Astrid Alves, DDS, MS (Prosthodontist) When you meet Dr. Astrid Alves, you will see right away her passion for dentistry. She is an expert prosthodontist, specializing in restorative dentistry, and her goal is to create a dental haven for those in Miami and Coral Gables. Dr. Alves is bilingual and welcomes patients of all ages to Forma.
Dr. Alves attended Universidad Santa Maria in Venezuela. To become an expert prosthodontist, she then completed three additional years of advanced training beyond dental school, earning her masters degree in an ADA-accredited graduate program at the University of North Carolina in Chapel Hill.
Prosthodontists are the recognized dental experts when anything needs to be replaced in your mouth. This can range from a single tooth, multiple teeth, or all teeth and gums. While general dentists can do some of these treatments, prosthodontists are the specialists dedicated to this type of care. Dr. Alves is highly trained in cosmetic dentistry, including whitening and veneers, fillings, dental implants, crowns, bridges, dentures, and treatment for temporomandibular disorders (TMJ/TMD).
Osbel Borges, DDS, PA (Oral and Maxillofacial Surgeon)Dr. Borges obtained his degree in Dental Medicine at Nova Southeastern University in Fort Lauderdale, Florida. He graduated with honors and was presented with an award in Outstanding Achievements and Performance in Oral and Maxillofacial Surgery by the American Association of Oral and Maxillofacial Surgery. He was also awarded membership to Omicron Kappa Upsilon Honor Dental Society.
His interest in medicine and surgery led Dr. Borges to the pursuit of additional training in Oral and Maxillofacial Surgery. He completed his residency training at Nova Southeastern University in south Florida, where he developed a special interest in dentoalveolar surgery, implant reconstruction, orthognathic surgery and facial trauma. He has also received extensive training in the diagnosis and management of oral cancer, and has played a key role in the surgical management of patients with anomalies of jaw relations.
Dr. Borges has hospital affiliations in Broward Health Medical Center and Memorial Regional Hospital, both level I trauma centers. He has participated in numerous advanced facial reconstruction cases and was chief of a busy head and neck cancer service, offering a wide array of treatment options to patients with head and neck malignancies.
Dr. Dania Melnick Alonso, who is simply referred to by her patients as Dr. Melnick, has always loved dentistry from a very young age. Her mother, a dentist in Coral Gables for 25 years, taught her the importance of approaching dentistry with a gentle, caring and patient-centered perspective. Dr. Melnick believes that treating each patient as if they were a member of her family is the key to her success.
Dr. Melnick attended the University of Miami, where she earned a bachelors degree in 1998. She attended Tufts University of Dental Medicine, where she graduated in 2002. After graduating, she immediately was sought out to work for an incredible practice right outside of Boston, Massachusetts. In 2005, Dr. Melnick decided it was time to return to Coral Gables to raise her family and practice dentistry.
Dr. Melnick makes sure she is on top of the latest scientific and technological trends. She has been using digital impressions in her everyday practice since 2008. Dr. Melnick states, Dentistry has never been more comfortable for our patients than it is today. Technology absolutely matters, and learning what is new in our profession will always be a top priority for me as a dentist.
Dental health is connected to a persons overall health, so it is important to know how everything is connected so a dentist can treat a patient as a whole. Dr. Melnick volunteers her time at Camillus Health as a member of the Camillus Health Clinical Quality Forum. She is involved in making sure that dental services that the clinic provides our homeless community is connecting dental health to overall health.

I was referred to Dr. Alves by another dental specialist in the area. She is competent and affable. I am a physician practicing in Coral Gables with 18 years dedicated to the high end ,cash only clientele. I am very well aware that the supporting staff can drive away patients, no matter how good treatment results you get, because the staff offers the not up to par customer experience the doctor emanates. The front desk person is always the director of first impressions. Well, at Forma she ruins the experience from the moment she starts to talk, although she makes an effort to be nice, fleeting effort though, I must say. She appears sullen, and her approach is inappropriate, unfriendly and rigid. I decided to leave without service on my last visit even though they had made me pay it in advance. I missed one appointment before and I got penalized. After that it is the accusation: "we can never find you". An outright lie. I requested to see their oral surgeon for a tongue biopsy and had to leave back to my office after 55 min waiting because he was nowhere close to finish the previous case. But for sure they emphasize the urgency for you to be on time because the surgeon rents space once a week and he needs to perceive that they keep him busy. They canceled my dental cleaning in January and when I called to reschedule...no, there is no appointment in 5 months. This is a very competitive industry. The practice lacks the maturity to tend to the highest end patients who do not mind to pay extra to get the polite, welcoming, and engaging treatment they are used to. I hope Dr. Alves replaces her "first impressions director" with someone that can ease you in the dentist's office without being irritated from the minute you walk in. I would not recommend this office to anyone seeking excellent customer service.

I have always been afraid of the dentist. But as soon as I walked into FORMA my nerves slowly went away. Valentina the dental assistant was gentle in taking my X-rays. When Dr. Alves came into the room it felt like we had known each other for the longest. She is so sweet and knowledgeable. Sisi answered all my insurance questions and helped me schedule my appointments accordingly. And of course, when I saw Dr. Borges for my extractions I had zero pain after and my appointment was amazing! Truly recommend this entire team, they make you feel like family!!!

I was a new patient, and EVERYONE in the office was so professional. Office staff was knowledgeable and very informative. The entire office was very up-to-date with modern equipment and it has such a pleasant environment. Dr Alves really loves what she does. I had a great experience and will be coming back regularly.
