Dr. Koyama has received extensive training in his specialty both at home and abroad. He obtained his DMD in 2000 from the University of Pennsylvania School of Dental Medicine. His residency in Oral and Maxillofacial Surgery was begun at the Medical University of South Carolina in Charleston and completed at Broward General Medical Center/NSU in Ft. Lauderdale, FL in 2004.

During his residency, he also gained experience in medicine, general anesthesia, general surgery, neurosurgery, ICU care, pediatric surgery, and cardiology, ER. Prior to his studies at the University of Pennsylvania, he obtained his DDS degree and had his Oral and Maxillofacial Surgery training at Tokyo Medical and Dental University, Japan. Following the completion of his residency in 2004, he served as an attending surgeon and a member of the trauma team to take care of severe facial trauma at a Level 1 trauma center in Broward County, FL. Dr. Koyama is proficient in the full scope of Oral and Maxillofacial Surgery. He has been granted full privilege at Lawnwood Regional Medical Center and is taking ER calls for the community for facial trauma.

Dr. Koyama relocated to the Treasure Coast to become the facial trauma surgeon for Lawnwood Regional Medical Center and established his own private practice to serve the community better. Dr. Koyama is Board Certified with the American Board of Oral and Maxillofacial Surgery and National Dental Board of Anesthesiology. He is a fellow of the American College of Surgeons and he has served chief of surgery for Lawnwood Regional Medical Center, Heart Institute, and Trauma Center.

The objective of his research was to understand the mechanism underlying oral carcinogenesis. He completed a four-year fellowship at Children's Hospital of Los Angeles. His findings were published in peer-reviewed journals. He is continuing his research effort with a focus on improving the surgery outcome and shortening the recovery time for patients.

As an educator, Dr. Koyama has been a full-time attending surgeon, an assistant professor for the Oral and Maxillofacial Surgery Residency program at Broward General Medical Center and NSU for three years where he has supervised the training of Oral and Maxillofacial Surgery residents. Dr. Koyamas broad superb clinic skills, combined with his scientific mind, will provide you with the highest quality service in this specialty.

Our surgical team is trained in assisting with Intravenous (IV) Sedation or outpatient General Anesthesia in our state-of-the-art office setting. Patients are continuously monitored during and after surgery.

The surgical team at Florida Institute of Oral and Maxillofacial Surgery are experienced, certified oral and maxillofacial surgical assistants, who assist in the administration of I.V. sedation and surgery. All staff are informed administrative personnel, well-versed in health and insurance policies, and are CPR, ACLS, Airway Management, HIPAA and OSHA certified.

Mom is allergic to some antibiotics so was referred here to have a tooth extraction. She called on Monday for an appt. Let VM as NO ONE EVER ANSWERS THE PHONE (called 3x). No one called back by 11 am next day, so I left a VM questioning whether they could care less. What is it about medical care in Florida? No one wants Medicare reimbursement, or no help or, as I see it, no one cares. Someone from the office called back and started arguing with me about their lackadasical devil may care attiude. Instead of calming the situation she escalated it to the point of a few choice expletives (well deserved, I might add! :-) ) He may be a good dentist but the staff sucks.

The receptionist to the doctor, they were all wonderful people. Monique was fabulous. She was very thorough and actually booked two appointments for me. The first was if another patient was unable to attend which was most likely. However, she also booked another appointment, just in case. So kind and forward thinking. Monique was correct, the other patient was unable to attend. Therefore my surgery was done within a week's time. The doctor explained well, answered my questions and then on surgery day, he reviewed everything once again. Danielle, I met on surgery day. She and Monique attended to all my needs and helped me to relax. They were both fun and kind. Dr. Koyama did his job, telling me exactly what he was just doing, before he did it! The doctor and those who work there, are phenomenal. I am grateful that I went to this doctor. I have already recommended him to two people who are in need!

During the open season on December 2021, Venassa scheduled me to see Dr. Koyama on January 31, 2022, for a tooth# 2 implant. Over the phone, we orally agreed that the consultation and x-rays for free, my wife was listening. My wife and I visited Dr. Koyama. He explained to us the procedures. We left and decided not to go back because certain x-rays is not covered by the insurace and no need for it. Dr. Kayma charged me for both D9310 and D0330 and got paid through MeitLife insurance. Dr. Koyama is dishonest, deceptive and shameful. Please do not go to see this dentist. He deserves minus stars. I

My husband was referred to this oral surgeon and the provider was completely unprofessional. He seemed unappreciative of the information I was providing as his wife, as many men request their wives to be in the room to help them remember information to give to the provider. I'm a nurse and do not openly reveal this as a means to ensure I (or my family) are treated the same as any other patient. When I asked what the referral provider noted, he said he only had the radiology report. I explained that the radiologist report does not contain the reason for the referral to your office from our ENT. He ignored my question and told my husband that he needs the CT scan disc before he can assess him, totally understandable. But when he asked if we had anymore questions I said "Yes, what are the next steps moving forward?". He quickly walked out of the room while pointing to my husband and saying "I already told him." I said, before the door closed, "Sir? Sir?" And he quietly closed the door on my face, as I was sitting next to the doorway. My husband was upset. We quickly left and as we did the nurse/medical asst, not sure as she didn't introduce herself or her role, said "Yes, we need the CT and we did find the referral documentation from the ENT". This proving my point that I asked pertinent questions that could not be answered as the provider did not review them prior to entering the room and did not appreciate me asking questions. Needless to say we will never be returning to this office and will ensure our ENT is aware of this unprofessional and misogynistic experience.

Dr. Koyama and staff are the best. The office is clean and the staff is so polite and accommodating. All my visits have been so pleasant. The doctor himself takes the time to explain the procedure clearly. Thank you to all the ladies that have worked with me. I look forward to my return visit in three months.

There is No comparison. From the Incredible Staff behind the desk, all the assistants, to Dr. Koyama. Bar none the best. Having a unique situation I am very glad we chose Florida institute. Everybody is fantastic there. I can't say thank you enough.

The office called me to come in an hour earlier. I then had to wait there for an hour, and I had to ask after 45 minutes of waiting what the hell was going on. Then they informed me due to a last minute surgery at Lakewood hospital is why he was late. So I finally take my wife in there and due to her small stature, they couldn't even take a panorama xray. So after waiting yet a little longer, the doctor, who I could barely understand, looked at the xrays sent from the dentist that recommended him. The doctor's recommendation was to schedule an appointment with a clinic 3 hours south, claiming they had the resources required to treat my wife. So this was a complete strike out - long wait, no xray, and no treatment or real solution.

Dr. Takashi Koyama has made me feel cared for at every visit. My life changed in a heartbeat when I broke my jaw trying to cannonball my son at a pool. It was a do as I say not as I do moment. Don't run by the pool...but look what I can do! Dr. Koyama is truly the best. Not just medically but personally...he cares about what he's doing and who he's doing it for. I couldn't have ask for better care.
