Community Health Systems, Inc. is a non-profit 501 (c) (3) 330 HRSA Grantee with Federally Qualified Health Center (FQHC) status. Established from the roots of Bloomington Community Health Center in Bloomington, CHSI has grown with community health centers in the counties of Riverside, San Bernardino, and San Diego. These centers have been developed in accordance with standards established for safety net providers by the U.S. Department of Health and Human Services (HHS), the Health Resources Services Administration (HRSA), the Public Health Service (PHS), and the Bureau of Primary Health Care (BPHC). The mission of CHSI is not to compete with other clinics that provide healthcare services, but to serve the indigent population and meet their healthcare needs. CHSI is a Health Center Program grantee under 42 U.S.C. 254b, a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n), and therefore receives Malpractice insurance coverage under the Federal Torts Claim Act (FTCA)

Our PCMH certification means that our health centers have continued to put our patients at the center of health services by connecting them with our expert providers and care teams. Using coordinated care and ongoing quality improvement, our model has enhanced the lives of our patients. Our resilient care teams work to recognize and support patients and their unique needs by approaching their care through a lens of coordinated care

We offer a Sliding-Fee-Scale (SFS) to individuals who cannot afford or do not qualify for healthcare coverage. Our medical, dental, women’s health, behavioral health, chiropractic and vision care services are all available through the SFS program, which is based on Income and Family Size

Adult Medical, Pediatric Services, Women’s Health, Prenatal Care, Senior Medical, Dental Care, Behavioral Health, Chiropractic Care, and Vision Care are all available throughout San Bernardino, Riverside, and San Diego Counties. Services will vary by location

The first step in enrolling a new patient at CHSI will vary, depending if you have insurance or not. Some patients may need to change their assigned doctor with their insurance, or some patients may need to enroll into a health program such as Medi-Cal, which could create a longer enrollment process with our Patient Services Representatives

Federally Qualified Health Centers are community-based health care providers that receive funds from the HRSA Health Center Program to provide primary care services in under-served areas. Federally Qualified Health Centers may be known as Community Health Centers, Migrant Heath Center, Health Care for the Homeless, and Health Centers for Residents of Public Housing
