Hi, I'm Dr. Riese. I was born and raised in Merrillville, Indiana. I attended IU Northwest and Indiana School of Dentistry where I received my DDS degree. My wife Jennifer and I moved to Monticello (Jennifer's hometown) in 1997 shortly after being married. I practiced as an associate up to 2006, at which time I purchased the practice. In the fall of 2011, we moved into our new facility.
We currently have six children. Living in Monticello, we have been involved in many activities such as church, school, and our great community. I enjoy reading mainly Christian non-fiction, pursuing and preserving America's Christian heritage, and just being a Dad. I'm blessed to be a part of giving and receiving that occurs while
Hi, my name is Cady Moss and I am a dental hygienist. I was raised near Lake Cicott and went to Pioneer High School. I graduated from IPFW in May 2013 with an Associates degree in dental hygiene. I also became licensed for local anesthesia and certified as an expanded functions dental assistant. I started working at Declaration Dental in January 2014. I love working with my fellow staff members and enjoy getting to meet new patients as well as catch up with current ones.
I was married in May 2016 to Matt Moss and we now live in Monticello. We both like to be active and we enjoy doing Tough Mudder races together. We also have a dog, Cassie, who loves to run even more than we do.

Hello, My name is Stacy Wildrick. I graduated from Logansport High School in 1993 and started my dental career immediatelyfollowing. I went to Ivy Tech Community College where I was president of my class and graduated with honors from the Dental Assisting program. I then received my certification from the University of Kentucky in 1995 as an Expanded Functions Dental Assistant. I followed that path for 27 years prior to making a careerchange in Oct of 2020 working the front desk position at Declaration Dental. I love working in a field where I can help people and make a difference in their lives and dentistry has allowed me to do that. I feel completely blessed to have the opportunity to work for not only such a caring, compassionate boss but a Dentist who genuinely cares and wants to help people!
My name is Chelsey Howat and I am an Expanded Dental Assistant at Declaration Dental. I've been here since May 29, 2013. I went to assisting school in Lafayette at Ivy Tech Community College. I live in Francesville. I attend the Teaberry Hill Estates Church in Medaryville. I enjoy spending time outdoors, exercising, and with my family.
Ihave been employed by Declaration Dental since December 2006. Prior to working for Declaration Dental I had worked in management for over 25 years. I have a B.S. in Management and Administration from Indiana University Northwest. My hobbies include coaching youth basketball and buying and selling typewriters. I live inHebronwith my wife Billie and children Austin and Amelia.My daughter Amanda and her husband Tristan live inBurnsHarborwith their children Julianna, Elaina and Nolan.
I have been in the dental field for over 10 years. I have worked Front Desk and also been an Office Manager. I enjoy all aspects of my job, but what I love most about what I do are the patients. I love being able to help them achieve their dental goals and seeing them leave with an amazing smile!

Always hated going to the dentist. When I moved into Monticello my wife and I decided to go to Declaration Dental. The office is clean, staff is welcoming and professional. The state of the art equipment, the over head video screen, lets me see what the staff sees and helps me understand the dental procedure being recommended. They provided me with a dental plan which I can use to determine my course of action. I would recommend Declaration Dental to anyone.

The staff are very friendly. I love that they play Christian music. The hygentist that cleaned my teeth was amazing she went above and beyond the dentist so friendly and kind. Stacy you are amazing thank you for scheduling my appointment. The gentleman that does all the billing very informative kind and caring I could not ask for better staff anyone looking for a new dentist give them a try they truly care.

Sandy was a wonderful hygienist. She was friendly and explained as she went. Dr. Reis was kind and spoke in a way I understood. I had an excellent experience, and that is saying a lot. I really dread going to the dentist, lol.

I cannot express my thanks to Dr. Riese and his staff for the service they provided to me when I broke a molar over their four day weekend closure. Dr. Riese had me meet him at the office on Monday, president's day and his day off, to assess my situation and alert his staff to schedule me in ASAP. My hat's off to you Doctor. Your operation at Declaration Dental is ONE CLASS ACT!!!

These folks are simply amazing! From the moment I made my appointment over the phone With Tim to the day I walked out with my smile styled, I felt at ease and relaxed. The Christian radio station in the background is soothing. Dr Riese is terrific at explaining what to expect. Thanks y'all for making me feel so special and cared for!!
