Family Health Care was one of the first community health centers in the nation to develop a one-stop shopping concept for health care services. Weve been in the business of taking care of families since 1967. We offer medical, dental, laboratory, x-ray, full-service retail pharmacies, health education, and school-based Child and Adolescent Health Centers in Baldwin, Grant and White Cloud. We are also an FTCA, FQHC, and PCMH deemed facilities accredited by the AAAHC.
Our community health outreach programs include In-Home Respite, Alzheimers Education and Referral, Teen Parent Services, the Grandparents Program, Parents as Teachers, Immunization and Well-Child Care, Behavioral Health, and Med-Read.

The patient-centered essence of the Medical Home practice model is an ideal fit with the mission of quality care and patient safety that drives AAAHC. We offer two options for organizations to achieve recognition as a Medical Home: Certification and Accreditation.
Medical Home On-Site Certification evaluates a practice against AAAHC Standards for a Medical Home. As with our accreditation program, our surveyors make an on-site visit to assess, at the point of care and from a patient perspective, how your organization meets these Standards.
Think of AAAHC Medical Home accreditation as the highest achievement for primary care. It centers on a comprehensive review of your entire organization. Our peer-based process is conducted by professionals who are experienced ambulatory health care providers. During the on-site survey, we work with you collaboratively and consultatively to assure that your patients receive high-quality care consistent with AAAHC Standards for the ideal Medical Home model.
AAAHC Medical Home accreditation is a symbol of prestige that you can be truly proud of; a mark of excellence honoring the fact that you adopt best practices and comply with nationally recognized Standards of care. For your patients, it provides comforting reassurance that they will receive high-quality care when they need it.
Family Health Care is a non-profit 501(c)(3) organization and Federally Qualified Health Center (FQHC). Family Health Care is under the direction and supervision of a Board of Directors composed of community members and patients. By law, Family Health Care is required to have 51 percent of their Board of Director seats filled by its own patients. We are proud to say that we succeed that requirement. Our board members volunteer their time and energy to ensure we properly serve our communities and advance towards reaching our mission and vision as an organization.
This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
What is a Community Health Center? Community Health centers are community-based and patient-directed organizations that serve populations with limited access to health care. Family Health Care is honored to be a Federally Qualified Health Center (FQHC) since 1967 as an FQHC, we are held to the highest standard of regulations under the federal government. It gives FHC pride to know that we are able to meet those standards while still serving residents across Michigan with high-quality, comprehensive, and affordable health care.

They have a successful track record second to none in the delivery of affordable quality care to more than 20 million people. The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into what patients want it to be. Medical homes can lead to higher quality and lower costs, and can improve patients and providers experience of care.

I was lied to here, I was told after an X-ray here that I had 14 cavities, and when they filled in two of my teeth they messed those teeth up more, and they never cleaned my teeth properly so I developed really bad tarter buildup. Plus not to mention the hygienist was super rude to me and my mother. My mother finally took me out of this dentist and placed me with Gingrich Dental Care in Big Rapids. They got really good X-rays and confirmed that I only had 8 small cavities, they fixed my fillings and got all the tarter buildup out of my teeth. Plus not to mention the dentist and hygienist actually cared about my sanity. Never coming back here again after the way I was lied to. And not to mention this place does NOT handle autistic children well, they're very snotty to the disabled.

This office is the best dental office i have ever had. They are always friendly and caring. They do a good thorough job. I had a broken tooth and they got me in right away and fixed it beautifully. when i initially started to go to them i had a lot of problems with my teeth , especially my front ones and they repaired them so i have a lovely smile now. I feel i am in good hands having this office for my dental health.

I've been going to see Dr Verdelli now for about a year. I truly couldn't be happier. No dental office or dentist is perfect, but she does an amazing job with my teeth and always explains what's going on ahead of time. I never feel pain when she's working and the rest of the staff is always super nice.

This office is the best dental office i have ever had. They are always friendly and caring. They do a good thorough job. I have been a patient of Dr Mir's for over a year. At each visit I give a specific reason or reasons for my visit and what my goals are for the particular appointment. She DOES NOT LISTEN TO HER PATIENTS!!!! She is very unapproachable and when you tell her and show her that her treatment plans are not working, she becomes "the victim" and everything is everyone else's fault. She makes false claims about her "behind the scenes activities" (as she puts it), case in point: she told me that they were fighting with my insurance company for "days" to get me scheduled for an MRI...now, insurance companies are all about records and they have only had 2 forms submitted and ZERO conversations with her office about my MRI request. An obvious lie! I was also told by Dr. Mir that "I have no control over my staff, I just work here. You should tell our office manager. If I tell them they won't do anything, but, if an angry customer complains we can get things fixed..." when O asked about appointment scheduling and procedure request delay issues. I asked before I left to talk to the Office Manager (I know who she is and saw her moments before my request) and the front desk operators were quite rude and demanded to know what about and trying to just get me to leave. They waited a few minutes and told me she wasn't there...as ai walked out, I saw her talking to them...how unprofessional and childish. So, it is up to me to fix their system issues and when you try, they won't talk to you... unbelievable

Emergency dental is very useful for dire situations. A lady dentist did an absolutely wonderful job with my wisdom tooth extraction. I'd highly recommend this location for dental services.

They milk your insurance for everything they can get out of it. Messed up my teeth more than they already were. Do yourself a favor don't go here, and do not see Dr. Mintzer. Worst dentist ever.
