After returning from his service in World World 2, Dr. Emil Kubash, a native of Cudahy, founded Cudahy Dental Associates with Dr. James Kassner in 1958. The founding office was located in downtown Cudahy on the second floor above a drug store. Dr. Kubash and Kassner recognized the benefits of group practice and in 1964 expanded their existing office and began planning for a larger office.

In 1973 Cudahy Dental Associates opened at its current location on Packard Avenue. The practice was designed to be state of the art and was the largest dental office in the state of Wisconsin at the time. Shortly after moving to the Packard Ave location, Dr. Laurence Fridel, Dr. Thomas Busby, Dr. Frederick Austermann, Dr. Dennis Toy, Dr. John Grant, Dr. Michael Ketten, Dr. Arnold Cwik, Dr. Albert Huebner, Dr. Daniel Kult and Dr. Bruce Hartwell became part of Cudahy Dental Associates. As this doctors transitioned to retirement a new group of partners joined the practice and continue its legacy today.

The dentists of Cudahy Dental have a long tradition of top notch dental care and have continued to invest in their facility and technology inorder to deliver the best patient care. Drs Couillard,Jumes and Melcherhave continued this culture of hard work and commitment to excellence that the founding and previous partners had instilled in Cudahy Dental.

One of the most notable technological advances made was the introduction of the Cerec digital workflow. This allows the dentists to complete procedures like permanent crowns, veneers, inlays and onlays for their patients in a singe visit; and aid in digitalizing many other procedures. Cudahy Dental has also invested in 3D xray imaging allowing for treatment planning and completion of implants and more complex procedures.

For your convenience, we accept cash, personal checks, money orders, Visa, MasterCard and Discover. Payment is expected at the time services are performed. Many dental benefit plans include patient copayments. Cudahy Dental requires that all copayments be paid at the time of service. When more extensive dental care is necessary, financial arrangements can be made with our office.

The doctors at Cudahy Dental understand that the cost of dental treatment can be significant and is routinely one of the most important things for patients to know when planning for the care they need. Our financial coordinators are happy to answer any questions about our customary fees for any planned procedures. Some dental benefit companies require that a pre-treatment estimation be sent for pre-approval before beginning treatment. Our financial coordinators will gladly submit a pre-treatment estimate to your dental benefits company so that you will have a better idea of what your benefits cover and what your out-of-pocket costs will be.

As a service to our patients, Cudahy Dental accepts multiple dental benefit plans. The financial coordinators at Cudahy Dental are happy to answer any questions about benefit plans we are in-network with, as these can change over time. Our financial coordinators also prepare and file claims to your dental benefits company as a courtesy to you. To assure prompt attentionto claims, it is essential that we have accurate and up-to-date information. However, we remind you that your specific policy is an agreement between you and your dental benefits company. Please keep in mind that you are responsible for your total obligation should your dental benefits result in less coverage than anticipated.

The fees charged for services rendered to those who are insured are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different dental benefits companies vary greatly in the types of coverage available. Some benefits companies take care of claims promptly while others delay payment for several months.

Kevin doesn't respond to discuss issues, and they are discriminating. Dr. Lubin try's to make you feel less than and it is humiliating. I even apologized thinking it was me. I am sorry but you can give a sad story how you have been around for so long that doesn't change the fact you treated me with disrespect... shame on you Dr Lubin and shame on you Kevin

I see Dr. Lubin and take my toddler to Dr. Reyerson. I couldn't be more happy with their care. My 3 year old is happily brushing her teeth every morning because Ms. Doctor said so :) Dr. Lubin did a lot of work on my pearly whites but, unlike with previous providers, I knew what and why is getting done. The level of detail and perfection while being mindful of the cost and timeliness is unmatched. The front desk team is super responsive and helpful (there were some technical issues with insurance and records from previous providers and they took the leading resolving everything). Teeth cleaning crew is amazing too. If you call and need to wait for your appointment, it is worth the wait!

It took 2 years to complete all the dental work I needed! I appreciated the patience & kindness from the staff, Dr. Jumes & Matt! I'm grateful, everyone went out of their way to make an experience I was not looking forward to so much more tolerable! Thank you so much for my new smile!

I have been going here for years. Everyone here is kind and friendly and takes their time with you. The care I've received has been great. The staff know their stuff. Highly recommend!

At times people are easy to complain to many but they forget to recognize when recognition is due. I'm absolutely amazed on the great service I received. I joke around with my friends about going to dentistries and meeting the receptionist because they are always crabby, # Not here. From the moment I walked in the receptionist was extremely kind she obtained my insurance information and got me set up. The female that took my X-rays was so sweet and had a great personality, I love that. I came for a cleaning and the hygienist was detailed and kind. My dentist and her assistant were the best. When I tell my friends how my services were they all want this dentist number not only did they see me within a week but the Dentist explained every single little detail on how my teeth worked, what was going on, how we were going to fix it and what to do in the future to prevent any further damage. Dr. Couillard and her assistant were great and I couldn't me more thankful. Thank you for fixing my rooth canal you guys are the best indeed You can tell employees are very passionate about their jobs here

The dentists here are great, however the office staff are a big disappointment. They're prejudice, lack decorum and were flat out dishonest after I did further investigating.

