Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.
If you are looking for a highly trained and experienced dentist in Jacksonville, you have come to the right place. At our practice, you will receive the highest quality dental care. Our dental office uses the latest state-of-the-art equipment and cutting edge technology and we uphold the strictest sterilization techniques. We know that many people may feel anxious about coming to the dentist, so it is our goal to make your visit with us as pain and anxiety free as possible. We view it as our mission to educate our patients about all of their oral health care options and to help guide them to choose a treatment plan that is most suitable and appropriate for their needs.
We accept Mastercard and Visa credit cards as well as cash and personal checks. We accept many insurance plans including all PPO plans. We are contracted in network with Cigna Radius, Assurant, United Concordia, DELTA Premiere, GEHA, BCBS of Florida, and Guardian. In addition, we accept financing options through CareCredit. Receptionist very rude. Thinks that it is ok for me to use the pen and pad she handed to me without cleaning it. When asked for sanitizer, she avoided to let me pump the sanitizer bottle. I've been vaccinated for over two months. I also, wear my mask regardless. Filled up requested form. At my appointment time she told me that we will do only x-ray today!!! I have waited for this appointment for over two weeks and made it clear that I am interested in cleaning my teeth and I repeatedly asked if you honor the $59 flyer I have. The answer was yes. Obviously, they did NOT honor their word. Also, receptionist did not care to correct the situations. I'm hoping that they destroyed my driver's license copies!! Because I forgot to ask them to destroy it.... and I'm concern.

Dr. Chiafair and his staff have always gone above and beyond to accommodate my cosmetic dental needs. I would consider myself a pain but they never make me feel that way. I recently lost my father and Dr. Chiafair went above and beyond to bring me laughter while working on my smile. I have known Dr. Chiafair and his awesome office manager Renee for years and hope to see them many more. I love my smile and owe it all to the great care of my favorite dentist!!

I'm happy I have found this office.I went to put braces on and I can say Dr. Joseph Chiafair is very nice and very professional and makes you feel very comfortable. Very friendly staff and very helpful. It was also easy to set up an appointment and get my teeth taken care of sooner rather than later! I would and do recommend them to my friends. I sincerely recommend!

I had a very negative experience with this dentist. First, the equipment is old. The premises do not appear very clean. During one visit, there were apparent remains in the toilet. Dr. Chiafair was not particularly friendly. He had me waiting for almost 20 minutes for a filling. When he arrived he performed the filling in a rather gruff, abrasive, rather than gentle manner. Shortly afterwards, something felt strange with with my tooth. Due to my schedule, I was unable to address it immediately, but later brought the problem to his attention and he insisted on charging me a full fee to re-do work already performed by him previously. Needless to say, I wasn't too happy about that. Also, one of his top rated dental hygienist left for more permanent work at another practice, and the new replacement (a recent graduate) did not perform my cleaning to the same standard. In short, beware.
