Dr. Cullom was born and raised in Indianapolis, IN. He attended Indiana University Bloomington, where he earned bachelor and master degrees in Exercise Physiology. He returned to Indianapolis where he obtained his General Dentistry degree from Indiana University School of Dentistry. Before entering private practice, Dr. Cullom furthered his education by completing a General Dentistry Residency at the University of Alabama-Birmingham Hospital. There, he was exposed to advanced training in oral surgery, IV sedation and treatment of medically complex patients. In 2005, Dr. Cullom moved to Florida and worked for Florida Dental Centers until 2011, when he purchased the practice of Dr. Robert Pastorius, a well-respected dentist of over 30 years.
The son of a school teacher and police officer, Dr. Cullom was raised under the old-fashioned principles of hard work and treating others right. His love for dentistry evolved from having positive dental experiences with his childhood dentist and mentor, Dr. John Bozic, in addition to his love for science and helping others. He continues to stay abreast of the latest dental advancements through continuing education courses and participation in the local Perio-Prosthetic Study Club. Dr. Cullom strives to provide the most gentle and compassionate care to all patients. He takes the time to get to know his patients and listens to their desires and concerns so they can have the best dental experience possible. Dr. Cullom feels the most rewarding part of his profession is helping a patient transform from a dental-phobic with pain and multiple dental health issues to being pain-free, excited to go to the dentist and thrilled with their new, healthy smile.
In his spare time, Dr. Cullom enjoys spending time with his wife and family, coaching his daughters soccer team, volunteering at the Free Clinic, participating in his churchs Patriarch bible study group, traveling and working out.
Diana grew up in Lutz and the Florida Keys before moving to St. Petersburg in 1996. She attended college for business and later obtained her real estate license. She owned and operated her own consulting and brokerage firm, selling businesses and franchises throughout Florida and the US. Dianaassistedher husbandopeningCrescent Lake Family Dentistry. She oversawtheconstruction/renovation ofthe1920 building from start to finish.Her goal for his practice is to provide a warm and inviting atmosphere making patients feel at ease while receiving top-notch, personalized, quality care. She and Dr. Cullom have three wonderful children Joseph, Autumn and Audrey, andtwostubborn french bulldogsnamed Milo& Max. In her free time she loves to spend time with friends and family, including her seven siblings, architectural design and renovation projects, business ventures, charity work, travel, reading/research, music festivals and almost any outdoor activity.
Her name says it all! Our patients love our hygienists and so do we! Joy has over 20 years experience in the dental field. She started her career working front desk for a previous dental office. She then went to Tallahassee Community College where she obtained her dental hygiene degree. A few years into practice, Joy furthered her education and earned a Bachelors Degree of Applied Science in Dental Hygiene from Saint Petersburg College. This enhanced Joys career by allowing her to teach. She has ten years of experience as adjunct faculty in the dental hygiene program at Saint Petersburg College and Valencia College in Orlando. Her knowledge and experience from the clinical and teaching setting has allowed her to provide exceptional care for our patients. Joy is a member of the American Dental Hygiene Association and Pinellas West Coast Hygiene Dental Association.
Rita Kay grew up in Wheelersburg, Ohio. She has acquired a wealth of knowledge in the dental field, having started her assisting career in 1976. She was previously employed by two well-known and respected doctors before coming to join our team after they retired. We have been blessed to have her in our dental home since May of 2013. She is a very detailed, thorough and compassionate dental assistant who loves to make every patient feel comfortable. Rita is extremely active and creative. In her free time she loves to spend time with family, friends and her cat Spencer Keith, paint and sew. She is also very active volunteering with church missions, the food pantry, and assisting in women and childrens ministries.
Beth began her career in private dental practice in 2001. A native Floridian, Beth was born on the East Coast and grew up in Central Florida. After graduating high school, she obtained her Associates of Arts degree at Lake Sumter Community College. Beth then moved to Gainesville where she was accepted into the dental hygiene program at Santa Fe College. Upon completion of her Associates of Science degree in Dental Hygiene, Beth moved to Tampa Bay. She considers herself lucky to have studied alongside the dental students and doctors at U.F., and in the dental clinics at Shands Teaching Hospital. Beth loves working with people of all ages, and finds her interactions with patients the most rewarding aspect of her career. Her hobbies include writing, drawing, community theater, and jogging. She also loves to surf and snowboard. Beth most enjoys spending time with her husband and two children, going to the beach, riding bicycles, and cooking.
Vanessa was born in Havana Cuba and is fluent in both Spanish and English. Her passion for dentistry began in high school. She performed her internship at Crescent Lake Family Dentistry, while finishing her dental assisting program. Vanessa graduated at the top of her class, earning certifications in radiography and dental assisting with expanded functions. Vanessa displayed such passion and eagerness to learn this profession, she was hired on the first day of her internship. She is extremely thorough and detailed oriented. Vanessa loves helping patients and staff in any way she can. In her spare time she loves to run, spends time with her family, her furry four-legged friend Emma and spreading the good word.
Kaylenn was born and raised in Tampa. She graduated at the top of her classearning certifications in both radiography and dental assisting with expanded functions. She obtained her associates degree and is currently studyingfor her Dental Hygiene License at State College of Florida. There isnt much in the office she isnt involved in. Kaylennalways helps without hesitation when others need a helping hand. These qualities are how she earned a position as an administrative assistant. She does everything from helping patients up front, to assisting Dr. Cullom or our hygienists with clinical work. Her favorite part about working at Crescent Lake Family Dentistry is helping and communicating with patients. Kaylenn lovesfestivals, the beach, and watching sports.

