Clark Holmes Smith Oral Facial Surgery has an incredible team of individuals who share the goal of providing the best possible surgical care in a friendly, welcoming, and clean environment. The surgical staff are experienced certified oral and maxillofacial surgical assistants who assist in administration of IV sedation and surgery. Their backgrounds include experience in dental assisting and registered nursing. All staff are informed administrative personnel, well-versed in health and insurance policies, and all are CPR certified.

Oral and maxillofacial surgery requires up to 6 additional years of hospital based surgical and anesthesia training. As oral and maxillofacial surgeons, Dr. Holmes, Dr. Clark and Dr. Smith and Dr. Charles R, IV treat a wide variety of problems relating to the mouth, teeth and facial regions. Our surgeons practice a full scope of oral and maxillofacial surgery with expertise in corrective jaw surgery, wisdom tooth removal and oral pathology. They can also diagnose and treat facial pain, facial injuries and perform a full range of dental implant procedures.

Our staff is trained in assisting our surgeons with IV sedation within our office setting. Patients are continuously monitored during and after surgery. In addition, general anesthesia provided by a board-certified anesthesiologist is available within our facility.

Dr. Mark Clark graduated Pi Tau Sigma with a Bachelor of Science degree in Mechanical Engineering from the University of Alabama in 1972. He then attended dental school at the University of Alabama at Birmingham School of Dentistry and graduated with honors in 1976. After earning his medical degree from the University of Alabama at Birmingham in 1980, he served his general surgery internship from 1980 through 1981. He completed his residency in oral and maxillofacial surgery, at which time he held the position of chief resident, in 1982.

Dr. Clark opened his private practice of oral and maxillofacial surgery in 1983 in Birmingham. He previously served as Chief of Oral and Maxillofacial Surgery at St. Vincents Hospital for nine years. He maintains a position of clinical assistant professor at the University of Alabama at Birmingham School of Dentistry. He serves on the board of the McCallum UAB Alumni Society. He is a Fellow and member of the American Association of Oral and Maxillofacial Surgeons, a member of the American Dental Association, American Medical Association, and Southeastern Society of Oral and Maxillofacial Surgeons as well as many local organizations.

In July 2002, he and Dr. Holmes joined practices at a state-of-the-art outpatient surgery facility located in the historic Southside neighborhood of Birmingham. They have since opened other locations in downtown Trussville, Gardendale, and in the community of Inverness. In July 2019, Dr. Smith joined the practice and the trio opened a fifth location in Alexander City soon after. The group offers a wide range of treatments including extractions, orthognathic surgery, dental implants, cosmetic, and reconstructive procedures.

Blown away at the kindness and professionalism of the staff at Clark Holmes in Trussville. Can not say enough good things. We had to take our special needs adult son in for a procedure. He hates needles and this was our fourth attempt to deal with this issue (we tried to save the tooth). After failed attempts at UAB, we opted to just remove the tooth. He was anxious and not wanting an IV, but the staff was patient and helped him have courage and now my sweet boy is pain free!

The TLDR version is, dental office failed to submit documents, the office knows anesthesia is a covered benefit and is still charging me the office rate of $330 and not my dental plans contracted rate $114. I went to Clark, Smith Holmes for a tooth extraction and a bone graph. The surgeon did an amazing job. My issue is the billing. At this point I need to note, this isn't about getting my insurance to send money, I'm having to pay this out of pocket, its getting the office to charge me my dental plan rates and not the office rates. I paid $330 for anesthesia and was told once my Cigna insurance approves the anesthesia they would change it to the contracted rate of $104 and refund the $220 different. My insurance denied the claim, not because it isn't covered, but because the office failed to submit supporting documents. I have called my insurance 10-15 times since June and have been told the same thing every time, its covered they just need the documents but they have not been sent in. I have called the dental office and they say they've sent them but my insurance cannot locate said documents. Due to all of this my Cigna insurance has escalated this to their appeal team and I was told today they have tried to reach Clark Smith Holmes on 08/26 and 09/06 with no success. I even sent out on BBB about this and they still won't respond
