Dr. Jeremiah Cook was born and raised in Highland, Utah. He graduated summa cum laude with a Bachelor of Science degree in Biology from Utah Valley University where he met and married his wife Annilee. He received his Doctor of Dental Surgery degree from Baylor College of Dentistry where he graduated with honors and received the Quintessence award for clinical achievement in Periodontics. He completed his specialty training in Periodontics and implant dentistry at the University of Texas Health Science Center at San Antonio where he also earned his Master of Science in Periodontics. Dr. Cook is a Diplomate (board-certified) of the American Board of Periodontology. Dr. Cook has advanced training in cosmetic periodontal therapy, bone grafting, dental implants and IV conscious sedation anesthesia
Dr. Cook is a board certified periodontist who is an expert in his field and well known for his caring attitude and warm sense of humor. He has changed many lives by improving their oral health and smiles.
Dr. Cook and his wife have 3 children and reside in Prosper, TX. They have made Texas their home after spending a decade here. Dr. Cook is excited about being a periodontist and serving the community.

Dr cook , He's awesome ! , He is Communicative ,thorough ,tells you what he's doing , and going to do ,to ease any fears ,and I lucked out finding him. His girl that assists him , Gabby, Is also personable, Friendly, bubbly and attentive and calms your nerves as well .She is also a great asset . I feel however that your girls in the front office ,are a reflection of the office and the first faces you come in contact with . I feel they should be bubblier , and nice People are going in for expensive procedures when you go to a periodontist and no one really wants to , thry have to . They are nervous and about to pay a lot of money so his front girls should be friendlier . Just saying .but I'm still giving him a 5 despite that.

Absolutely hands down the best Dental Provider I ever entrusted to manage my dental care! He goes above and beyond to meet his patients needs! Took the time out to call me the night of surgery day to see how I was doing!..better yet opened his office "ON A SUNDAY" to clear a painful infection! Love u Dr Ccok!

Doctor Cook and his staff are real professionals, they went out of their way to get me into their office when I was in a lot of pain from an infected tooth. Doctor Cook clearly laid out my options without any pressure, thoroughly explained the procedures and afforded me as much time and questions as I needed. He proceeded to work on my tooth and made 100% sure I was not in pain, was empathetic and comforting as we went through the procedure and found myself virtually painless after the procedure. His staff debriefed me and equipped me for all possible scenarios that I could experience when I got home. Dr Cook even personally called me that night to check on me. This is a first class facility with a bright and talented Periodontist that truly cares and delivers a high quality product and patient experience. Truly a 5 star treatment. Fred Hammond

I was informed by my regular dentist that I would have to have one of my teeth extracted due to a cavity below the gum line. I am a complete coward when it comes to any type of dental surgery.. Dr. Cook was extremely patient with me and made the procedure as painless as possible. The office also followed up with me to make sure that they were no complications or issues. If you are in the need of any type of dental surgery I highly recommend this office..
