TAMPA, Fla. (October 13, 2020) Coast Dental & Orthodontics is notifying patients about a data theft investigation. The information stolen involves certain patients who received care at the Coast Dental offices in Conyers and North Druid Hills in Georgia.

During the night of August 6, 2020 into August 7, 2020, a locked moving truck containing equipment and patient records was stolen from a parking lot in Atlanta, Georgia. The police recovered the truck on or about August 8, 2020 and the truck was secured by the police and placed in an impound lot. Coast Dental placed a lock on the truck while it was in the secured impound lot until the truck could be moved from impound. On August 25, 2020, Coast Dental recovered access and possession of the truck and the items inside the truck. Coast Dental secured the contents and transferred the property to its facility in Florida. From approximately August 26 through August 28, 2020, the contents were inventoried, and upon a full investigation, it was discovered that among other equipment and property, approximately 1,700 patient records containing patient information were compromised or missing.

In an abundance of caution, on October 13, 2020, Coast started mailing letters to affected patients, notifying them of this incident and also offering access to credit monitoring services at no cost to those whose social security numbers may have been compromised. Patients whose information was involved are advised to follow the instructions in the letter to protect their data.

Patients who believe they may be affected by this incident may call (855) 252-2731 Monday - Friday between 9 a.m. and 11 p.m., and Saturday - Sunday between 11 a.m. and 8 p.m. EST., excluding major U.S. holidays.

Coast Dental is committed to protecting the confidentiality and security of patient information. In response to the incident, Coast Dental has provided additional education to its workforce members and refined processes to better secure patient information.

IF I COULD GIVE THIS PLACE 0 STARS I WOULD. EVERY SINGLE LOCATION, INCLUDING CORPORATE! STARTED MY CLEAR ALIGNMENT TREATMENT AND THEY DID NOT FINISH BUT I STILL GOT CHARGED. WHEN I TRIED TO GET NEW IMPRESSIONS MADE FOR THEM TO COMPLETE THE TREATMENT, THEY DECLINED!! WHY?? THIS IS LITERALLY THE WORST EXPERIENCE I HAVE HAD AT A DENTIST OFFICE AND THEY WILL BE HEARING FROM MY LAWYER!!

Part 1. I went to coast dental in Port Richey on 11/2021 for a cleaning and first-time visit. When I got there all they did was x-rays and tell me I had a small cavity which what was shown on the x ray. I had to return to get that tooth fixed which I did on 11/4/2021. Dr. Horowitz stated the filling went well it was close to root but overall, it was fixed. Later that night the numbing went away, and I had pain but thinking it was just sensitive since the tooth was just worked on and since the cavity was so close to the root that was why the pain occurred. Yaa, no it became worse 2 days later. I called the office and went back to be told we would have to remove the filling and put medication in for a month with a temp filling then return back to the clinic to put a new permanent filling to let the root calm down in hopes it would work, but if not I would need a root canal...(I was very upset at this point) After a month I went back like I was suppose too, he removed the temp filling and did the permanent filling... as he said everything went well. Later that night I did not have pain, but I noticed that the tooth was rough... not smooth and I had a big gap in my teeth so now food was getting caught in the tooth causing my gum to be inflamed (which I had document pictures of) but at this point I was going to just going to live with it and not go back. I was going to find a new dentist anyway because I still was never called to get my cleaning... On 08/2022 my tooth started hurting again and I decided NOT to go back to Dr. Horowitz and get a second opinion. I went to a dentist in trinity and had a great experience. I did not tell them about the pain in the tooth because I wanted to see what they would say. They advised me that after my x-rays were taken that the same tooth compared to Dr. Horowitz x-rays on 11/21 (which I had pictures of) the cavity was not all taken out and the tooth was starting to decay...and if I did not fix it I would need a root canal (funny like Dr. Horowitz said) also looking at the filling visually the Dentist said the filling was not even performed correctly because it was not properly sealed plus it was rough (I this point is when I started to tell the dentist what had happen). The dentist was in disbelief and stated you need a refund... and we need to fix this tooth before you need a root canal. I called Coast Dental Corporate's number for a refund and to complain. When calling 1 was scheduling 2 was billing questions 3 was calling office 4 was patient relations (complaint line) every number I hit I got the scheduling department and was advised that there was no number for Corporate. There was no supervisor or upper management to speak with but I can give them the message and they will send to "Corporate"... yea no, the messages went to the office lol which whatever that's fine because I've called and left a message for Stephanie the manager when I found out about my x rays and no call back. Then 2nd time on 10/05/2022 (I didn't leave a message) but funny how I called corporate again to send another message only to get a call 3 hours later from the office and it was Stephanie the manager, I will get to how that phone call went in a minute.