Hands down the best dentist out there. Dr. Cullom has a steady hand that moves with confidence and knowledge. The true definition of what a dentist, and medical professional should be. The supporting staff is also just as exceptional, kind, caring, and knowledgeable as well, and make you feel like family. The office is also decorated handsomely with a homey feel, and is clean to the level of my standards, and expectations. I can't say enough good thing's about this place, and am glad to have friends at a dental office, should a situation arise. Best dentist & supporting staff ever. Real talk.

They have done a marvelous job of renovating and redecorating the building. It is very pleasing to the eye, very bright, and welcoming when you enter. Staff are very pleasant. Dr. Cullom is very good and caring as well.

By far the best dentist I have ever been to. Dr. Cullom did a lot of work on my teeth. I love my smile now! He is compassionate and will never make you feel bad for what you may have going on in your mouth. I have been to many dentists and Dr. Cullom is the only one I will ever go to again! Let me tell you Dr Damian Cullom did a ton of work on my teeth and now I can smile with confidence. This guy is super caring and is truly the best at what he does. I've been to a lot of dentists and these folks are the best! Everyone is friendly and happy to help in any way they can. Dr. Cullom has been fixing some errors of previous dentists and he does a great job of explaining everything along the way. If I ever move away from St Pete, I think I'll fly back just to work with this team again.

Dr. Cullom and his team are fantastic. I recently went in to get a second opinion on some recommended dental work. Dr. Cullom went out of his was to explain his diagnosis and offer options. The whole staff is top notch. I am switching my family relationship over today.

Absolutely everyone here is wonderful, professional, respectful, and compassionate. A great team, where each individual knows what they are doing. The team is understanding about my disabilities and mobility limitations, and seems open to be as accommodating as possible. The building has wheelchair access by way of manual lift. Most importantly: I have never felt judged for my teeth and my ability to care for myself in this office.

I became a new patient of Dr. Cullom's last October after my previous dentist of nearly forty years retired. I was a little reluctant to start with someone new, but was soon very comfortable with the caring manner and friendliness of Dr. Cullom and his staff. A couple weeks ago when a tooth was bothering me, he made time in his already booked schedule to see me that afternoon. I really appreciated it very much and felt so much better after seeing him. I highly recommend Dr. Cullum. He and his staff are great!

Clean, hygienic, exceptional dentist. Smiling, kind, understanding and painless. I'm a big baby when it comes to dentists, but they made me feel as comfortable as possible. The dentist is kind and he was gentle. Thank you so much , Crescent Lake Family Dentistry, for a pleasant experience. BUYER BEWARE Let me start off by saying Dr Cullom was very good and very thorough with his process and procedures. The work he did was very good. The issue is with the front desk. I walked in to see the new office and fill out some paperwork and possibly get some work done. I filled out the paperwork and followed up the next day to deliver the paperwork along with my insurance card. They said the paperwork looks good took the insurance card and asked if I wanted to set up an app. So I set one up for the following week. After the app I was told of an area of concern for a cavity so I went ahead and scheduled it to be done the next week and did so. The next week I got the work done and the Dr was great and did a wonderful job. I paid my copay as I left that day. Come to find out a couple weeks later I receive a bill in the mail for almost $300. Then I was brought back to when I was leaving that day and a lady in there was complaining about the exact same thing and ended up just paying the bill and walking out. This didn't register much with me until I was in that same position. I called and asked why I got a bill and the front desk informed me they don't take my insurance. This was almost 3 weeks after I had given them my ins and they went ahead and set up my app with me feeling everything was good to go. After a long talk with his wife and another girl I was told directly that they messed up and are at fault for not letting me know before hand but will NOT work with me on the bill. And also said, "well we just cant make money on ur services if we drop the price." Even after they admitted fault they wouldn't budge and for the fact of making money. To me its not about the money spent because a service was performed its the fact that it was the fault of the front desk and they passed it on to the customer. I'm sure me and the lady complaining that day aren't the only ones. It also took them over 3 weeks to cash my check...the same check that they just couldn't do without. UPDATE: I wrote this review around 9pm Sept 14. By 9am Sept 15 I had a threatening and accusatory voicemail demanding I take this review down. I still have this voicemail. She demanded I take down this review or they will go to my supervisor at my job. Today Sept 22, this lady showed up to my workplace trying to get me in trouble. Completely unacceptable and very disturbing a wife of an owner would go to these lengths. I hope this helps for anybody reading this. Update 2: I find it funny u mention my supervisor when a google review is off work time and you have now brought this to a whole new level which is fine with me. Let that sink in for whoever will eventually read this. A google review has triggered someone so much that they will call and harass you and show up at your workplace. If that's not absurd and bizarre I don't know what is. As a matter of fact I am actually holding in my hand a statement from said supervisor contradicting yet another false statement.
