CommuniCare Health Centers is a Federally Qualified Health Center providing health care to those in need since 1972. Using the patient-centered medical home model of care, CCHC puts patients first by providing comprehensive quality health care services delivered by a dedicated team of providers and support staff. Our services include primary medical and dental health care, behavioral health services, substance use treatment, health education and support services. Our goal is to develop and maintain a long-term healing partnership with each of our patients to achieve the best possible health outcomes. Serving communities throughout the region, CommuniCare Health Centers cares for one of every ten residents of Yolo County.

CommuniCare Health Centers is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n) with respect to certain health and health-related claims, including medical malpractice claims, for itself and its covered individuals.

We would like to acknowledge the land on which our health centers are built and on which we live, work and serve our community. For thousands of years, this land has been the home of Patwin people. The Patwin people have remained committed to the stewardship of this land for many centuries. Much Yolo County land has also been shared by the Miwok people. We are honored and grateful to be here today on their traditional lands.

We acknowledge the Nisenan people are still here among us today, though they are nearly invisible and their story continues to be excluded from history. We understand we are on Nisenan land that was never ceded and the original tribal families have yet to recover from the near genocide of their people. As residents and visitors in Nisenan land, we stand beside the Nisenan people and their right to sovereignty, self-governance and self-determination through the stabilization of their people, homelands and culture.

Worst clinic ever! Very poor management!! My friend tried to open a new file in this clinic and it's been more than 2 weeks! They have no clue what they're doing. Very bad experience

I could cry...in the best way. I've had horrible experiences at doctors and dentists growing up. This place is the first and only place I feel comfortable, supported, and able to reach my health goals. Dr. Southwick Frances is the first doctor I've had in 28 years that will tackle each and every one of your health issues, like frickin' dominos. She believes you, supports you, and helps you make a solid plan. I've improved my health more with her in 2 years than I have in a decade. She will get it done. She will take care of you. Just superior patient care all around. It doesn't stop there. Calvin Yee, DDS is the sweetest, most gentle, supportive dentist I've ever experienced. He even expressed support for my general health and my mental health?! Unheard of. I'm used to terrifying experiences of shaming, irritibility, and aggressive handling. Both him and his assistant, Genevieve, made me feel safe, welcome, and actually excited to be taking care my oral health. Again, I could cry. The mental health programs have also changed my life. Jean is the sweetest, most empathetic and validating therapist. Dr. Kunkel works with you to decide what medications you feel comfortable trying. If it weren't for Dr. Kunkel, I would still be living with untreated adult ADHD-Inattentive. Finally being treated has changed my life for the better and given me what it takes to tackle all my other health issues. I could go on forever, but this is the best clinic. Make an appointment, you won't regret it. If you don't connect with a provider, simply request to switch/make a new patient appointment with another one. I had a doctor I didn't click with at first, but switched to Dr. Southwick and I will never look back! Come in, they will take care of you, everything about you

I initially received god service from the dental area when I was between having and not having insurance. I went because it was affordable and accessible. However, I tried to call to reschedule an appointment yesterday and the person kept hanging up when they were attempting to pick up the phone from being on hold. It happened several times, so I could not even get through to cancel and reschedule my appointment. This took over a 1/2 hour of my time and I was calling the day before, so they could fill the appointment. As a result, I had to cancel the appointment the day of my appointment at 8:00 a.m. since I could not get through the previous day. The receptionist said, "We can't reschedule you because you canceled in April." I responded, "Yes, I was sick. Do you want sick people coming to appointments?" I did not have COVID but who penalizes a person for rescheduling an appointment because they are sick? Especially during the pandemic. I spent more than a reasonable amount of time this morning trying to explain to this receptionist multiple times so she could understand the situation. She said she took notes and will let her supervisor know and said "We know how to use the phones here in dental." Apparently not because the person yesterday kept disconnecting me when trying to pick up the phone. It was frustrating. Lots of money was spent on refurbishing the building, but apparently time and resources need to be spent on training, creating better policies and procedures, and the phone system. I have heard about this problem from others before and I have now experienced it. This is not professional and needs to be fixed immediately.

One of the receptionists was rude during my family's visit. We were discussing my monthly income when figuring out a payment plan. She said, "you make more than most people that come here for services". That was not a necessary comment. Bad customer service!

I was uninsured when I first went to Communicare. Everyone I came in contact with was kind and respectful. The medical care was excellent and I appreciated the attention paid to preventative care. In addition, they helped me with insurance options and other programs. I continued to use them after I had more resources. Now I have insurance that doesn't cover them, but I am donating to them this year because they show compassion and respect for regular people who don't have a lot to fall back on. Most places treat uninsured and Medi-Cal patients like dirt. Communicare is an outstanding exception.

I've been going to the dental clinic here for over 10 years and to the health clinic for over 3 years. I have been extremely happy with the service I received. The people are caring and knowledgeable. The only complaint I would have is that sometimes the receptionist have not been good, but I'd rather have weaker receptionist and excellent medical treatment then the reverse.
