Stephen F. Austin Community Health Network, dba Community Health Network (CHN), was established in 2008 to provide accessible and affordable healthcare. That year, the Center had one primary care site in Alvin, Texas, with one provider and three exam rooms. Since that time, the Center has grown substantially, adding pediatrics, pharmacy, a dental department, womens health care, behavioral health counselors, and psychiatry.
The values of the Center are quality care at reasonable costs, education to promote healthy living, access to needed care, transparency in all aspects of our operations, competent and empathetic staff, high professional standards and commitment to advances in healthcare.
Stephen F. Austin Community Health Center, Inc., dba Community Health Network (CHN), receives HHS funding and has Federal Public Health Service (PHS) deemed status through the Federal Tort Claims Act (FTCA).
As such, CHN is granted medical malpractice liability protection through the federal government acting as its primary insurer. As a deemed health center, CHN and its employees, working within the approved scope of project are considered federal employees and are immune from lawsuits for medical malpractice.
CHN accepts most private insurances, Medicaid, Medicare, and CHIP. Uninsured individuals may be eligible for discounted self-pay services based on their family size and household income. CHN strives to provide the highest value of care and therefore offers substantial benefits for people who are uninsured and self-pay.
We have controlled the cost of office visits, and we have kept lab and pharmacy costs far below other providers. Low-income, uninsured patients may apply for discounts to the cost of their care. CHN does not turn away anyone away due to an inability to pay.
In short, CHIP is free or low-cost health insurance for children who live in households that generate too much income to be eligible for Medicaid. While these families make too much to qualify for Medicaid, they often cant afford private coverage. CHIP offers these families a variety of coverage options for little to no cost depending on the familys eligibility.

Print out the application. Send your application back to HHSC with the items they need to prove your income (paycheck stubs) and receipts showing what you pay for childcare, child support, or care for someone with a disability.
You can use a pre-paid envelope or fax everything to 1.877.447. 2839. It can take up to 45 days for The HHSC to review your application. After that, you can call 211 to check the status of your application.
CHIP does not cover long-term services and support. However, if you have a child with a disability, the Medicaid Buy-In for Children program may be able to provide you with coverage. This program allows families with children with disabilities to make monthly payments and be covered by Medicaid.
The Childrens Health Insurance Program (CHIP) is a program for children in families who earn too much to qualify for Medicaid but dont earn enough to afford private health insurance. CHIP is mainly funded by the federal government but is often disseminated through the state. CHIP was created to ensure that all children have access to healthcare.
