Dr. Forni is a graduate of University of Florida College of Dentistry. He is a Board Certified Dentist in the state of Florida. He has been practicing in Lake Wales since 2001. He completed his undergraduate studies at the University of Central Florida with a Bachelors in Biology. Dr. Forni graduated dental school at the top of his class, receiving high honors and dean's list recognition. He is the President of the Polk County Dental Association, an active member of the Florida Dental Association, American Dental Association, West Coast Dental Association, Florida Academy of Cosmetic Dentistry and the American Dental Implant Association. Dr. Forni supersedes requirements for continuing education to offer patients the latest procedures and techniques. He has completed postdoctoral training in esthetic, cosmetic, and implant dentistry, and focuses on gentle care with function and beauty.
Dr. Pero was born an raised in Florida and identified his interest in dentistry early on. While completing his undergraduate studies at the University of Central Florida, his interest became a passion. He focused his studies in the sciences and became an active member of the Pre-Dental Student Association. Dr. Pero attended the University of Florida College of Dentistry, graduated as a top student, and his research has been published in literature. He is now the Vice President of the Polk County Dental Association and a member of both the Florida and American Dental Associations. He has completed postdoctoral training in implant dentistry. Dr. Peros compassionate demeanor and excellent skills have allowed him to participate in dental mission trips, and externships to help those in need. Locally, he enjoys rehabilitating abused and neglected boxers as a part of a foster family for Florida Boxer Rescue.
Dr. Mohammad is a Polk County native who grew up in Winter Haven, Florida! He graduated Summa Cum Laude from UCF in 2015 at the beginning of his dental journey. He earned his Doctorate of Dental Surgery at the University of Maryland School of Dentistry, graduating Magna Cum Laude and an elected member of the Nations Dental Honor Society. He completed two post-grad, advanced training programs and prides himself in exceptional patient care.
Dr. Mohammad has lived in 3 other states; Maryland, New Jersey, and Texas. Hes a huge football fan, a self proclaimed foodie, and enjoys being outdoors. His favorite collegiate team is the UCF Knights and professional team is the Tampa Bay Buccaneers. His favorite foods include Neapolitan style pizza and Philly cheese steaks. Outdoors, he enjoys going on hikes and one of his goals is to visit all 63 national parks. He also enjoys kayaking and paddle boarding at the rivers and springs around central Florida. He has a passion for dentistry and is looking forward to the transformation of the profession with digital technology. He is happy to be back in Florida, and is excited to be a part of Forni Dental serving his hometown community!

Awesome place! I was having trouble with one of my wisdom teeth and not sure about the one next to it. Not only did they take ex-rays but asked if I would like both teeth taken out now. Heck yeah! Once in my chair they took out what I thought was a wine menu ( I thought my daughter in law would love this place!) and offered serval comfort items. I'm like what the what! Is this really happening? I'm telling everyone I know about this gold nugget!

The staff are so kind! My son has a fear of going to dentists and was crying in waiting room for his first visit here, not knowing what to expect, but Even the patients were encouraging him about how he'll be happy with his experience. Once seated with Amanda, she calmed him just with her kind and patient personality, that meant a lot to me. Then Dr Pero, he is also great! My son even said "I like this dentist" mid treatment. On the way home he said he liked how Dr Pero kept explaining what was next and asked is that ok. My son left happy from a dentist for the first time! I'm so very pleased with Forni dental! Thank you very much!

Due to fear I avoided dentists for over 6 years. I decided to take advantage of the $99 deal. Who wouldn't? I was treated by Dr. Mohammad who took his time with me and made treatment almost painless. Luckily he noticed a lesion and after biopsy it was caught before becoming cancer. I just want to whole heartedly thank him and forni dental and encourage anybody needing dental treatment to go to forni dental

Be careful which doctor is doing the procedure. Not all of them have the same knowledge and experience!!! Assistants are not well prepared for the proper cross contamination protocols and often put their dirty gloves inside the clean stuff, same with some doctors that just reverse the gloves instead of disposed them (disgusting)....also.. One doctor sees you and another doctor performs the job andvyou as a patient have the right to request doctor you like the most. SADLY... They don't value you as a patient, they value your money ... you are a number for them!!! Be aware

Forni dental is awesome! I was never seen there before (new patient), and had an issue with a cracked tooth. I called 10 minutes before they closed and they were able to get me an appointment for noon the next day! Before Stephanie took me to my room she gave me a brief tour of the facility and constantly asked me if i needed anything to make my visit more comforting. After being seen, they lay out what they will be doing during your next several visits and what they estimate you will have to pay after your insurance. No hidden costs or wondering what you're gonna have to pay at the end, it's already planned out! If you're looking for a new dentist, this is the place!

Very professional dentist office. The entire staff from the time you walk in until getting whatever procedure you are there for is absolutely amazing. Walked in with tons of pain and they took great care of me. Definitely recommend this office indeed. Everyone from all the ladies to my dentist Mr.mouhamed were excellent . 5 stars indeed. I will do just about anything to keep away from the dentist. My whole body revolts at the idea of sitting back in a reclining chair. It all changed with Dr Forni. You are put at ease the minute you walk in the door. The communication never ends whether before or during any procedure. You know what's happening second to second. Need to have relaxed dentistry just ask. And checking on your comfort is constant. The attention to detail during the work is phenomenal, your input is not just welcomed - it's expected. I'm aware that there are always schedules, but you'll never feel like you're being pushed out for another patient. You won't find that level of professionalism, integrity or willingness to listen to you the patient in dental care elsewhere. Dr Forni and his team are the olympians if dental care - And they're in our area.

I've been waiting so many years for my new smile transformation I can honestly say that Dr. Forni and his staff went above and beyond to make me feel relaxed comfortable basically made me feel like myself again. There wasn't any awkward conversations no odd staring! Everyone was warm and inviting there was so much support and excitement for me to become the new me as I went through my new smile journey. This is one of many qualities that I love so much about Dr. Forni office! Now we all know when our smile is not up to par we can feel less then, ashamed as well as depressed on a daily basis. Just thinking about the embarrassment that I might experience just going into a dentist office, had terrified me it brought tears to my eyes. So believe me when I tell you calling Dr. Forni office will be your first step to beginning a new chapter in your life and I'm speaking from experience. Terrific dental office experience beginning with the scheduler named Brandy who didn't forget to call me with a missed appointment. Hygienist Dental Assistant Kelli was positive and affirming and Dr. Forni operated with care/concern and the work performed was excellent! What more can be said?

I recently relocated to FL from NY and was looking for a dental office with finance options bc I have no dental insurance. I called Forni bc they are literally at the end of my street and the price was very reasonable for a cleaning and they even did an x-ray of an area that was bothering me for free! The staff was so friendly and explained everything, even got a tour of the office. Plus the new patient gift was great! I will definitely be coming back :)
