Dr. Cook grew up in Zearing and graduating from NESCO high school. After earning her Certified Dental Assistant award from Marshalltown Community College she started her first assisting job in 1989. The dentist she worked with believed that everyone has a unique set of talents and gifts. Discovering those talents and learning how to utilize them would create the best chance at personal success and happiness. Dr. Cook credits him for inspiring her to explore those gifts and further her education.

While continuing to work as an assistant, Dr. Cook took classes at Iowa State University, Drake and Grandview, gathering all she needed to for dental school. She graduated from the University of Iowa in 2001. She enjoyed her time at Iowa and worked hard to help her fellow classmates, earning the Deans Leadership Award all four years.

Dr. Cook explored many different areas and decided that the Roland Story Community proved to be the perfect fit for her family. She opened Cook Family Dentistry in 2001 and has been committed to providing gentle and thorough care for all ages. Dr. Cook appreciates the special connections and relationships that only a small community can enjoy.

Dr. Cook is a member of the American Dental Association, Academy of General Dentistry, Iowa Dental Association, American Academy of Facial Esthetics and the Story County Dental Study Club. Locally, Dr. Cook belongs to the Story City Greater Chamber Connection, a Main Street Partnership, Roland Story PTO, Roland Story Booster Club, is a past president of the Story City Chamber of Commerce.

Kris is one of our outstanding dental hygienists at Cook Family Dentistry. She graduated from DMACC with her dental hygiene and anesthetic licenses and has been with us since 2003. Kris loves visiting with her patients, she is a great listener and works hard to assure they have a great experience. She loves what she does and is passionate about educating her patients on the importance of dental hygiene.

Kris has the ability to make everyone feel at ease and has a very gentle technique to ensure you are always comfortable. She has enjoyed getting to know her patients and their families over the years. Watching them grow and sharing in their life experiences is just one of the ways that makes Kris so genuine and sincere. Patients definitely appreciate her knowledge and ability to communicate effectively with them.

Kyla is one of our Certified Dental Assistants at Cook Family Dentistry. She attended DMACC where she earned her Dental Assisting Diploma and has since obtained her Expanded Functions endorsement and stays current with continuing dental education classes. Kyla brings over 10 years of experience to caring for her patients, they really appreciate her kind, gentle and compassionate personality.

Colleen is one of our Registered Dental Assistants at Cook Family Dentistry. She attended DMACC in Ankeny and has since earned her Expanded Functions endorsement. She has grown up in this community and raised her family here. She has been a part of our dental team since 2003.

Colleen is known for her warm personality and is great at making patients feel comfortable during their visits. She has a friendly personality and is full of energy and ideas. Colleen has many creative talents that she brings to our team. Not only does she assist with patients, she spends time in our lab making dental appliances such as sports guards and bleach trays. Colleen is very talented and enjoys interior design and decorating. If you look around our office you will see her work in our dcor. She has a special touch that gives our office a very cozy and comfortable feel.

Nancy is our office manager and has been a loyal employee since the opening of our office in 2001. She is highly invested in our patients and works to ensure they have the best experience possible. She takes pride in providing the care and attention necessary to help our patients. Nancy enjoys learning about people and building relationships. Nancy is our account and insurance guru and will work hard to help you in any way she can.

When Nancy is not working, she enjoys being with her family, especially the time spent together in Clear Lake. Nancy has been married for over 31 years to her husband Jim and they have two daughters Jessie and Kylie. Jessie is married to Joey and they have three adorable boys Lincoln, Nash and Briggs. Kylie is married to her husband Andrew and they have an adorable son named Quincy. Nancy is extremely proud to work with a team of professionals who are committed to educating and improving our patients overall long-term oral health.

Meredith has been practicing Dental Hygiene since 1991. During those years she has worked in the States of Illinois and Iowa. She started her dental career working as a dental assistant in a General Practice that also did orthodontics. She has always been intrigued by the dental field and remembers at a young age knowing she wanted to be a hygienist. Meredith can relate well to all kinds of people and loves the variety of working with different ages.

Meredith and her husband Brian have four children, Payton, Tayler, Baylie, and Hayden. She enjoys being with her family and together they stay active attending athletic events. With 4 children being involved in sports, you can usually find her in the bleachers.

