If you received your COVID-19 vaccination from Premier Health, getting your proof of vaccination is simple. The fastest way to get this information is by using your Premier Health MyChart account. Using your MyChart account means you do not need to fill out any paperwork before gaining access to your vaccine record.

We now offer a QR code option for sharing your vaccination records. Within your COVID-19 vaccination record, select the green button labeled QR codes. This will supply a QR code that can be scanned at participating locations to share your COVID-19 information.

If you would like to print your COVID-19 vaccination record, please use the website version of MyChart. Follow the instructions above. Once you see your vaccine information, click the printer icon at the top right of your screen. Printing may not be possible from a mobile device.

Premier Health has earned Magnet recognition as a system from the American Nurses Credentialing Center. We are one of only 20 health care systems in the U.S. and only the second in Ohio to receive professional nursings highest honor. Research shows that Magnet-designated hospitals provide a safer environment with better outcomes for patients, such as fewer complications and injuries, lower mortality rates, and shorter stays.

As part of Premier Healths first-ever system Magnet designation, Miami Valley Hospital has achieved its fourth Magnet designation since 2004. Premier Health is the first health system in Ohio and second in the nation to achieve system-wide Geriatric Emergency Department Accreditation (GEDA) in June 2020. This distinction from the American College of Emergency Physicians (ACEP) recognizes emergency departments that provide excellent care for older adults.

The bronze standard Level 3 Geriatric Emergency Department Accreditation applies to Atrium Medical Center in Middletown (which was accredited in October 2019), Miami Valley Hospital in Dayton, Miami Valley Hospital Austin Boulevard Emergency Center in Miamisburg, Miami Valley Hospital Jamestown Emergency Center in Jamestown, Miami Valley Hospital North in Englewood, Miami Valley Hospital South in Centerville, and Upper Valley Medical Center in Troy.

Mary Boosalis, president and CEO, Premier Health, was recognized by Modern Healthcare as one of its 2019 Top 25 Women Leaders. The highly selective awards program, which recognizes female health care executives from across the country every two years, acknowledges executives who are influencing policy and care delivery models nationwide. In doing so, it highlights the continued need to nurture diversity in health care leadership.

The award recognized Boosalis leadership in delivering health care in innovative ways, including positioning Premier Health as the first health system in the Dayton region to offer care virtually, whereby people can access providers via video or phone chat 24 hours a day, seven days a week. Premier Health employees Marc Belcastro II, Holly Walsh, and MiLinda Zabramba, MD, were awarded the Dayton Business Journal's 40 Under 40 Award for 2021, which is determined largely by community service and professional accomplishments.
