In the mid-1980s a group of local residents joined together, concerned with the lack of quality and affordable healthcare providers in New Square, New York and the surrounding area. At the time, New Square was federally designated as a Medically Underserved Area, with residents forced to seek care outside of the community and often at great distances. In response, local residents began the grassroots movement that culminated in the founding of Refuah Health Center in 1992. We opened our doors the following year in 1993 with a single mission.
In 2002, Refuah applied for and received section 330(e) Community Health Center funding. As the areas leading Federally-Qualified Community Health Center (FQHC), we provide comprehensive safety-net services across the Hudson Valley.
Over the past 25 years we have grown exponentially into a full-service, integrated, multi-specialty healthcare organization with four service sites and a fleet of mobile medical units. At Refuah we go beyond treating our patients; we strive to keep our community healthy by providing gold standard diagnostics, as well as patient education and support. The needs of our Members are our first priority.
To that end, Refuah employs a patient-centered model of care focused on increasing access and enhancing Members experience. Refuah employs an integrated team of leading clinicians in adult and pediatric primary care, enhanced by a variety of medical specialties. Many of our doctors are nationally-renown, and are often consulted by outside providers for their expertise, or cited in professional literature.
In addition, we boast a large and highly regarded comprehensive dental program, as well as a fully-staffed and reputable behavioral and mental health division that is integrated into our primary care program. We like to offer our Members a one-stop-shop. You can come to us for your medical, mental and dental health. We also offer diagnostic radiology and laboratory services on-site, as well as a pharmacy, wellness center, and patient education outreach sessions.
RefuahHealth is designated by the US Department of Health and Human Services as a Federally-Qualified Health Center, is an FTCA (Federal Tort Claims Act) Deemed facilityand is recognized by the National Community for Quality Assurance for its innovative and successful Diabetes program as well as having achieved the highest level, Level 3, Patient-Centered Medical Home designations for all service sites.
Refuah was selected to serve as the lead agency of one of the 25 Performing Provider Systems across New York State under the Delivery System Reform Incentive Payment (DSRIP) program. Refuah Community Health Collaborative (RefuahCHC) is the only system led by a federally qualified health center.

Refuah Health Center is the lead agency of the Refuah Community Health Collaborative (RefuahCHC), the only Performing Provider System (PPS) led by a Federally-Qualified Health Center (FQHC). RefuahCHC partner organizations provide access to high-quality care and services across Rockland & Orange Counties. Though one of the smallest, RefuahCHC is the highest performing PPS in New York State. RefuahCHC is population-focused and committed to serving the individual and improving the community. Hands-on leadership and PPS partners dedication and effort ensures continued success.
Refuah Health Center is aPerforming Provider System under the States Delivery System Reform Incentive Payment(DSRIP) program, with a strong commitment to primary care transformation. Refuah has been aregional leader working with other DSRIP providers and local county services for enhancedcollaboration for behavioral health integration with primary care and for crisis stabilizationservices. Continuing the health care services provided by Refuah will assist the community andthe individuals served by providing primary care services to bridge gaps in the safety net, andfurther the goals of primary care transformation under the DSRIP program.
To achieve these goals, the program supported the creation of 25 regional, integrated Performing Provider Systems (PPS). The PPSs are tasked with developing and implementing innovative strategies to improve care and reduce costs for Medicaid populations.
