Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by completing the forms online prior to your arrival for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

We accept Mastercard and Visa credit cards, as well as cash and personal checks. We accept all PPO insurance plans (NO HMO). We are in network with United Healthcare, Aetna, Cigna, Delta Dental and United Concordia. To find out if we accept your plan, please call our office.

My husband and I have been going to their dental office for over 25+ years. Our kids have grown up with them and are now full grown adults that continue to go to their dental office. Their staff is very professional, friendly, and polite. Dr. Walther and Dr. Matthew are both very good in what they do and go out of their way to explain and provide the best treatment possible for your needs. I've worked with them both and trust them both. Above all, they're just genuinely nice people. Their schedules are very accommodating. I highly recommend them because I know they will genuinely provide you with excellent care.

Always a Pleasure Seeing Jody, Sarah, Dr Mathews and Dr Corey Walthers.. They all know what to do for when you come in for your teeth!! All are very friendly and are Great People to Know!!

Dr. Walter and his team are the best! The entire experience from arrival to departure is first class. If you have any anxiety over going to the dentist, it will cease to exist once you become a patient. I want to send a special shout out to Sara the hygienist! She rocks!!!

Dr Corey Walther's Office is great. I felt 100% at ease attending my appointment as they were vigilant with practicing safety precautions and sanitation. Per usual, always professional and efficient. This place is neat and clean. Helpful staff, knowledgeable doctors and hygiene specialist. Only one comment to mention - it does not accept Discover network and there is no online option to make payment.

Everyone is so friendly, it's like the old Cheers television show. You wanna go where everybody knows your name, and they're always glad you came. Dr.Corey J. Walther is my new dentist, and I was with old dentist for 30 years I am impressed with his work and so happy I found someone who takes pride in his work and does a excellent job.
