Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

We accept many insurance plans including Delta Dental PPO/Premier, Cigna, Metlife PDP+, Dental Health Alliance, Connection Dental, United Health Care PPO, as well as most other major PPO's. Throughout the year, our dentists and staff travel to provide desperately needed dental care to under-served nations throughout the world. In many cases, we do this in conjunction with charitable organizations that operate every day to provide needed goods and services to these nations and communities. My teeth are complicated. One issue is that the braces and rubber band system used in me in 1985 pulled my molars down so far that the roots have always been exposed. I have avoided having anything done for decades because of the pain that was pretty normal for me. But finally, I gave in... this dental office is awesome! They filled the open area, replaced defective fillings, removed a cavity, crowned the area...and with very little discomfort! Super professional and amazingly skilled!

All 5 star reviews and absolutely worth it. The doctor made sure I was comfortable, took care of business, his staff was charming and professional. I could not recommend a Parker dentist more. Truly a great experience, if I could give 6 stars I would.

I had a crown replaced due to decay underneath. Dr. Mahoney and her assistant were great. The entire staff are very pleasant and I would recommend this dental office to anyone looking for a dentist.

Caitlin, the newly appointed accounts person is HORRIBLE! She began trying to explain my billing to me and made every excuse in the book. She deflected what is happening with my billings and focused on an alleged missed appointment-that I wasn't charged for that? She could not stay on point and suggested that I speak to yet another person who "handles accounts;." She is the worst guest service person I have ever encountered! She never listened to anything I said and would not review any of the charges on my account. She wants to deflect responsibility for the meaningful questions I have. My service with Dr. Kim and my hygienist has been satisfactory, but billing is a major part of your business as well. If you have incompetence represent your business, you will have many disgruntled customers. The service I received in the chair was fine, and the desk as well, until I spoke to Caitlin!

Best experience at the dentist! My hygienist, Denise, was super personable, professional, thorough, and kind. She is very experienced and confident in what she does and knows. I loved being in her care. I even enjoyed some serious belly laughs with her (when I didn't have an instrument in my mouth).

Denise was the best hygienist ever. I had a lot of gum pockets which I've never had before and Denise assured me we could work together to reverse it!! Very impressed with Denise and All the staff and their state of the art equipment.

It's about time I wrote a review for this wonderful place. I can honestly say I look forward to every single appointment I have (every six months on the dot!) because I AWAYS leave with a smile on my face. A HUGE part in this is the staff - Denise in particular is an absolute delight! She is the sweetest human I've ever had the pleasure of meeting and she takes such great care of me while I'm there every single time! Samantha at the front desk is also amazing! She always greets me/sends me off with a smile and such a joy to talk to while checking out! She answers all of my questions and always has great communication when it comes to my appointments! As if that weren't enough, my teeth have never been healthier! I cannot recommend them enough - a billion stars out of five! <3

My husband, daughter and myself all had appts today. Staff was amazing. So pleasant and kind. Staff went over each care suggestion, discussed options and allowed us to make decisions without pressure. We were very impressed with the level of care received.

I've been to too many dentists to count in the 6 years I've lived in the Denver area and I have finally found my "home." The entire staff from start to finish were welcoming, professional and I felt like they listened to my concerns. Dr. Kim was great and this was the best dental experience I've had in a LONG time. They were timely, efficient and I felt confident in my dental care when I walked out of the door. Highly recommend.

Absolutely fantastic dentist office! Diane is the best dental hygienist I've been too in 30 years! She's caring, gentle and really knows her stuff! I use the nitrous since I don't like needles and they always make sure I'm comfortable! The front office staff is always kind and welcoming! All together a great experience

Creekside Dental is an outstanding practice! Everyone there is so friendly, if you're scared of the dentist this is the place to go to get rid of your fears. You'll always be welcomed and feel right at home! I don't think I'll ever switch practices! So excited to start my Invisalign treatment as well!
