Dr. David Forlano was born and raised in Brooklyn, NY. He was certain about pursuing a career in dentistry while attending Xavier High School in New York City where he discovered his passion for art and science. He received his Bachelor of Science degree in Biology from Fordham University in 1985.

In 1988, Dr. Forlano graduated Georgetown University School of Dentistry. Upon graduation, he received the American Equilibration Award for the most outstanding student in the science of the TemporoMandibular Joint. He has studied occlusion and TMD at the Hospital for Joint Diseases in New York City. He has spoken nationally and internationally on occlusion, orthodontics and implantology.

Dr. Forlano is one of very few dentists to achieve Fellowship awards in three different dental disciplines; General Dentistry through the Academy of General Dentistry, Implantology through the International Congress of Oral Implantologists and Orthodontics through the International Association for Orthodontics. He maintains active membership in these organizations, as well as in the American Academy of Facial Esthetics, the American Dental Association and the Dental Organization for Conscious Sedation. He is a Diplomate of the International Congress of Oral Implantologists. His diverse experience results in a self-contained interdisciplinary approach to comprehensive dental care.

Dr Forlano is president of David Forlano, DDS, PC, a professional dental practice providing a wide range of services to those seeking lifelong, comprehensive dental health. Centrally located on the south shore of Long Island in a new state-of-the-art facility, the practice attracts clientele from Manhattan to the Hamptons.

For over 10 years, Dr. Forlano was on faculty at New York University College of Dentistry. He served as a Clinical Instructor in Implantology at the Linhart Continuing Education Program where he taught and mentored other dentists the science of dental implant placement and restoration. He was also the Case Presentation Leader for the Thursday session, helping the dentists prepare their cases for approval and presentation before the surgical directors, faculty and participants.

Dr. Forlano is the president and founder of Chairside Implant Services. Chairside Implant Services provides clinical implant services, mentorship and education to professional dental practices in New York, New Jersey and Massachusetts. He is also the creator of the Comprehensive Dental Assessment, a quantitative analysis of an individuals masticatory system. Dr. Forlano is also a member of PEARL (Practitioners Engaged in Applied Research and Learning). PEARL is an National Institute of Heath network of private practice dentists who conduct clinical studies in the course of routine patient care.

Dr. Forlano lives on Long Island with his wife, Karen, and his King Charles Cavalier, Lily. Their two children, Kristen and David, are persuing careers in dentistry at Stony Brook University School of Dental Medicine and Columbia University College of Dental Medicine, respectively.

Dr. Kristen Forlano was born and raised on Long Island, NY. Her passion for dentistry began at age 5. She received her Bachelor of Science degree in Biological Sciences and Anthropology from Fordham University in 2016. During her undergraduate career, she pursued the pre-dental track and served as the president of the Pre-Dental Society her senior year.

In 2020, Dr. Kristen Forlano graduated from Stony Brook University School of Dental Medicine and received her Doctor of Dental Surgery. During her dental school education, she participated in the American Student Dental Association and attended several ADA District meetings. Dr. Kristen Forlano also participated in dental outreach including Give Kids A Smile hosted by the Nassau County and Suffolk County Dental Societies. As a student dentist, Dr. Kristen Forlano volunteered with World of Smiles to provide dental care to the children of the schools of Fundacin Mission International Rescue and related orphanages in La Romana, Dominican Republic.

Dr. Kristen Forlano moved onto to her General Practice Residency at Stony Brook University Hospital from 2020-2021. During her residency, she provided comprehensive dental care to the patients at Stony Brook School of Dental Medicine. She received training in the fundamentals of restorative, fixed and removable procedures. Dr. Kristen Forlano also received training in simple oral surgery procedures, including extractions and socket preservations, CAD/CAM technology, and Nitrous Oxide administration. At Stony Brook University Hospital, Dr. Kristen Forlano served as the general dental resident on call and worked to treat emergent patients with dental trauma, acute pain, and abscesses in the emergency room. Her residency training also included treating pediatric and special needs dental patient in the operating room at Stony Brook University Hospital. Dr. Kristen Forlano is proud to have received her dental training in Stony Brook with a foundation built upon evidence based research and strong clinical training.

Dr. Kristen Forlano is currently a member of the American Dental Association, the New York Dental Association, and the Suffolk County Dental Society. She is a Certified Invisalign provider and pursuing Fellowship in the Academy of General Dentistry as well as the American College of Clear Aligners. Dr. Kristen Forlano is thrilled to begin her career in private practice dentistry at our office. Outside of dentistry, Dr. Kristen Forlanos interests include fitness, golf, tennis, biking and skiing. She is an avid Cavalier King Charles Spaniel lover and lives with her dog Lily.

Dr. Jeffrey A. Sherman was raised in Queens, NY and graduated from Adelphi University where he majored in Biology and Chemistry and received a Bachelor of Arts degree. He persued his studies of dentistry in Washington, DC and then completed a general dental practice residency in Wilmington, Delaware in 1974. Dr. Sherman continued his studies at Brookdale Hospital in Brooklyn, NY in an Implant and Oral Surgery program with an emphasis on electrosurgery.

In 1978, Dr. Sherman was awarded Diplomate status by the American Board of Oral Electrosurgery. He has been closely involved with the research and development of both electrosurgery and radiosurgery in dentistry and carried out clinical trials at his private practice in Oakdale, NY. Dr. Sherman has published three textbooks on both electrosurgery and radiosurgery and three clinical videos, in addition to numerous articles in both national and international dental journals. He has presented over a thousand lectures both nationally and internationally and is regarded as one of the world experts in the field of radiosurgery. Dr. Sherman was President of the American Academy of Dental Electrosurgery, and President of the World Academy of Radiosurgery. He was acknowledged for his expertise and efforts in dentistry with Fellowship awards in both the prestigious American College of Dentists and the International College of Dentists. Dentistry Today has recognized Dr. Sherman as one of the Top 100 Leaders in Continuing Education for more than 10 years.