Terrible. They charged my insurance and make me pay out of the pocket and the doctor left my treatment incomplete because Dr Nan left the company and the office reject continue the treatment with another Coastal doctor. They told me that they were going to reimburse my money and after 6 months still waiting for my money back. I need to pay another doctor to start over the treatment for 2 crowns and I cannot do it because I don't have the money and my insurance is almost complete my deductible. My tooth is not good and I have pain and I need to fix this ASAP. I called so many time the office in Brandon and they don't call me back but when I was in the treatment they were calling me constantly to get the info from my insurance. I do not recommend this company they are thieves.

This place owes me over $800 in refunds for work that they have not done. It has been eight months and they've been giving me the runaround regarding my refund. Although they promised to refund me, they have not done so. This all happened because I am a cash pay Client and they charged me upfront for all of the services that I received. I paid over $3500 for services over a few months time.

Don't go there. They will mess you bill up and you won't know until you are in collections. The dentist and the hygienist were nice. My issue is with the front office and their billing department. I started going to Coast almost a year ago. They recently sent me a bill for $370. When I called the front office they stated that the bill was an unpaid bill from my insurance company. When I asked what services did they not pay for the front office couldn't tell me. The front office stated that they have a billing dept and I had to call they to see why I was billed. The billing dept showed the outstanding balance but they stated only the front office could tell me why I was billed.

WARNING!!!! The dental assistant (male) at COAST DENTAL in Apopka left me with a bruised neck and chest. HE dropped from about 2 to 3 feet and weighing 5 to 10 pound X-Ray bib on my neck and chest. Then HE proceeded to shove large x-ray film in my mouth with me kicking and screaming. All the while telling me over and over again "this is the way it is, this is the way it's done". Had to pay $47.00 for this. Reported to their Corporate office with absolutely no response. Did file a police report and reported to the Elder Abuse Hotline, Florida Medical Association, Florida Blue Medicare, State Attorney General Office, and BBB. Anyone else out there who has had a similar experience there would like to hear from you. Post here. Will never go back. Thanks. Update. Since my post have read several Google reviews where he abuses elderly people as well others with this very aggressive behavior. SO LETS KEEP OUR COMMUNITY SAFE. SHARE THIS WITH FRIENDS AND ESPECIALLY THE ELDERLY Have received notification from BBB. Coast Dental denied my request for 47.00 refund. So be aware before you go. Or better yet DO NOT GO TO COAST DENTAL. THEY DO NOT KEEP THEIR CLIENTS SAFE.

Had to pay for a procedure in advance even though they claim to have payment plans. Had just started on multiple visits when the dentist quit. I then had to move out of state. I have been trying to get a refund for 3 months. Left numerous messages. Nothing. All they will say is that it's a corporate thing and it may take multiple months to process. So buyer beware. They are very hesitant to return your money.

Lawrenceville, GA location I hate DENTIST because of this place. WORST EVER!! Let's start with a billing issue since 2018 that still has not been resolved. A credit is shown in the office system however corporate sends me a bill approx every 8 months. I repeat the cycle of contacting the office (now on the 3rd office mgr) who confirms the credit and the amount and sends an email to collections regarding the situation. I asked for the corporate office information and was told "we can't even contact them and we don't have their information". Meanwhile, I'm m receiving calls regarding this bill and threatened to be put in collections which also affects my credit. The oral surgeon while doing a root canal continued es to drill while I screamed in pain or that they did such a sloppy job with my temporary mold for my four front teeth (caps were needed after a car accident). The mold was practically thrown together and looked like a glob of gum that I went home in tears because she had to pick her son up from the airport. Could not get an appt for over two years due to no hygienist and them never answering the phone or returning voice mails.

This is the most unprofessional dental office. We've waited twenty minutes and there was no sign saying they were at lunch. What business has unlocked doors with no sign about the wait. This place is poor service.

They are sending me bill for last few months, which I don't have. Nobody picks up phone. Voice mail is full, email not going through. Nobody is willing to fix the problem and bill has final notice or they will send me to collection.

I would do no stars if it would let me...I have tried for amonth to get my money back for the 2 sets of dentures that they made that never fit. Supposedly the office manager put in for a refund Dec. 23...no call, no email, NO MONEY. DO NOT DO BUSINESS WITH THEM....

Disgusting company. They owe me a refund for around $500 it's been over a year and I haven't received any money. I have called them multiple time and they still haven't resolved the issue. I even spoke to someone in corporate and they assured me that this will be taken care of and here we are going on another year and absolutely nothing. The black women that took my payment in the Ruskin office stole my money and now works at the riverview office. Don't trust them. This is probably the reason they can not keep a dental hygienist in the Ruskin office. Pay me my money.
