Community Health is a primary care network that provides nationally-recognized programs, a focus on wellness, dental, behavioral health and pediatric specialties, walk-in Express Care, a culture of community and quality health care that almost everyone, insured or uninsured, has come to depend on.

Community Health, based in Rutland, is the largest of the 12 federally qualified health centers (FQHC) in the state of Vermont. Nationally, more than 1,200 FQHCs provide primary health care to one out of every 14 people. In Vermont, FQHCs provide care to approximately one in four Vermonters.

Community Health opened in 2004 as Community Health Centers of the Rutland Region, providing a network of medical, dental and behavioral health care. Its grown to include six medical facilities, three dental offices (two adult and one kids dental), a behavioral health clinic and a pharmacy. We have locations in Rutland, Brandon, West Pawlet, Shoreham and Castleton, providing care to over 41,800 patients in the Rutland County/southern Addison County region.

First established in 1965 as part of President Lyndon Johnsons War on Poverty, FQHCs are primary care centers, located in underserved and rural areas, that provide high quality, cost-effective health care regardless of an individuals ability to pay. The success of the FQHCs was recognized by Congress when creating the Affordable Care Act (ACA) in 2010 and the ACA legislation permanently authorized the program.

Community Health Primary Care Offices offer a full range of services including preventative health care (annual physicals, work physicals, school, campand sports physicals), womens health care, well-child care, including immunizations, management of chronic or serious illness, minor office procedures (including laceration repair and removal of skin lesions), and workers comp injuries. X-ray and laboratory services are available at some of our locations. Hospital care for Community Health is provided by Rutland Regional Medical Center Hospital (RRMC). RRMC is a hospital-based practice specializing exclusively in providing in-patient care at RRMC. Arrangements for admission to other hospitals can be arranged as needed. Community Health Pediatrics provides hospital care for our pediatric patients.

Our Brandon Community Pharmacy is located inside Community Health Brandon. You may use the services of our pharmacy, or you may use a pharmacy of your own choice. Our pharmacy accepts the same insurances that are accepted at our other offices.

Community Health Centers of the Rutland Region, Inc., a Federally Qualified Health Center (FQHC), is deemed by the Bureau of Primary Health Care of the U.S. Department of Health and Human Services to be a federal employee for purposes of medical malpractice claims and, as such, qualified for protection under the Federal Tort Claims Act.

This health center receives U.S. Department of Health and Human Services (HHS) funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

According to the FTCA requirements, medical malpractice claims against the health center are reviewed and/or litigated by the U.S. Department of Health and Human Services. An individual who has a claim against the health center is to file an administrative claim with the U.S. Department of Health and Human Services at the following address:

At Community Health, we provide vaccinations for flu, hepatitis, pneumonia, tetanus, HPV, childhood diseases and other illnesses. Your provider has information about the available vaccines, the recommended age groups and boosters needed to maintain full protection.

Children are immunized from measles, mumps, rubella and other diseases; teenagers between 11 and 12 years old can receive an HPV vaccine preventing them from developing certain cancers in the future; and flu shots are available every year starting in October for the whole family. The coronavirus vaccines are available for children as young as 5 years. For those with chronic health problems like diabetes or asthma, flu and coronavirus symptoms can make their chronic health conditions worse.

Schedule one appt 6 months ago, they give me the wrong date and tell me there's nothing I can do when I show up...schedule me then for today's appt, I call two weeks prior to confirm when my appt is. I arrive after driving an hour and a half to get here, to be told my appt was pulled for lack of confirmation. Who is running they're clerical division, it's a joke.

Don't even wast your time here front office is very rude and unprofessional. They have cancelled or messed up my appointment several times and refuse to take responsibility for it instead turn it around and blame it on you .

Dr. Alison Landgraph and her assistant deserve 10 stars, but 5 will have to do. They were very caring, quick, and efficient with my fillings. My wife and I have both had a prior filling from other dentists, and with Dr Landgraph's impeccable talent, our experience with her was so much better! Thank you Dr. Landgraph for taking care of my families dental needs. You have some very satisfied customers!

