Also, provide us with your insurance card if available. You can alternatively give us the phone number, group name, and subscriber number in the absence of your insurance card. These are the details needed to ensure speedy processing of your insurance claim.
If you have any other health condition for which you are currently taking medication, it is important to let us know. These conditions and drugs include high blood pressure, diabetes, rheumatic fever, aspirin, heart drugs etc.
We request that you send the X-ray results of tests done by your physician, if any. These X-rays form a vital part in giving you a proper diagnosis. You can also send these results to our email address if you used digital radiography for the X-ray. We can then produce any number of copies as needed.
Kim joined our staff in 2013; bringing with her much experience, a positive attitude, and a friendly and out-going personality. Her ultimate goal is to assure that each and every patient has the best possible experience while in our office. Kim challenges herself on a day to day basis to learn something new to better herself professionally and personally. When Kim is not at work, she enjoys spending time with her daughter, family, and friends, exercise, and anything that contains being outdoors in the summer.

Nikki has been with Dentkos Endodontics for over 3 years. She is currently attending college for Electroneuro diagnostics. Nikki enjoys reading, hiking, traveling, and spending time with friends and family.
Renee joined our team in 2018. She brings with her 34 years of experience, earning her CDA in 1989. Renee is a dedicated employee with a kind heart, and fantastic sense of humor. She is passionate about educating patients, making them feel relaxed, and comfortable while in our office.
Renee lives in Granger, OH with her husband Mike and two adult sons. Outside of the office, she enjoys spending time with family and friends, various outdoor activities including volunteer work, going for walks, and gardening.
Nicole has been working as a chairside assistant since she graduated from the Cleveland Institute of Dental and Medical Assistant in 2008. Usually found working alongside the dentist, Nicole helps with the procedures and treatments youll receive at our office. Nicole is a dedicated individual who looks forward to building a relationship with you and your family. She believes that going to work every day holds something new and is another opportunity to ensure patients feel comfortable and leave happy.

I just wanna say how amazing this place is! Dr. Dentkos did so good on my root canal! His dental assistants I can not express how nice they were to me. Eased my anxiety. Explained everything in detail. It was so quick n easy and painless! If I could I would give them 10 stars! Thank you guys so much!

Was given Novocain and my jaw locked shut. Had to go back a week later for the same procedure. They took the same x rays from the week before and now I'm being charged for services not rendered. Very disappointed. I need to call the office but can't because they close at 4. I work til 4. When do they expect me to call? I have students until 4! Would not go back at all. Very unprofessional and unkind.

This guy & his staff were amazing! They're very friendly. I had 2 root canals done in one month- didn't even feel like I had one done the next day after both procedures. I recommend this place to anyone & everyone

Had my 1st ever root canal done today at Dentkos Endodontics .. the staff was great .. everything was explained to me on what was going to happen .. and Dr Dentkos was wonderful .. I highly recommend Dentkos Endodontics .. 10/10

This is the best dental office I've ever been to! I had a horrible experience with a different dentist when I tried to get a root canal, and paired with my horrible anxiety, I was really afraid to try to go back to a different dentist to get it finished. But Dr. Dentkos, his assistant, and the receptionist were AMAZING! They are so kind and patient and wonderful at what they do. I've never felt more comfortable and cared for during a dental procedure. I can't say enough about how great they were. 10/10 recommend if you need a root canal or any other endodontic procedure!

We came to Dr. Dentkos for a second opinion on our 8 year old. The previous facility we went to suggested a very aggressive plan. Dr. Dentkos went over our son's situation and looked at his teeth explaining everything thoroughly. Everyone in the office was welcoming and friendly. We ended up not needing to do any procedures at all! I am so happy that we went with this office and wouldn't go anywhere else.

Dr. Dentkos went over our son's situation and looked at his teeth explaining everything thoroughly. Everyone in the office was welcoming and friendly. Dr. Dentkos and his staff are absolutely wonderful. The prospect of needing a root canal can cause a bit of anxiety.
