Dr. Forni is a graduate of University of Florida College of Dentistry. He is a Board Certified Dentist in the state of Florida. He has been practicing in Lake Wales since 2001. He completed his undergraduate studies at the University of Central Florida with a Bachelors in Biology. Dr. Forni graduated dental school at the top of his class, receiving high honors and dean's list recognition. He is the President of the Polk County Dental Association, an active member of the Florida Dental Association, American Dental Association, West Coast Dental Association, Florida Academy of Cosmetic Dentistry and the American Dental Implant Association. Dr. Forni supersedes requirements for continuing education to offer patients the latest procedures and techniques. He has completed postdoctoral training in esthetic, cosmetic, and implant dentistry, and focuses on gentle care with function and beauty.

Dr. Pero was born an raised in Florida and identified his interest in dentistry early on. While completing his undergraduate studies at the University of Central Florida, his interest became a passion. He focused his studies in the sciences and became an active member of the Pre-Dental Student Association. Dr. Pero attended the University of Florida College of Dentistry, graduated as a top student, and his research has been published in literature. He is now the Vice President of the Polk County Dental Association and a member of both the Florida and American Dental Associations. He has completed postdoctoral training in implant dentistry. Dr. Peros compassionate demeanor and excellent skills have allowed him to participate in dental mission trips, and externships to help those in need. Locally, he enjoys rehabilitating abused and neglected boxers as a part of a foster family for Florida Boxer Rescue.

Dr. Mohammad is a Polk County native who grew up in Winter Haven, Florida! He graduated Summa Cum Laude from UCF in 2015 at the beginning of his dental journey. He earned his Doctorate of Dental Surgery at the University of Maryland School of Dentistry, graduating Magna Cum Laude and an elected member of the Nations Dental Honor Society. He completed two post-grad, advanced training programs and prides himself in exceptional patient care.

Dr. Mohammad has lived in 3 other states; Maryland, New Jersey, and Texas. Hes a huge football fan, a self proclaimed foodie, and enjoys being outdoors. His favorite collegiate team is the UCF Knights and professional team is the Tampa Bay Buccaneers. His favorite foods include Neapolitan style pizza and Philly cheese steaks. Outdoors, he enjoys going on hikes and one of his goals is to visit all 63 national parks. He also enjoys kayaking and paddle boarding at the rivers and springs around central Florida. He has a passion for dentistry and is looking forward to the transformation of the profession with digital technology. He is happy to be back in Florida, and is excited to be a part of Forni Dental serving his hometown community!
