Since our inception in 1974, EAFHC has been an integral part of the regional health delivery network in eastern Arkansas. We currently have seven primary care clinic sites in Crittenden, Poinsett, Mississippi, and Phillips counties, on the eastern edge of Arkansas, as well as an AIDS care center in Crittenden County. Our staff is committed to providing the highest quality of care and excellent customer service to our patients.
In 2017, EAFHC was recognized as a 'Health Center Quality Leader' by the Health Resources and Service Administration (HRSA), and five of our seven primary care clinics are recognized by the National Committee for Quality Assurance (NCQA) as a patient-centered medical home (West Memphis, Blytheville, Helen, Lepanto, and Trumann).
With our patient-centered medical home model, EAFHC provides a full range of primary medical care and dental services. Our providers treat patients of all ages from newborns to geriatrics working with them to develop comprehensive treatment plans that emphasize wellness and prevention.
In addition to medical and dental services, EAFHC offers support services that focus on helping patients maintain compliance with their treatment plans. These support services include case management, nutrition counseling, prescriptive diet and exercise programs, and extensive health education programs.
2001: EAFHC responded to the growing HIV/AIDS population in Crittenden County and surrounding counties by applying for and receiving federal funds to address this need through the Arkansas Delta AIDS Care Center.
2014: Each of EAFHCs five primary care clinics was recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). 2014: In October 2014, EAFHC opened its new main location in West Memphis, expanding from the 38-year-old, 7,700 square-foot building to a 35,200 square-foot, state-of-the-art medical and dental facility.
The facility includes 26 medical exam rooms, 2 medical procedure rooms, an ophthalmology treatment room, 9 dental suites, a 340B pharmacy, a comprehensive testing suite that houses lab and X-ray services, a large community room, an educational classroom space, and space for administrative services. The exterior of the facility features a walking trail around the perimeter of the property that is available to patients, staff, and the public.
The much-needed additional space enabled EAFHC to hire four medical and two dental providers creating the capacity to treat nearly 10,000 new patients and provide nearly 27,000 new visits to a community that lacks access to even the most basic health care services.
2017: On August 18, 2017 EAFHC unveiled the East Arkansas Family Health Center Fitness Park. This park features a 1/4 mile walking trail, along with 875 square feet of workout space, 15 fitness stations that are durable, and weather-resistant outdoor exercise equipment.
This health center receives Health and Human Services (HHS) funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

East Arkansas Family Health Center, Inc. welcomes all private-pay, fully and partially insured, Medicaid, and Medicare patients. For patients without insurance, we offer a sliding fee scale based on income and family size to ensure that quality, compassionate health care is readily accessible to everyone.
If you have insurance: If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, it will be necessary to pay at the time of your visit. Payment will also be required for all non-covered services. For your convenience, we accept cash, Visa, MasterCard, and Discover.
If you do not have insurance: We believe that quality health care should be available to everyone, whether or not you have health insurance. If you are uninsured, you may be eligible for various programs as listed below, if you meet the income requirement:
Our Fees: We maintain an established fee schedule for all medical and dental services provided. The fee schedule is reviewed annually by the EAFHC board of directors to ensure sound financial advantage to our patients, and to ensure that the fees are consistent with community standards.
