At Holyoke Health, we serve more than 24,000 patients a year from communities throughout the Pioneer Valley. We have a committed staff of 300, including 30 full time medical and dental providers, 30 nurses, 50 bilingual medical and dental assistants, case managers, outreach workers, administrators, and financial counselors. We also have Community Health Workers who are available to answer questions outside of a doctors visit.
Because we are a federally funded non-profit community health center, Holyoke Health never turns a patient seeking care away due to inability to pay; we offer a sliding fee scale for patients who do not have insurance and a Managed Care Department who assist people with enrolling on a health insurance plan and with Masshealth. With over 350 employees, including medical and dental providers, pharmacists, nurses, medical and dental assistants, case managers, community outreach workers, financial counselors, and more, HHC serves over 20,000 patients annually. HHC has an on-site pharmacy at each location in Holyoke and Chicopee. The pharmacies are staffed with 14 full-time Pharmacists, Pharmacy Residents, 1 Adjunct Pharmacy Professor, 11 Certified Technicians, 10 Clerks and 2 Community Health Workers.
With a pharmacy location in Holyoke and in Chicopee, HHC provides pharmacy services to all patients in the community; 340B access for patients with HHC primary care and retail access for patients with primary care services outside of the HHC medical home. The on-site community pharmacy, fills approximately 6,000 prescriptions per week, is owned and operated by HHC and is an integral part of all services offered in the HHC medical home. The pharmacies accept many insurances including MassHealth, MCOs, Medicare, CCA, MedicareD and Health Safety Net. There are insurance enrollmentservices available within the pharmacy in order to assist patients with their insurance needs. Our pharmacy also provides clinical pharmacy services, where pharmacists are integrated into our provider team. We provide direct patient care, including utilization of CDTM agreements.
