Five Rivers Health Centers consists of three former hospital residency clinic departments of Miami Valley Hospital and Good Samaritan Hospital Family Health Center, Medical Surgical Health Center, and Center for Womens Health.Five Rivers Health Centers was approved on April 10, 2012 to be a Federally Qualified Health Center (FQHC) Look-Alike and a FQHC on November 1, 2013.
In 2013 we expanded our group even more by adding a Pediatric practice in January, the Five Rivers Dental Center and Pharmacy in September. In August of 2015, FRHC experienced additional expansion by adding the Samaritan Homeless Clinic to our mission. Collectively, these sites provide medical and dental and behavioral health care at eight convenient Dayton locations.In 2017, the Five Rivers Greene County Health Center was added and in 2018, Star Pediatrics was added. Both of these locations are in Xenia, Ohio.
We are preparing the site at 721 Miami Chapel Road for our newest location, the Five Rivers Health Centers Edgemont Campus. We look forward to consolidating our Dental Center, Center for Women's Health, Specialty Clinic, Pediatrics and Medical Surgical Health Center. All will be under one-roof at our new location, being built now. Grand opening is expected in January, 2022.

Dentist is rude, inquired about a procedure and his answer was appalling, he told me that I would have to win the lotto to afford a procedure! Very unprofessional!

They care about their patients and have been repairing my teeth that were botched by a former dentist for 3 months now

I'm so unhappy with the service here. I was given antibiotics for decaying tooth with roots exposed, scheduled out 3 weeks with severe pain. Ive called 3 times to get an earlier date to extract the tooth but the keep giving me the run around. I'm calling aspen for emergency service and will never return to Five Star Dental in Xenia ohio again.
