Periodontal treatment is necessary when various conditions affect the health of your gums and the regions of your jaw bone that hold your teeth in place. Retaining your teeth is directly dependent on proper periodontal care and maintenance. Healthy gums enhance the appearance of your teeth, like a frame around a beautiful painting. When your gums become unhealthy, they can either recede or become swollen and red. In later stages, the supporting bone is destroyed and your teeth will shift, loosen, or fall out. These changes not only affect your ability to chew and speak. They also spoil your smile.

Periodontal diseases are ongoing infections of the gums that gradually destroy the support of your natural teeth. Periodontal disease affects one or more of the periodontal tissues: alveolar bone, periodontal ligament, cementum, or gingiva. While there are many diseases which affect the tooth-supporting structures, plaque-induced inflammatory lesions make up the majority of periodontal issues, and are divided into two categories: gingivitis and periodontitis. While gingivitis, the less serious of the diseases, may never progress into periodontitis, it always precedes periodontitis.

Dental plaque is the primary cause of gingivitis in genetically-susceptible individuals. Plaque is a sticky colorless film, composed primarily of food particles and various types of bacteria, which adhere to your teeth at and below the gum line. Plaque constantly forms on your teeth, even minutes after cleaning. Bacteria found in plaque produce toxins or poisons that irritate the gums. Gums may become inflamed, red, swollen, and bleed easily. If this irritation is prolonged, the gums separate from the teeth causing pockets (spaces) to form. If daily brushing and flossing is neglected, plaque can also harden into a rough, porous substance known as calculus (or tartar). This can occur both above and below the gum line.

If gingivitis progresses into periodontitis, the supporting gum tissue and bone that holds teeth in place deteriorates. The progressive loss of this bone can lead to loosening and subsequent loss of teeth. Periodontitis is affected by bacteria that adhere to the tooths surface, along with an overly aggressive immune response to these bacteria.

Periodontal disease is dangerous in that it is often painless and symptomless. Eighty percent of Americans will be afflicted with periodontal disease by age 45, and 4 out of 5 patients with the disease are unaware they have it. It is important to maintain proper home oral care and regular dentist visits to reduce the risk of obtaining this disease.

Autogenous bone grafts, also known as autografts, are made from your own bone, taken from somewhere else in the body. The bone is typically harvested from the chin, jaw, lower leg bone, hip, or the skull. Autogenous bone grafts are advantageous in that the graft material is live bone, meaning it contains living cellular elements that enhance bone growth.

However, one downside to the autograft is that it requires a second procedure to harvest bone from elsewhere in the body. Depending on your condition, a second procedure may not be in your best interest.

Allogenic bone, or allograft, is dead bone harvested from a cadaver, then processed using a freeze-dry method to extract the water via a vacuum. Unlike autogenous bone, allogenic bone cannot produce new bone on its own. Rather, it serves as a framework or scaffold over which bone from the surrounding bony walls can grow to fill the defect or void.

Xenogenic bone is derived from non-living bone of another species, usually a cow. The bone is processed at very high temperatures to avoid the potential for immune rejection and contamination. Like allogenic grafts, xenogenic grafts serve as a framework for bone from the surrounding area to grow and fill the void.

Both allogenic and xenogenic bone grafting are advantageous in that they do not require a second procedure to harvest your own bone, as with autografts. However, because these options lack autografts bone-forming properties, bone regeneration may take longer than with autografts, with a less predictable outcome.

This product is processed allograft bone, containing collagen, proteins, and growth factors that are extracted from the allograft bone. It is available in the form of powder, putty, chips, or as a gel that can be injected through a syringe.

Graft composites consist of other bone graft materials and growth factors to achieve the benefits of a variety of substances. Some combinations may include: collagen/ceramic composite, which closely resembles the composition of natural bone, DBM combined with bone marrow cells, which aid in the growth of new bone, or a collagen/ceramic/autograft composite.

Synthetic materials also have the advantage of not requiring a second procedure to harvest bone, reducing risk and pain. Each bone grafting option has its own risks and benefits. Dr. Peltzman, Dr. Jose or Dr. Torres will determine which type of bone graft material is right for you.

Dr. Barry Peltzman spent his early years on the streets of Brooklyn, New York, lettered in three sports in high school and went on to the city college of New York (Baruch College). After graduating Magna Cum Laude, Dr. Peltzman went to Stonybrook University where he received his Doctor of dental surgery. After a hospital residency in general dentistry, Dr. Peltzman went to the University of Maryland at Baltimore to get his specialization in Periodontics. After graduating his periodontal program, Dr. Peltzman taught at the University of Maryland Dental school and was a research associate in a landmark study on bone grafting. He subsequently opened his Columbia practice in 1983 and is a Diplomate of the American Board of Periodontology.

Dr. Peltzman is married to Tyra Peltzman and they share two daughters, 18 & 21. They have a dog, cat and a parrot. He has two older children, a daughter 35 and a son, 38 from a previous marriage. His major hobbies include Fantasy baseball and football along with collecting sports Memorabilia. Dr. Peltzman practices five days a week and maintains his credentialing with continuing education courses.