I had called and made appointments for my wife, son and I. I had needed a tooth pulled, my wife needed some work done and I just wanted to get my sons teeth looked at. I have Humana and we had state insurance at the time. They did not mention anything about it. I made sure with state that we could go to a dentist and just use my regular insurance through my work. They pulled my tooth, no problem the dentist was super nice and really knew what she was doing, a different dentist worked on my wife for a couple visits until they said we could no longer go there because of our state insurance it was considered insurance fraud. Which I didn't totally understand because I have Humana, so I didn't know why it had to do with state at all. We were told we could not be seen until we got rid of our state insurance. So my wife called and got it taken care of. We called back to cudahy dental and talked with Joyce. She said that we still had state insurance even though my wife just got off the phone with state and canceled it. We thought ok. It might take a day or two. That was back in august. Fast forward to now it is December 21 my wife is still having terrible tooth pain. We haven't had state insurance since September and they still say we have state insurance. We do not. We have proof that we have not had it since September. I have no clue why they are lying or misunderstanding, my Humana insurance is the best the two dentists have ever seen. So I don't understand why it's an issue. But my wife has been in pain for 4 months because they still say we have state insurance but we don't. I liked my dentist and my wife liked hers, but there is a serious disconnect between the front and the back. So now I owe a little more than 2 grand in medical bills from going to the er for her pain. We've been to a couple dentists who have all said we don't have that insurance. All because someone doesn't know how to do their job.

I just fired the whole place and moved to South Milwaukee Family Dental about 2 blocks away. I called the supervisor Kevin about how they are assigning patients to dentists, and the bouncing back and forth between dentists. That's when everything blew up. I went to Cudahy Dental for about 30 years, then my dentist retired, and since it has all new people to me. According to Kevin, the Asian (relevant) dentist was saying she is uncomfortable around me because I was making offensive comments. He said she claims that I said that she brought the virus from China. Kevin was not having any of my claim that I said I no such thing. Rather he kept accusing me while denying he was accusing me. So, I fired the dentist for that and then I fired Cudahy Dental because of Kevin. Kevin never answered the question I called him about. We have too many people in this world who invent in their own head what they want to hear or believe. It is not to the positive benefit to the target but a negative accusatory charge. They twist and torture what was said so they can argue or be offended. I don't play that game. Instead I have learned in 60 years to cut off relations with these people. It is not possible interact intelligently with people who invent stuff in their own head and then attribute it to me. I have learned that these people are incurable. I don't want to play amateur psychologist to figure out what their hang ups are. Who knows what they will invent next? I just want reasonably satisfactory dental care. What really was said: During the filling (she always did good work by the way) I specifically asked the dentist what racial background she was because "I did not want to offend her." (quote) She didn't specify. I held up my mask and noticed the masks I was wearing had Chinese writing on them and said some people say the virus came from China, kind of interesting. Then she did the fillings. Kevin also said the assistant agreed that I said it. Apparently, the supervisor Kevin don't think maybe the assistant don't want to anger and get on the bad side of the dentist. Or factor in that we are in a politically charged everyone wearing masks pandemic thing. Even with that in view I do not give them a pass for claiming I said something that I did not. There is no way what I said could be heard as - that I accused her of bringing the virus from China. I am not going to tolerate my dental office twisting any little thing into an offence and thrusting me into some sort of hyper offensiveness or hyper defensiveness fabrication. Even though Dr. Caputo, Dr. Melcher and all the hygienists were great, they also won't get my business. I will be getting my wife to go somewhere else also. I never had this in the last 40 some years with 30 some being at Cudahy Dental. Like I said these fabricators are incurable. They hear what they want to hear. I have moved to a different dental practice.

Grew up going to this office as a child and young adult, couldn't have been more disappointed upon my return. Set up a new patient appointment back in December, since I had lived in another city and been seeing another dentist for the past 9 years. Initial appointment went very well. Dr Melcher was great, approachable and friendly. I booked my next appointment for the Doctor's recommended treatment. I received a letter in the mail 6 days before my appointment explaining, in 3 sentences, that I would be expected to pay all costs not covered by insurance upfront before receiving treatment. I called to ask about a payment plan, I was told someone from billing would call me the next day. Next day came, and I never heard from anyone. I ended up having to reach out to them again. It took less than 45 seconds for them to state we don't do payment plans, full payment will have to be made upfront. The website states that "When more extensive dental care is necessary, financial arrangements can be made with our office." This was not at all what I experienced. It took less than 20 minutes to find another dentist in the area for on my insurance who was willing to work with patients when it comes to payment. I would avoid this place, unless you have the money to pay upfront! Only reason for giving 2 stars and not 1 is because the Doctor himself was great. It's unfortunate to not be able to see a dentist I liked and trusted because of financial front office policy.

i had one REALLY sweet dentist. and one REALLY rude orthodontist. which left a bad taste in my mouth, literally. had invisalign that they wouldn't transfer to another orthodontist after that one was the WORST for a year. so i'm out $5000...!?????? that we paid in full but okay. never ever. again.

As a human with teeth that can only be described as a genetic pop-up-book from hell; I assure you, dear reader, that I've had my fair share of dental work. Dr. Caputo was my gateway into this practice and I've since seen Dr. Melcher, and most recently, Dr. Wang. I've had fillings, crowns, and because it's 2020, a root canal. This team is skilled, kind, and full of good people that look out for your wellbeing.

Stay Away Rude front office great dentist but have no clue how to properly operate a business they called 3 days prior to my appointment to reschedule me in 3 months Horrible business practices. Now I must switch Dentist in the middle of the year
