The primary care medical home is accountable for meeting the large majority of each patients physical and mental health care needs, including prevention and wellness, acute care, and chronic care. Providing comprehensive care requires a team of care providers. This team might include physicians, advanced practice nurses, physician assistants, nurses, pharmacists, nutritionists, social workers, educators, and care coordinators. Although some medical home practices may bring together large and diverse teams of care providers to meet the needs of their patients, many others, including smaller practices, will build virtual teams linking themselves and their patients to providers and services in their communities.

The primary care medical home coordinates care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports. Such coordination is particularly critical during transitions between sites of care, such as when patients are being discharged from the hospital. Medical home practices also excel at building clear and open communication among patients and families, the medical home, and members of the broader care team.

The primary care medical home demonstrates a commitment to quality and quality improvement by ongoing engagement in activities such as using evidence-based medicine and clinical decision-support tools to guide shared decision making with patients and families, engaging in performance measurement and improvement, measuring and responding to patient experiences and patient satisfaction, and practicing population health management. Sharing robust quality, safety data and improvement activities publicly is also an important marker of a system-level commitment to quality.

The primary care medical home provides health care that is relationship-based with an orientation toward the whole person. Partnering with patients and their families requires understanding and respecting each patients unique needs, culture, values, and preferences. The medical home practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core members of the care team, medical home practices ensure that they are fully informed partners in establishing care plans.

The primary care medical home delivers accessible services with shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the care team, and alternative methods of communication such as email and telephone care. The medical home practice is responsive to patients preferences regarding access.

Sonya (Makah) has been IHCs CEO since 2014. She holds a Bachelors in Business Management and an MBA in Finance and Management. She is a U. S. Army Veteran, serving as a U. S. Paratrooper (Airborne) and deployed several times. Prior to working with IHC, she served as the CEO of the Lower Elwha Klallam Tribe for eight years. She is proud to work with the IHC team to serve all of the members, and enjoys finding new and innovative ways to support the community. Richard Grooms earned a Bachelors Degree in Accounting and an MBA in Healthcare Management. He has been in accounting for more than 25 years and in Healthcare for the last 12. He found his calling when he was offered a controller position in an FQHC in Wisconsin. Like everyone at IHC, Richard is dedicated to increasing access to care and removing barriers to wellness. As a senior leader he works across departments to achieve the most efficient and cost-effective way to provide and collect for services.

Pablo joined IHCs Board in August 2005. He is a master artist and craftsman and a graduate of San Jose State University. He holds masters and bachelors degrees from San Jose State University, and has a 35 year record of teaching in the Santa Clara County office of the Education Alternative School. Now retired, he is very involved in the 500 mile run, which he has actively supported for the last 34 years. For the run, he is a spiritual leader and caretaker of the drum. He is a supporter of the Bear dance and Sun Dance and other ceremonial events in the San Jose area, and is nationally and regionally recognized for his visual art work. He has three children, all of whom are adults, and currently has 3 beautiful grandchildren.

Charlene joined the IHC Board in 2020. She was born and raised in San Jose. She is a descendant of the Ohlone people. She is a strong community advocate and community volunteer. She is honored to help support the work of IHCSV. Charlene offers extensive board experience, having served on a number of non-profit boards and committees in both California and Minnesota. In terms of professional experience, Charlene has served as an Adjunct Professor teaching Ethnic Studies. She has also held several positions in corporate legal departments negotiating contracts, addressing issues of compliance and matters of privacy. As a life-long learner, Charlene has earned a Bachelors degree in Human Service Administration, from San Jose State University, a M.A. in Mexican-American Studies from San Jose State University, and a Juris Doctorate from Mitchell Hamline School of Law where she studied Indian Law and Health Law.

Paulette Thomas Pico (Navajo Nation) Dzitahnii (Mountain Cove Clan) and born for Naashtzh Dine (Zuni Clan), Maternal Grandfathers clan Ts Nahabinii (Sleep-Rock People Clan) and Paternal Grandfathers clan Taheedlinii (Water-Flows-Together Clan). Paulette is carrying on the family tradition by working as a Principal System Engineer with Roche Molecular Systems. She is a Sequoyah Fellow of the American Indian Science and Engineering Society (AISES) and a member of the Society for Advancement of Chicanos/Hispanics and Native Americans in Science (SACNAS). Paulette and her family actively participate in community events which promote diversity and cultural awareness such as Interfaith events, school events, American Indian gatherings while being a parent to three. Paulette joined the board in July of 2016 and proudly serves as the Secretary.

She is a former Special Education Paraeducator for Santa Clara County Office of Education, where she worked for 15- years. Since 2006, Lisa has worked in the capacity of Senior Citizen Care Provider for In Home Supportive Services of Santa Clara County. As a student, Lisa studied medicine and the pharmaceuticals industry. She enjoys creative art and Mother Nature. Lisa looks forward to her future endeavors as an active IHCSCV Board Member, strengthening the Circle for Health, Balance and Wellness.

She is a member of the IHCs Diabetes Community Advisory Committee (DCAC) and has been involved with the Committees efforts to reach out and educate the community about diabetes. She is also a Patient Advocate in the Community Health Partnerships Patient Advocacy Program that trains clients how to speak to legislators and other officials about the need for health care services.
