Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

I feel like they are more focused on squeezing every dime out of every interaction possible which is understandable but I feel like my dentists office shouldn't feel like a sales organization. I'm still not sure if they even listen to a word I was saying the last time I was in. I've been here several times and officially don't plan on going back but they still are a functional dentist office

Dr Shah is an incredible dentist. So meticulous. If she had been my dentist the past 20 years I'd have faired much better in the long run. I am so grateful she takes care of my teeth and smile now. Besides being excellent at her profession, she has a very kind "bedside" manner.

Dr. Shah and her team are the best around! Always very kind and professional. Even though everyone is wearing face masks you can tell they are smiling and always have a welcoming environment. I came in for a crown on one of my front teeth and Dr. Shah and her team went above and beyond to make sure I was satisfied with the end result. 100% Recommended to all my friends and family!

I love this practice. Everyone is so welcoming, Kim the hygienist is friendly and gentle. The doctor is very thorough and takes her time to make sure I understand what will be/needs to be done! I thought it would be hard to switch dentists after 20 years, especially someone like me who is not fond of dental work, but I feel like I've been going there forever!

I have been a patient of Dental Associates of Plainfield for 20 years. When Dr. Hodavol retired and the practice was bought by another group of dentists, I wasn't sure what to expect since I loved my dentist and hygienist, Sue, but decided to give Dr. Shah and the team a chance. I could not be happier. I just had a crown put on, my first, and Dr. Shah and Pari were amazing. I have a great fear of the dentist, always have, but they put me at ease and I am so grateful. Kim is an amazing hygienist, as well. I highly recommend this dental practice. As a side note, they are doing a fantastic job with the Covid protocol. I felt extremely safe and at ease.
