In 1971, during the early days of the Community Health Center Movement, Clinica Sierra Vista was started in the small Kern County farming community of Weedpatch. A group of visionaries set out to provide quality health care to migrant farmworkers who harvested the lush agricultural fields in the southern San Joaquin Valley, but struggled to afford even basic needs like food, shelter, and health care.

Were honored to continue to serve the men and women of the fields. We have also expanded our footprint throughout the Central Valley, offering care and support to the inner city, the rural and isolated, those of low, moderate, and fixed incomes, and families from an array of cultural backgrounds who speak several languages. We dont inquire about immigration status because we simply dont need to know. If you come to us, we will treat you like any other patient.

We attribute our growth and success to the simple guiding principle established in our early days, and it has never changed: Health care is a right for all, not a privilege for some. We often hear from patients that their 1st visit to a doctor was at a Clinica Sierra Vista community health center. We consider that a sacred responsibility, and we are humbled by their trust.

Clinica Sierra Vista is a comprehensive healthcare organization serving the primary medical, dental, and behavioral health needs of about 200,000 people in Kern and Fresno. With the continued support of federal, state, and local governments, we have been able to reach out to thousands of new patients, offering comprehensive programs tailored to the needs of those we serve over a vast geographical region in the heartland of California.

We know that health and wellness cannot be narrowly defined, that it encompasses a persons entire life. Thats why we are leaders in delivering services to address problems like homelessness, substance use, teen pregnancy, and other struggles that can compromise physical and mental health. With great pride, we can also say that we are recognized as a social justice organization, advocating tirelessly on behalf of our patients in many areas, not just health care. We believe the vulnerable need a voice, and we have tried to add ours to the chorus since we founded Clinica Sierra Vista.

We plan to continue to work to build a system of care that will assure the survival of a locally governed and controlled healthcare agency. Our success far exceeds the original projections and expectations of the funding agencies and founders of a small, rural health program in Weedpatch. That success has been mainly from the dedication of this organizations board and staff. Our team has demonstrated a deep understanding of rural healthcare delivery, community relationships, and an intimate, personal knowledge of patient needs.

Clinica Sierra Vista is a Federally Qualified Health Center (FQHC) system. It receives the Department of Health and Human Services (HHS) funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

  • Clinica Sierra Vistas target aim is to transform our care delivery model to prioritize population health. We aim to do that by enhancing Clinica Sierra Vistas Team Based Care delivery model. The model will include holistic defect-free healthcare with no waiting through intentional culture development while eliminating errors and non-value-added work from our system. STRATEGIC DIRECTION ALIGNMENT 1. The guiding A3 aligns with our strategic plan strategies 1. The goals outlined for the next two years align with our strategies of developing a system that is reliable and safe 2. That develops our people to their full potential 3. Building quality into every process in flow 4. Incorporating these things into our processes is the innovation we strive to achieve 2. Foundationally, the Clinica Sierra Vista Production System (CSVPS) will empower our teams to problem solve and look for innovative ways to help CSV accomplish its mission Center of Excellence GOAL (AIM / TARGET CONDITION) To establish a center of excellence using Daily Management, focusing on stability, kaizen, and staff engagement promoting innovative approaches to improving patient experience and with the following characteristics: 1. Population health through patient-centered, family-focused health outcomes and a standard that allows for empanelment and a team-based care to be assigned to each patient 2. Standard work developed for each cycle of work that includes and exceeds regulatory guidelines and metrics 3. Enhance the usage of reporting tool in EPIC 4. Outstanding internal and external customer service through advanced training methods 5. Employee and patient satisfaction surveys 6. State of art facilities 7. Welcoming, clean, patient friendly environments 8. Centers with all health services included (One stop shop)

    So I waited two weeks for my dental appointment and when I get there they said if this will just be an assessment and if I want to continue seeing my dentist here that I need to establish medical care as well so pretty much a waste of my time because their medical clinic is not very good so I do not want to see the medical clinic

Dont waste your time in line early for nothing..Was in major pain got here 530am first in line then 8am came to be told to come ba k at 1030 and then was seen at 11 and told I have a root canal and it would take a long time to work on, funny the same oriental doctor did my other root canal took over an hour was awesome but nope today it was it will take a long time so he referred me out told me to call other places, I told him please do something work on my tooth in the front anything I was waiting since 530am he said nope we focused on that one area, wow I know it's becausethey didn't want to go into their lunch how selfish, a ten min appointment took over 3 hours of a wait and then he sent me off with antibiotics and two days later still didn't get them sent to. The he pharmacy, called the clinic no antibiotic on file..wow staff is so lazy!

    They replaced a filling that I lost in a car accident because it was exposed and hurting but after they replaced it the pain became worse from jaw to ear I lost sleep, time and suffered more than before the procedure. They don't respond to text questions, take forever to pick up the phone and then told me i have to wait every morning in line as a walk in to maybe be seen that day but have to wait hours for there mistake in unbelievable

    Don't go to this place, Rude and Unhelpful people work there. Even if you are a Walk-In they tell you there is a chance you wont see anybody still, so you sit there from 8 am to 5 pm for no reason even for emergencies.

    My experience with this clinic is the worst, I arrive at 7:30 in the morning as a walk in patient a did all the paperwork I told the receptionist that a had alot of pain, waited for my turn it was 2:40pm so I ask the receptionist what was going on she say since your a walk in patient you have to wait till all the appointments are done. Me with my pain I told the girl you could have said this when I started this morning,not wait 6hr later to tell me this, so I didn't see the dr left with pain Very angry, Would not recommend this place for no one In less you have no $ to take care of dentists.

    The general dentist lady who works on tooth fillings ect. Is so rude! I asked for white fillings not silver and she got mad because she didn't want to deal with that and do the extra work. It should be what the patient wants and there option not the dentist. Aslo, when numbing the gum she waits NO time for the gel to numb and insert the needle ... she instantly Injects needle into gum and doesn't do it gently it was so painful!!

    Honestly, I have had some unfortunate appointments made in the past where it turned out that the appointment was not even made on the day they had scheduled me for. My recommendation is to save those cards that they give you for the appointments otherwise they may not honor it. Fast forward to 2 years later, I have no problems with my scheduled appointments let's hope that it stays like that.
