Below are the Medical History and Privacy Policy forms you'll need to complete on your first visit. You can expedite matters by downloading these forms, completing them at home and handing them to our staff when you arrive for your initial visit to our office.
On your first visit, your dentist will review your medical and dental histories and evaluate your oral health. We will take high definition pictures of your teeth and we will explain to you whether or not you will require any treatment. Our team will be happy to answer any questions you might have.

I went to the New Brunswick office. I have a tooth with huge cavity and I need a cost estimate for extraction and implant since Medicare will not cover it. This doctor just took x ray , even didn't look at my tooth, and just refer me out to another dental surgery office two houses down on the street. I felt unwelcome by that doctor and been pushed to see another doctor. I doubt all the good comments not true in the review , very disappointed.

I've had terrible experiences in the past with dentists and for that reason I let go of my dental health for a couple of years. After lots of research, I decided to give Premier Family Dental a try and I could not have made a better decision. The staff is great! The dentists very professional. I have gone through extensive dental procedures with them and not one day felt any type of pain. In fact, I struggle to stay awake while they are doing a dental procedure because of how relaxed I feel. All my appointments always start promptly. I love it. 100% recommended.

The staff at Premiere Family Dental are very professional and make you feel welcoming as soon as you step in their office. Chriss is very caring with her patients and gentle with her work. The whole cleaning process was very comfortable and the scheduling is very easy.

My husband and I absolutely love this office. I personally love how gentle, knowledgeable and caring Dr. Naba Hadi is. She explains everything thoroughly and constantly asks if you feel comfortable throughout the process. Sandra is very sweet and welcoming. She helps accommodate my crazy schedule as I work for high-volume lawyers and am a wife and mom. Dr. Shahmehdi makes you feel comfortable also and does excellent work. My husband and I have gotten our cavities fixed, root canals, and dental cleaning. This office and staff work with you. As long as they're in business, I don't see us going anywhere. I would give this place 10 stars if I could. Once my toddler gets all her teeth, we will schedule an appointment for her also! LOL I'm recommending this office to everyone. They're very clean and organized and they'll send reminder texts/emails if you want also. We're so happy we found this dentist.
