two years. In our practice, we believe in keeping up with current best practices, including rapid technological advancements, so we can provide the most up-to-date treatment modalities and recommendations.
On average, Dr. Foutz has attended around 100 hours of continuing education per year since graduating from dental school. These classes include topics from medical emergencies and sterilization practices, to advanced themes including digital scans, in office milling of crowns, to screening for and managing sleep apnea.
These continuing education classes also offer opportunities to meet specialists and grow our referral network to provide top-notch care. They offer the chance to participate in community service, like the Mission of Mercy and Freedom Day events. It also provides opportunities to talk with other local, regional, and national dentists about what techniques and materials work best for their patients.

Dr. Foutz is an excellent dentist both in his skill level and his use of state of the art technology. One can actually receive a permanent crown the same day! Everyone is friendly and accommodating. You will be so satisfied to have Dr Foutz as your dentist.

I appreciate the friendly atmosphere and the quality of service is great.. I just saw the hygienist for a cleaning and she did an excellent job... I also appreciated the fact that she didn't chat all the while cleaning my teeth so I didn't have to try and talk during the cleaning...

The Foutz family is the best!! I used to live here but moved out of state and it's been so hard to get a new dentist where I live. I knew I was coming to OR for a visit and I called trying to get an appt. I gave them my dates and they worked their magic to accommodate my trip! The staff is so friendly and sweet, the facility is clean and homie and the service is always top of the line! Thank you to everyone there!

Dr. Foutz did a procedure on my special needs daughter just a couple of days ago. It went great. I had explained my concerns with him when it came to my daughter and how she would react, and he actually listened to me. I have never had a Dr. listen to me before. My daughter is doing great after her surgery, and I feel it is due to the fact, that the Dr. listened and she wasn't stressed out. I would highly recommend Dr. Foutz and staff.

I have been going to Dr. Foutz for a few years now and I always get excellent care from my hygienist and Dr. Foutz. And the front desk ladies are also very nice and welcoming. They also give you gift cards if you refer someone and they show up. I like how Dr. Foutz doesn't start drilling holes right away as soon as he sees something he takes his time and watches how a cavity progresses before doing something drastic.

Nicest people, excellent care from the front desk to the hygienist. Was establishing care with this office and was very impressed with their thoroughness , explanations and gentle care. Dr.Foutz pointed something out in my mouth that was never mentioned by my previous dental office. I'm glad I've made the switch to this office.

Their office is great! I'm wasn't a patient of his. But will be now. They got me in on short notice for a severe toothache. Everyone was very professional and the assistant was very gentle while taking the X-rays. I highly recommend!
