CCHC is a leader in care coordination, with programs that provide services across an integrated spectrum of areas including Health & Prevention; Education & Life Skills; Housing & Shelter, and Intervention & Support. We currently operate seven sites throughout the Chicagoland area, including three community health centers,as well as a mobile health van providing a wide range of primary medical care and social services.
CCHC continues to expand the growing needs of the public, not only by providing quality health care and services but also by sparking economic development and revitalization of the neighborhoods we serve.