Amanda does such a great job on my teeth!! She is very knowledgeable and takes time to explain what she is doing any why. She thoroughly cleans my teeth with out rushing whatsoever, staying on time so you are in and out. My smile is big and beautiful thanks to her!! Oh and she gives me a "goodie" bag!! =)

Waited two months for an appointment for today. Tooth root is exposed. They deemed my pain tolerable. 5.5 months down the road is my next appointment to get it fixed! Yes, I have dental insurance. I guess 5 more months of sleepless nights. UPDATE: The folks there at Community Dental Squeezed me into a much earlier appointment for a root canal. I am now pain free and getting back to eating and sleeping like normal people. I never could of imagined how much a tooth ache could negatively impact my daily life and personal attitude! For that alone, I will be forever grateful.

I went to Community Dental five years ago and I Had a bad experience I went today and the experience I had deserves a 5 star rating! I felt that the dentist cared about my teeth and I also did not feel judged!! Looking forward to my next appointment to get my fillings fixed and my other teeth that need a little TLC thanks Community Dental team!

I waited 2 months to get an appointment. I just wanted a cleaning, but was told I needed to schedule for a new patient exam. I showed up for my appointment only to be told they could not complete my exam because my insurance wouldn't pay for new x rays because I had x rays last year and they needed my records from my old dentist before they could complete the exam. I told them I just wanted a cleaning, but they told me they would not clean teeth until I had an exam. They rescheduled me and told me they would request my records from my previous dentist and I just needed to call my old dentist and give permission for them to send the records, which I did. On the day of my rescheduled appointment, I called to make sure they received the records. They had not. It turns out they had not even requested them. I was then told I had to fill out a form to request the records and unless I want to pay for more x rays, I have to reschedule. They said they would email me the form to fill out. I did then later receive an email from them asking me to fill out the attached form, but there was no attachment. I gave up and found another dentist.

I didn't go to the dentist for about 14 years and ended up having a previous filing fall out, which was causing me such bad pain. Because of COVID it took a little longer to be seen but the day I got in there I thought I was going to lose the tooth. They fixed it that no and I had no other problems so I went back for a regular visit today which was another great visit. As someone who hates going to doctors and dentist I can definitely rate 5 stars and say these people are amazing

Oh, I never reviewed. I sure was glad to find a Dentist that would take Medicaid and/or Medicare! I thought it might be lacking, but no way! Staff is great, and treat me well especially after I explained my dental fears weren't about the tools or procedures... My copay is small, and they don't bully me into buying *exclusive* treatments I surely need, unlike my last dentist I paid a fortune out of pocket for!

I had a root canal done. The workers staff and doctors are very helpful. Explained everything before and as it was happening. I gave the 5 stars for the professionalism and the work . This was my second visit more to come no problems. Definitely worth checking them out.

Stay far far away unless you are prepared to book a year in advance and still get cancelled on. In November I scheduled an appointment for MARCH, their earliest availability. In January they accidentally cancelled it when they should have just put me on the waitlist like I requested because of a broken tooth dental emergency and the pain I am in. I called today and their scheduler Emily refused to admit the mistake and make it right. They said I could come in for a checkup and cleaning on the 22nd, but Id have to wait for the next appointment in JUNE. What an absolute waste of my time, and the pain in my mouth was all for nothing, all because I was trying to go somewhere that I could afford.

Dr. Rivard is wonderful. I am not the easiest dental patient as it's a huge trigger for my anxiety and past bad experiences as a child. She is kind, understanding, patient, and makes sure I am as comfortable as can be. She takes her time and explains things, and answers questions. I would recommend her over and over again!

Do not book. Front desk staff are careless and irresponsible. Two months ago I made an appointment here with someone while they had just started the renovations to the building and called two weeks ago to confirm my child's appointment, everythinglooked fine. After, calling to check date and time to make sure everything is great for the trip, they lost contacts and information to the appointment. I have worked in several dentist offices and never been irresponsible. I rate the new place a 1 star because its apparent they are run down.
