Primary Care Clinics are required to submit an Annual Utilization Report of Primary Care Clinics by February 15th each year for the prior calendar year. Family HealthCare Network is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). We offer services without regard to religion, race, color, national origin, disability, age, sex, and number of pregnancies, and do not impose a duration residence requirement, nor do we require that the client be referred by a physician.

Since 1976, FHCN has stood by its mission to provide quality health care to all. We have the honor of serving 329,370 patients in our service area.
