Chad C. Street DMD, MD was born and raised in Pikeville, Kentucky. He returned home after completing Medical School and Oral and Maxillofacial Surgery training at theUniversity of Kentuckyto open East Kentucky Oral and Maxillofacial Surgery.

Dr. Street has a specialty license in Oral and Maxillofacial Surgery to practice in the state of Kentucky. Dr. Street completed his dental, medical, and oral and maxillofacial surgery training at theUniversity of Kentucky.Dr. Street's undergraduate training was completed atPikeville College.
