Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.
Michael Katzap DDS graduated with a DDS 17 years ago. After completing four years of hospital training and a comprehensive fellowship in the placement and restoration of dental implants, Dr. Katzap opened his practice in Rego Park focusing on comprehensive cosmetic restoration of smiles.
My practice is called Paramount Dentistry. It is because the patient's oral health is of paramount importance to me. It is the reason I spent four years in hospital training BEYOND dental school, and have over 6000 hours of continuing education. We strive to provide complete oral health care to our patient in one location and in a comfortable setting. We offer from simple to complex, and innovative treatment approaches. We participate with major PPO insurance plans and even try to use medical insurance to assist our patients in obtaining benefits for their needed treatment.

Dr. Katzap is an amazing dentist! I was always so scared to go to a dentist but with dr. Katzap I feel safe and the fears I had are gone! He is professional, genuine and caring for his patients! All the stuff are nice and professional too, all that makes the dentist visit a pleasant experience!

I have been to different dentists throughout the years and I'm very fearful of sitting in the dental chair. I contemplated for a while before I decided to make an appointment with Dr. Michael Katzap who I know for many years after we did a huge project for him. I had my first visit and procedure done yesterday (5/19/22), and I'm super happy that I made the decision to go there. The moment I arrived you can feel the welcoming atmosphere, very courteous and friendly that makes you feel comfortable right away. I felt no pain during the procedure, it was fast, easy and well done. I can clearly say that Dr. Michael Katzap is very knowledgeable. The best so far.

Dr Katzap has been a great dentist. I have a severe phobia to dental work based off a past intense traumatic experience and he has been patient, kind and understanding. He has helped me understand the work I am about to get done or need to in the future and answers any questions I have. There is never a long wait in his office and it's a very clean location. My mom has been seeing him for years and also praises him. And my husband now too is his patient and is quite happy.

I can't believe I've been going to Dr. Katzap for over 10 years! He's the only doctor on my entire health roster who has ALWAYS made me feel at home whenever I go for any dental appointment. Over the years, my insurances have definitely fluctuated but I would and currently pay out-of-pocket because his work is just that valuable to me. He has always been patient and very thorough in explaining every procedure to keep my my mind at ease. I trust his work wholeheartedly and appreciate all the years of professionalism throughout my chipped teeth, countless fillings, minor extractions, a root canal, a permanent crown and next, a possible implant. In addition to having a really great, sarcastic sense of humor, Dr. Katzap sometimes has goodie bags filled with dental care products to continue oral hygiene at home. As much as I miss that bowl of flavored chapsticks, my smile is always a little bit brighter because of my dentist appointment. Thank you for being such a great person.

Great experience. I went there for dental cleaning. The hygienist spent a good amount of time to carefully clean my teeth. The dentist was very nice and honest. They were both very patient to answer all my questions.

Professional and effectives assessment of work that was needed. Flawless execution of all the dental work I needed in a timely manor. Works well with insurance companies for coverage. I have been a patient of Dr. Katzap for many years. He's an excellent doctor! He's very knowledgeable, innovative, always has many options to a problem, explains well, and most importantly his hands have almost a magic touch. If I have a problem or a concern, I never doubt him, and on a contrary, I trust him 100% (he has performed rather complicated surgeries for me as well as preventative care). Dr. Katzap manages to put my mind at ease while I'm in his chair, he's very personable, and efficient. He truly cares about his patients, and always follows up after visits. He's ahead of his times and I'd recommend him to my family, friends, and anyone I care about. His office staff are very helpful and experienced. I have always had a positive experience!

I just had my first time back at the dentist's office in many years and I was very nervous. Elizabeth did the most gentle (but still thorough) cleaning I've ever received and kept checking on me to make sure I was OK, which I appreciated. Dr. Katzap is down to earth and explained everything well. I definitely trust this team with my teeth and just one session with them has pretty much defeated my dentist phobia, so thank you for that!
