At Cano Health, our innovative analytical and care management platform (CanoPanorama) is used to identify care opportunities and deliver better outcomes for our patients. In our relentless pursuit of a healthier patient population, we utilize our proprietary population health management platform, CanoPanorama, to improve our patient care and clinical outcomes.
Risk stratified members are identified for Population Health Program participation, and our care management module is utilized for Care Planning intervention and activities through our care management platform.
To demonstrate our commitment to provide the highest quality patient care, Cano Health has implemented a 247 medical urgency line as part of our Cano At Home program. Through our Cano At Home program, we provide home visits to existing patients who have medical urgencies. Our team of health care practitioners is also dedicated to providing home visits for our existing patients with high-risk, complex cases that are enrolled in our care management program.
Cano Healths TEACH Program (Telehealth After COPDHospitalization), is designed to incorporate both home visits and telehealth, as part of our in-home management of health status for patients with Chronic Obstructive Pulmonary Disorder. Satisfaction with the care a patient is receiving is important in maintaining motivation and treatment compliance. A blend of telehealth and home visits can provide an opportunity to improve patient satisfaction as well as quality of life while potentially reducing readmission costs for patients with COPD. As soon as an eligible patient is discharged from the hospital and enrolled in Cano HealthsTEACH Program, the patient will receive home visits alternated with telehealth monitoring.
The primary goal of our care management team, in collaboration with our primary care physicians, is to improve the overall health and quality of life for our patients through the early identification of needs and coordination of care and services for those patients identified as complex or high-risk.

Called to make appointment. They asked what insurance i have. When I tell them name of my insurance, they said, Oh we don't want to take bew patients with that insurance. I wish I could give them 0 star

My appointment has been scheduled wrong TWICE And it has ALSO been canceled TWICE. mind you I was going in as a new patient. Extremely unprofessional. I will be going to another dentist that takes their patients seriously and values their time. I will not recommend this place to no one!

would ANY be kind enough to chime in on the performance and knowledge of the dentist? I personally would appreciate the effort of shared experience. The additional experience of the front staff is an added bonus. Though the greater weight I am seeking is of the dentist. I have extensive needs, years of not having an ability to tend to the NEEDS now I'm in major despair, desperate & terrified! Enduring many negatives this has created is just horrible; declining health of many sorts bone loss, heart, lungs, blood its really scary! Plus sadly people DO make judgements on ones oral health i.e. appearance at which I UNDERSTAND this I too am human Its just a fact. Its humiliating! I understand this so well NOW! BETTER LATE THAN NEVER! I have made changes myself and I hope TO help others see from a new perspective too. All changes begin with SELF and I personally am trying, I welcome you to join! Not only change of self, MORESO the change needs to be made in our health care system. Dental along with any and all parts of body should NEVER be separated from our MEDICAL care! Medical INSURANCE should cover our whole entire being!!. These separate plans are in my mind focus on the root of all evil MONEY. Knowing that DENTAL HEALTH HOLDS the GREATER role in ones ENTIRE BEING and wellness. Its the root tool for survival. To EAT and SPEAK think people, cause and effect, every action as well as inaction create reaction. It's not rocket science. NO ONE SHOULD EVERHAVE TO ENDURE the PURE ANGUISH this creates. The harming effects continue to grow gaining strength while you become weak in all areas of life. This ill oral health I am enduring has in fact created more negative health issues throughout my body. These issues are covered under my :MEDICAL insurance.
