The Oral Surgery Center of Albuquerque is a provider for most major insurance companies, including those listed below. As a courtesy, we will file the claim for surgery to your insurance. It is important that we get all of the updated insurance information to prevent any delays in payment. After the exam, we will contact your insurance to get your benefits and let you know what your estimated out of pocket expense will be.
We will collect what insurance doesnt cover at the time of service. Please remember that the portion paid at time of service is an estimated portion. Payment is not guaranteed by your insurance company until the payment is received. Any balance after insurance pays will be sent to the patient. Payment plans considered upon request.
David Collette is the oldest of nine children, seven boys and two girls. He obtained a bachelors degree in biochemistry at Brigham Young University where he met his wife, Catherine. He attended Case Western Reserve University School of Dental Medicine where he obtained his DMD degree in May 2008.
Upon graduation he was awarded the prestigious Callahan Prize. He remained at Case Western to complete medical school, obtaining his MD degree in 2011. He also completed his oral and maxillofacial surgery training in Cleveland with rotations at University Hospitals and the Cleveland Clinic as well as the Louis Stokes VA. While in dental school he co-founded the Viva Guaymas Foundation (a service mission to Mexico) and served as class president. He is a native of Albuquerque, New Mexico, although he moved several times, also living in Idaho, Utah, Vanuatu and France. David enjoys photography and anything that involves being outdoors.