Over the past several years I had lost a lot of my teeth. It become more noticeable when I began losing some of my frontal teeth. It looked really unattractive and it affected my self esteem and confidence. I could no longer smile. Luckily the COVID mask mandate helped me to use face masks to conceal the ugliness of my teeth. Now that the COVID mask mandate is lifted I can no longer continue to wear masks under that pretense. I realized that the solution would be dental implants. I went to several of the big name implant organizations but the costs were prohibitive. I finally went to the Columbia Center for Implants and Periodontics in Columbia,Maryland where Dr. Keyla Torres examined me and gave me the the estimated treatment plan and cost. I totally felt at ease with her. She has an attractive and appealing personality. When I learnt she was going to be the dental surgeon for my implants I quickly decided to go with them. She asked the account department to discount my cost which brought my balance several thousands of dollars cheaper than the other organizations I talked with. Long story short I went there and had my implants done. Dr. Torres was my doctor. She was so warm and friendly and yet professional in her treatment approach. Her staff was awesome and phenomenal. All the nurses and dentists there were very warm and made me feel at home. Their warming care and treatment seemingly boosted the anesthetics and made me feel less pain. They took a picture of my smile before and after the surgery. The picture of my smile after was unbelievably fantastic. I could not contain my emotions and spontaneously busted into some buga dance. I was pleasantly surprised as Dr. Torres and his nursing team joined me in the dance as we all celebrated the beautiful transformation of my smiles. The most impressive part of it all was the follow up calls. Dr. Torres and the nurses called several times to check on me after I returned home. That's Total, Loving and Comprehensive Dental Care.

Dr. Jose (Joe's) and Columbia Center for Implants are both amazing. I cracked off half a tooth and they were able to get me in the next morning to take a look. Dr. Jose stayed late a few days later to remove the remaining portion of my tooth and get it ready for an implant. Both he and all of the office staff are exceptionally kind and courteous. They go through everything with you ahead of time and then follow up after your procedure, even if it is after hours. I have already recommended them to family and friends. I will not go anywhere else after the amazing experience I have had here.

First, let me say that the office is beautiful from the ceiling to the floors! Very modern space age with "The Jetsons" vibe! The girls out front and the assistants were the perfect mix of professional and friendly, making us feel at ease. The doc explained things using a patient level vocabulary and our trust went thru the roof when he told us that his services wouldn't be needed in the near future. He didn't give us the feeling of being a money hungry vulture. The written estimate was far, far lower than the others and will absolutely be using them in the future!

I have been a patient of Dr. Jose for over a year. He really meant what he said in taking great care of patients. Not to mention his skill and caring spirit again, last May, I updated him my gum conditions. I was so pleased and relieved and grateful that he was able to operate on me right away even it meant added time to his busy schedule. The surgery was very successful. Thanks Dr Jose.

Dr.Jose was a great find. I just relocated to the area and had finished up extensive orthodontic work. He and his staff were extremely friendly and efficient with stepping in to take on my surgical plans. The facilities are top notch and the implant procedure was (relatively) painless.

My experience thus far from Dr Jose and his team has been excellent. I am paranoid of dentist and Dr Jose has been very gentle and caring with the procedures he has done for me. My 1st surgery was June 29 for a bone graph and then another on July 15. The 1st time and 2nd time I had surgery, I received a follow-up call in the evening. The nurse was checking up to see how I was feeling and if I was having any discomfort and also to see if i had any follow-up questions. I want to thank Dr Jose and his team again. Very professional office.

Dr.Jose and all of the employees are really nice and helpful. Dr.Jose has been really nice and reassuring bout everything and explained everything he was doing. He always got back to me when had a question bout things or would have the office call and set up appointments to be seen. It is a very clean environment and everyone makes you feel welcomed. I would definitely recommend them to anyone who need implants or need work done.

Despite Dr. Jose being highly recommended, I was still very skeptical and apprehensive about oral surgery. From the initial meeting, Dr. Jose made me fell quite comfortable. He attentiveness to my questions and concerns were very reassuring. Fast forward to extraction day, of course I tried to put on a relaxed persona, while still being very uneasy about having a tooth removed, for the first time. Dr. Jose and his staff created a relaxed and fun environment and in no time, I was done and getting instructions as to how to care for the tooth removal. I'd recommend Dr. Jose to and friends or family members. Great staff and environment.

This is a 5 Star Plus Practice! The staff is so friendly that you can relax. Dr. Jose is not only personable, but there was no pain while he did excellent gum surgery. I go there from Silver Spring (20 miles) for periodic cleanings and just had the pleasure (yes, pleasure) of meeting Dr. Peltzman, who did a great job. I highly recommend this practice.

I had a surgical procedure on my gums. Dr. Josue was very gentle and calming. I was worried about feeling pain. I did not feel any pain whatsoever. I followed the regimen he prescribed after the procedure and still did not feel any discomfort or pain. The center was clean and the staff was friendly.

Dr. Jose did a good job fixing my front tooth so far. He removed the broken tooth and started with a bone graft before I get my permanent implant once it heals completely in three months.