Dr. Sherman has been active in the Suffolk County Dental Society. He has served on the Peer Review, Education, and Dental Laboratory and Trades committees of the society, as well as being their photographer for over 30 years. He is a past president of the Suffolk County Dental Society and served as General Chairman of the Greater Long Island Dental Meeting (GLIDM). GLIDM is the dental convention sponsored by both the Nassau and Suffolk County Dental Societies, drawing over 4,000 dentists and dental professionals annually. In 2012, Dr. Sherman was presented with the Robert Raskin Meritorious Service Award, the highest award given by the Suffolk County Dental Society, which is presented for dedicated service and enduring contributions to the society and the profession. Dr. Sherman is a Life Member of both the American Dental Association and the Academy of General Dentistry.

Locally, Dr. Sherman was a founding member of the Oakdale Chamber of Commerce. He volunteered for Donated Dental Services, which is overseen by Catholic Chatities and provides dental care to disabled, elderly, and medically-compromised people. As a Peace Corps provider, Dr. Sherman offered complimentary dental exams and x-rays to Peace Corps applicants. He was also a sponsor of The Lend A Hand Project, whose mission is to offer support to those members of our community who need extra help. Dr. Sherman is retired and lives in Austin, Texas with his wife, Roz, and close to their married daughter and two grandchildren. They have an engaged son who lives in Columbia, South Carolina.

Melissa is a 2017 graduate of SUNY Farmingdale School of Dental Hygiene. Melissa received the John Oppie McCall Award for excellence in clinical treatment of periodontitis. Melissa joined the team in June 2019 and continues to advance the field of dental hygiene through research and active participation in the Greater New York Dental Meeting. Melissa holds certificates in administering local anesthesia and laser periodontal therapy.

Yelixa graduated from SUNY Farmingdale School of Dental Hygiene in 2017. With two years of hygiene experience under her belt, Yelixa joined the team in February of 2019. Previously, Yelixa had completed her dental assisting training at BOCES and served as an assistant during her senior year of high school. Yelixa holds certificates in administering local anesthesia and laser therapy through the International Center for Laser Technology.

Karen earned her Bachelor of Fine Arts degree from CW Post/Long Island University in Public Relations. She also earned a Masters degree in Education. She joined our team in 2014 as our concierge. Karen coordinates our educational events, assists in accommodations for those traveling from out-of-state or with special needs, facilitates communication with our clients, and manages Dr. Forlano's speaking engagements.

Mandi graduated from the BOCES Dental Assisting program in 2011 and joined our team in 2012. Mandi is one of very few dental assistants in New York State to be certified and licensed. In addition to being your chairside concierge, Mandi is our organizational productivity expert, maintaining a vast inventory or dental materials and supplies. Mandi is formally trained in CEREC technology and oversees scanning, designing and milling our diverse inventory of CEREC restorations.

Kahelia received dental assisting training while in high school and continued her dental education at Jamaica Hospital. She is one of very few dental assistants in New York State to be certified and licensed. Kahelia joined our team in June of 2017. She is trained in general dentistry, radiology, oral surgery and dental implantology. Kahelia specializes in the art of staining and glazing our true-to-life CEREC restorations.

Rosa joined our team in 2020. She has been a dental assistant for five years. Rosa moved to the United States from the Dominican Republic when she was eight years old, and has dual citizenship. She recently graduated from Suffolk Community College with her Associates Degree in Science, and is currently studying for her Bachelors Degree in Biology, with plans to continue on to Dental school. Rosa is trained in general and pediatric dentistry, radiology, and orthodontics.

Marvin was born and raised in the Philippines where he earned a dental degree. He practiced dentistry there for several years before moving to the United States. Marvin joined our team as a dental assistant in 2011. In addition to chairside dental assisting, Marvin manages the laboratory & prosthetic aspect of our practice.

Our state of the art diagnostic abilities center around the OrthoPhos DS Digital Imaging System. With up to 90% less radiation than traditional dental x-rays, this system enables us to capture images in less than 30 seconds. Not only images of the teeth, but images of developing tooth buds, sinus cavities, temporo-mandibular joints and the alveolar jaw bones.

Our network of diagnostic software, including Sidexis, Simplant and OrthoCad, enable us to manipulate and visualize hard tissue in 3 dimensions - a feat impossible with standard dental x-rays. Combined with CAD/CAM technology, we will ensure an accurate diagnosis and precise dental treatment.

Cone Beam Computed Tomography has been a major breakthrough in modern dentistry. Truly state of the art, CBCT provides an incredibly accurate 3-dimensional image of the jaws and teeth. We use two mobile CBCT units, which means scans can be performed right here at our doorstep, at your home or place of business. No more traveling for your scans.

We use gentle piezoelectric surgical instrumentation for many surgical procedures. It is much kinder to the gums, blood vessels, nerves and the membranes which line delicate areas, such as the sinuses. Ultrasonic vibration produced by a special surgical instrument is used to achieve unparalleled precision and patient comfort.

Instead of a mouthful of impression material, we use CEREC technology to digitally scan teeth for crowns, bridges and inlays. Once the digital file is acquired, our state-of-the-art equipment can mill a restoration from a solid block of porcelain, lithium disilicate and even zirconia...enabling us to deliver precisely fitting restorations the same day. No more wearing temporary crowns while waiting for lab work. With our two CEREC acquisition units, two milling units, and two firing ovens, we can assure a smooth and precise workflow for same-day restorations.
