Family Health Care was one of the first community health centers in the nation to develop a one-stop shopping concept for health care services. Weve been in the business of taking care of families since 1967. We offer medical, dental, laboratory, x-ray, full-service retail pharmacies, health education, and school-based Child and Adolescent Health Centers in Baldwin, Grant and White Cloud. We are also an FTCA, FQHC, and PCMH deemed facilities accredited by the AAAHC.

Our community health outreach programs include In-Home Respite, Alzheimers Education and Referral, Teen Parent Services, the Grandparents Program, Parents as Teachers, Immunization and Well-Child Care, Behavioral Health, and Med-Read.

The patient-centered essence of the Medical Home practice model is an ideal fit with the mission of quality care and patient safety that drives AAAHC. We offer two options for organizations to achieve recognition as a Medical Home: Certification and Accreditation.

Medical Home On-Site Certification evaluates a practice against AAAHC Standards for a Medical Home. As with our accreditation program, our surveyors make an on-site visit to assess, at the point of care and from a patient perspective, how your organization meets these Standards.

Think of AAAHC Medical Home accreditation as the highest achievement for primary care. It centers on a comprehensive review of your entire organization. Our peer-based process is conducted by professionals who are experienced ambulatory health care providers. During the on-site survey, we work with you collaboratively and consultatively to assure that your patients receive high-quality care consistent with AAAHC Standards for the ideal Medical Home model.

AAAHC Medical Home accreditation is a symbol of prestige that you can be truly proud of; a mark of excellence honoring the fact that you adopt best practices and comply with nationally recognized Standards of care. For your patients, it provides comforting reassurance that they will receive high-quality care when they need it.

Family Health Care is a non-profit 501(c)(3) organization and Federally Qualified Health Center (FQHC). Family Health Care is under the direction and supervision of a Board of Directors composed of community members and patients. By law, Family Health Care is required to have 51 percent of their Board of Director seats filled by its own patients. We are proud to say that we succeed that requirement. Our board members volunteer their time and energy to ensure we properly serve our communities and advance towards reaching our mission and vision as an organization.

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

What is a Community Health Center? Community Health centers are community-based and patient-directed organizations that serve populations with limited access to health care. Family Health Care is honored to be a Federally Qualified Health Center (FQHC) since 1967 as an FQHC, we are held to the highest standard of regulations under the federal government. It gives FHC pride to know that we are able to meet those standards while still serving residents across Michigan with high-quality, comprehensive, and affordable health care.

Family Health Care is proud to be recognized as a Federally Qualified Health Center (FQHC). FQHCs are present in over 8,000 rural and urban communities across America. They have a successful track record second to none in the delivery of affordable quality care to more than 20 million people. They play a significant role in lowering health care costs and producing savings for the nations health care system while providing quality health care that is accessible to all.

The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into what patients want it to be. Medical homes can lead to higher quality and lower costs, and can improve patients and providers experience of care.

I was refereed by a sister office (in the Baldwin family care system) that was not doing their due diligence with children. Children need a pediatric dentist! This office has one thank goodness. This office if I had gone here sooner, could of solved a lot of problems. Kids going to the dentist is never going to look awesome, but this office was through. If they asked you to leave the room it was to spare you the imagery. Dental work on kids never looks awesome, but reduces rates of cancer, diabetes, sepsis, ear issues. Given that kids are not comfortable with dental work it was to spare you. Yes my son cried and he protested but on the ride home he was more worried about a lego wheel than his tooth because it no longer hurt. Hands off care and good feelings from other offices are just that well wishes and best of luck, and not treatment. Yes it hurts while being worked on they want you to avoid seeing that. Also They found a problems that should of been solved by the other "hands off" offices much sooner, and got it done. These people are over-worked underpaid, and underappreciated. If you guys saw medicaid reimbursement rates you would understand these people are saints. I send my strongest appreciations to this office for doing what needed to be done to get the information, and start a treatment plan that was much needed.

Very good older Dentist, reminds me of my grandpa. I don't mind him lecturing me a bit, I prolly need it. Friendly staff, hardly any wait time.

Would never recommend this office to anyone with young children. My six-year-old daughter had her 1st appointment at this office 6 months ago and I was told it was their policy that kids are to go back for their x-rays and cleaning without their parent present I found that to be more than a little strange but with covid I assumed they were just being cautious, so my daughter put on a brave face and said she would see how it goes for her 1st ever appointment without myself or her dad there supervising. After her appointment I got very little feedback from them as to how she did or the condition of her teeth. Afterwards my daughter expressed to me how uncomfortable she had been to be alone with those people she didn't even know and was so upset she was in tears! After that I promised her I wouldn't make her do that again by herself. Her next appointment was due and I told them of her experience and asked if I would be allowed to be present at her next scheduled cleaning but I was told that covid or not I won't be allowed back and that wouldn't change regardless of how my daughter felt. Their utter disregard for my child's feelings and my right to be there with my daughter and oversee her care at a facility we had never been to before was disgusting!
