was born and raised in Gadsden, AL. After finishing college at the University of Alabama in Tuscaloosa, he moved to Birmingham, AL to pursue his dental education at the UAB School of Dentistry. He graduated from the UAB School of Dentistry in 1982 and then moved back home to Gadsden to open his dental practice on South 3rd Street. In the early days, Dr. Maddox and his wife Cindy lived on one side of the office duplex while Dr. Maddox practiced dentistry on the other side. The practice as well as their family soon outgrew that arrangement, but nonetheless, the office on South 3rd Street has always been home! Dr. and Mrs. Maddox raised four daughters in Gadsden including Dr. Mary Lynn Shadix and Dr. Caroline Spruiell.

also attended the University of Alabama in Tuscaloosa for her undergraduate degree in Biology and then the UAB School of Dentistry, where she graduated with her DMD in 2017. She and her husband David moved back to Gadsden shortly after to start their family. She is excited to serve the community she grew up in and to continue building the practice her father started.

graduated from The University of Alabama in 2014 with her Bachelor of Science degree in Biology. After achieving a Master of Science degree in Biomedical and Health Sciences from UAB, she then went on to UAB School of Dentistry in 2020 to earn her DMD. Caroline joins her father, sister, and husband in both the Gadsden and Boaz offices. She enjoys cooking and spending time with her two dogs, Olive and McCoy.

was born and raised in Boaz, AL. He attended Auburn University in 2013 with a Bachelor of Science in Microbiology. He then received his Masters at Auburn in 2015 in Microbiology before spending a year in East Asia doing mission work. Following his year abroad, he attended UAB School of Dentistry, earning his DMD in 2020. He has returned to his hometown to practice dentistry with his father-in-law, wife, and sister-in-law to serve and give back to the community he loves. He enjoys playing golf and bird hunting with his dog McCoy.

I have been coming to their dentist office since I was nine years old and going to bring my kids as well. Their staff and all of the doctors have always treated me like a family. Had all my wisdom teeth pulled out at one point, no problem with pain meds. Moved to Birmingham five years ago and still coming every 6 months for the check ups. Thanks Family Dentistry Associates for your care and help.

I have used this office ever since Dr. Hawkins closed his office a few years ago. I typically had good experiences and was very impressed with Dr. Mary-Lynn Shadix. She was always very honest with me about my dental work and talked to me like I was her family. I have also had decent experiences with their office staff. However, this past week, I had a very different experience. This week was more of a dental nightmare. I do not want to go into all of the details, because that would take an hour. Dr. Shadix was not in the office, so I saw the new dentist, Dr. Spriuell. At first, she was okay, as I was relieved that she was about to pull a tooth that had been bothering me. Im thinking I probably should have reconsidered when she made a comment similar to something doesn't go as planned when pulling a tooth. I just laughed it off. Well, I can tell you by that following Wednesday there was nothing laughable about the dry socket that had developed. After my extraction, I was not given written care instructions, therefore I had improperly cared for my wound from what very little I had been told verbally. I did bring that to their attention on Wednesday when I was seen for my dry socket. I was then told it wasn't Dr. Spruiell's job to provide me with that information, but rather her assistants responsibility. Honestly, I don't care whose job it is but as a dental provider it is someone's job and responsibility should have been taking for failing to provide me with that information. So I ended up having to return the following Wednesday to have my dry socket checked. I NEVER saw the dentist and Dr. Spruiell NEVER even looked in my mouth. The dry socket was treated by the dental assistant. I cried the entire time. I was then given this brown stuff to put in the socket if it continued to bother me. I was puzzled that I didn't see Any dentist, so prior to driving off, I called the office and asked if she was in the office. I guess I wanted an explanation as to why I was not seen by her. I was then put on hold for a minute or two and then told by the OM that it was an obvious dry socket and she did not need to see me to know that. I was in the worst pain ever. The pain burned and radiated down the left side of my mouth and jaw. It even hurt to put my head on a pillow at night. My packing fell out the following morning. Luckily, my nightmare finally ended Friday afternoon, when I finally found a new dentist to see me on a Friday afternoon. The socket was repacked, and I was provided the proper antibiotics and pain medication. I really do hate that I had such a horrible experience where I once felt like family. I understand that every medical professional starts as a beginner and has to learn the ropes...but this girl has a good bit to learn including a bedside manner and empathy for those she treats. I work in the medical field as well and I would have NEVER treated someone, coming to me for help, the way I was treated by Dr. Spruiell. I do believe the gift of properly treating and caring for your patients is a gift and I felt that she was lacking.

I have seen all the dentists in this practice and they are all kind, caring professionals! I most recently saw Dr. Spruiell and was so impressed by her up to date knowledge and kind, genuine bedside manner. She was very knowledgeable and took the time to explain my entire case and answer all my questions. I look forward to seeing her in the future!

Best group of dental professionals! From making your appointment and checking in, to cleaning and checkup - caring, careful and thorough. Dr. Spruiell took care of me during my last appointment, and fixed a small chip in my tooth that had been bothering me for weeks. Took no time at all and you can't even tell where the chip was. Try them out, you won't be sorry!

Going to the dentist is not on my list of favorite activities, but I actually look forward to going to my regular 6-month appointments at Family Dentistry Associates! The entire staff from the front desk to the hygienists and dentists are all so friendly and outgoing; they make me feel like family. I was a little nervous to be seen by someone new this summer, but Dr. Caroline Spruiell put me at ease immediately. She treated me like a friend, not just a patient. She also saw my husband later and he was equally impressed. Our need for a quick appointment brought us to her care, but her kindness and excellent bedside manner will keep us coming back! I will be recommending Dr. Caroline Spruiell to all my friends and family.

I'm normally not one to leave reviews but really felt compelled to do so after my experience here. The office is clean, and every one of the staff members were so kind and helpful. I got to see Dr. Spruiell and she really had a unique way of both having confidence in her words and actions as the doctor, but also having the kindness and beside manner that made me feel like she really took the time to understand why I was there and listen to me. I normally fear the dentist and these visits but between Dr. Spruiell and that office staff, it actually made it a pleasant experience. I will definitely be recommending people to come here.

This is a wonderful practice with an even better staff. I have never met a kinder more compassionate and honest dentist than Dr. Spriuell. She is honest about treatment and you can tell she really cares about her patients. Dr. Spriuell's welcoming and sweet demeanor made me feel right at home and to top it off she's great at what she does. She actually listens to patient's too!