For a valid reason, I had to cancel appointments at here. They explain the rules to me and I followed the rules from then on. I did my best to follow the rules and then I found out they have banned me anyway. It seems like the dentistry can ban the patients from any reason. The lady on the phone told me that they can do whatever for the rules. I told to the lady that I wish they could have told me any of those before, and it's upsetting. She told me that they don't care how I feel uncomfortable, humiliated or upset. She also said that she doesn't feel sorry at all. From this, I could tell this lady was the same lady who made the problems and gave me some attitudes before, and I told her that I really don't like any of those. She was working hard on implying me that all the disgusting experiences happened here were never happened since this lady doesn't know or remember anything. Then if I tried to clarify any of those, this lady got extremely defensive and aggressively high-handed as before. I could strongly feel that the lady was twisting the truth. I really got upset with this because it means that all the things I remember were not real and that made me feel like I am a delusional person or a liar. I told that I don't like how they make people to take responsibility like this even they have faults. Then she said, "Oh Yeah, you are a perfect man. Just take things whatever you like to." She even told me to find another dentist that treats me good and that makes me feel nice. Abuse of power, abuse of authority, or abuse of right, whatever you call... Just simply Wow. Too rational. Please be careful, they give you a nice smile, but I don't think they have those manners, respect, or true compassion. It's a rational smile to put you into their tiny tyrant rule/world. It doesn't need to be like this, but unfortunately it happened. Whatever the problem happened here becomes never happened. There is no apology or fix, and even they would give you some serious attitude, that's still also ok, because they are the rules.(or I might say this lady is the boss and no one can stop.) It's always hard to consider in the business world to decide whether you put the value on money or people. I sadly found at here, it's definitely more about money, not about people, and it sent chills down my spine.. God bless America...

I just switched dentists because it was closer to my home. Awesome first visit. Staff was all great and they communicated what they were going to do and why ahead of time. Showed me detailed pictures of plaque buildup before cleaning and after. Took measurements to check for gum disease. Only took x-rays that were needed, as I was able to have previous x-rays from prior dentist sent to them electronically. Very pleased. Small town service at no extra cost! Would definitely recommend to others. Mark

I believe the doctor here works professionally, but maybe the receptionist has some issues. I had to cancel appointments three times this year based on proper reasons. First one is the weather issue which happened on the day of the appointment. The receptionist gave me a call and warned me that I need to give them a call at least before 24 hours of appointment if it needs to be canceled or reschedule again, even if it's a bad weather. Second one was the COVID-19 infection I got. I tried to call them earlier but they were closed, so I left the voice message. Got a call from her several days later. She warned me again even I tried to reach them out earlier... Third one is Mayo Clinic that I really needed to be there for my health issues I have. Gave them a call about few weeks ahead of the appointment. The receptionist told me that I can't keep canceling like this, otherwise I am not allowed to be there anymore, because it's business. When I was there last time, the doctor was telling me that since I don't live in Story city, she thought maybe it's better to see doctors in my city. So she told me that if I am interested in, she could let me know or recommend someone she knows. The receptionist attitude made me to decide I just don't want to deal with them. I gave a call and told the conversation I had with doctor. Then she told me, "We know some dentists in your city, but we don't recommend or refer anyone like this way, so we don't do it." She didn't listen what I said and simply denied the conversation I had with doctor. She also told me, "and if you are going to see other doctors or need to cancel again since you are thinking to change the doctor, you will never be able to see us again. I spoke on the phone with you and told this before. I remember you. This is business. You can't keep canceling like this because it's business." Then the receptionist told me that I have six cancellations. I know the three I canceled and have proper reasons. I just really surprised where just suddenly other three cancellations came from? She told me that I have missed cancellations twice in 2014, which is 7 years old record... I was surprised to hear she brought up 7 years old thing, but she said that those won't count. I asked her again about three mysterious additional appointments. And she told me that I missed one earlier this year, and two after that. Simply there was no clear answer to my question... So I had to again ask her the same question, and also asked why there is no exception for those three from recent even I have proper reasons? She told me that those two from recent won't count... she just doesn't answer to my questions So two from 2014 doesn't count, and two from recent doesn't count, because there were reasons. This clearly makes only one I have... I asked her, "okay, that makes one I believe, not six, right?". She said, "No, you have total six cancellations. We have a record from the past. I know you. I talked with you on the phone, and I told you about this before. We knew you were going to do this again because you have a history in our records. We knew you were going to give us a call and cancel in the minutes before of the appointment again today(which was not minutes before at all...). We have you on our file. We know you are a person who cannot make it. We expected this was going to happen again today. Ok? You can't do this because it's about business." Never answer to my questions... Cannot stop repeating/humiliating many times that I have a past history, along with six strangely exaggerated cancellations, which she hardly clarified the details. If I tried to clarify those, she started to say "WHAT DO YOU WANT!!?" I just told her that I don't want to deal with them. For the business wise, wrong, too arrogant, too tyrant and strangely too offensive. Telling me like I should serve them 120% of the way they expect, rather than they serve or care. I don't think a person who has a big condescending attitude toward others like this never feels sorry for anything, and I'm glad I will never see her again.

I was more than happy with the service my two young children received. The staff was warm, welcoming, and patient with the kids. I havent seen a dentist I would WVER recommend but this facility I would in a heart beat
