For 20 years, Cindy served as a dental assistant before studying to become a dental hygienist. She graduated from Columbus State University in 1998 with an Associate of Science degree in Dental Hygiene. For me, restoring patients dental health and making friends along the way are the most fulfilling aspects of my job.

I love seeing a patient walk out of the practice with a new beautiful smile. Thats the most fulfilling part of my job. Renea obtained certification in 2011 from an accredited dental program. She joined Choice One Dental of LaGrange shortly after and has enjoyed taking on additional duties in her role.

When Dr. Jacobs was just 2 years old, his mother had an idea that he would grow up to be a doctor. As he got older, his love for science and passion for others intersected in a path that led to a career in dentistry.

An Ohio native, Dr. Jacobs went to dental school in state. He graduated from The Ohio State University College of Dentistry in 2008, after completing his undergraduate degree at Bowling Green State University.

For nearly a decade, Dr. Jacobs has focused his skills on incorporating advanced, minimally invasive techniques into everyday dental care. He is frequently taking professional development courses on therapies like orthodontics, sleep dentistry, implantology and cosmetic dentistry.

Its once he sees the confidence and joy on his patients faces when he says he feels the most fulfilled in his career. Achieving a healthy and cosmetic balance is a completely life-changing experience for the individuals that he serves. DENTAL SERVICES

When he isnt at work, Dr. Jacobs and his wife Morgan are busy spending time with their two children. A favorite family outing is taking walks at Bogan Park. He also likes riding dirt bikes and brewing his own craft beer.

Back in October (now June). I had my first appointment and it was very pleasant. I was in and out in an hour, but had a cavity that required a filling and had to come back for cleanings. I was very happy with that experience. I came back for the filling in November, and THAT experience was horrible. I'm not sure the Drs name that preformed it, because I wasn't properly introduced nor have I seen her since. I remember Brooke assisting and I only know HER name because the other female Dr didn't know that it was apparently her middle name. During the entire filling, I wasn't talked through anything, given cleaning/eating/aftercare instructions, not even a "don't eat for a couple of hours" type of direction. I was barely acknowledged. She was getting impatient with me because the numbing cream/shots weren't taking effect like they should've, or fast enough for her liking, and I felt pain shooting down my jawline as she started drilling. When they did the X-ray at the end, she didn't get all the decay and had to go back in and redo it. She acknowledged the second time there was a bit of a gap between teeth, but not to me. To Brooke. As she did a floss, she literally said "that's going to have to be good enough". And didn't do another X-ray. I was then brought to the desk by Brooke. I knew I'd be uncomfortable for a week or two (thanks to google) but by January I was still in so much pain. I've left work early in tears because of this. My gums would get inflamed and irritated, especially where the numbing shots were placed down my gum line, not just surrounding that one tooth, and the gums would swell through the gap and cause more pressure and pain. And she didn't file it down enough so it's raised up compared to the rest, so I can hardly chew on that side. But if I chew predominately with my other side, the top teeth hit the tooth with the filling, causing even more pain. I happened to already have a follow up appointment for a cleaning (in Feb since my phone calls were never answered and messages never returned), and I brought it up. The person doing my cleaning seemed genuinely concerned and apologetic about my experience, and she brought in a different Dr who acknowledged the gap was bigger than it should've been and the pain/inflammation could be caused by food getting impacted, despite me brushing and flossing to avoid this. But also said it wasn't likely to be nerve pain, even though most of the pain was shooting down my jaw like a shock. I wish the Dr had the same concern/worry, honestly. I was told they'd call me to make an appointment to correct this. About 4 months later, more missed calls and even a message on the direct website for an appointment request (which was answered within two days for my original Oct appt) an appointment was finally made! And I only knew because of the text I received to save the date. Not even a call could be made. I show up to the appointment a month later (now June) and they did nothing but look at my X-ray, and my mouth, tell me that it likely IS nerve pain as the filling was so deep and close to the nerve, and the best thing to help with the gap as well would be a crown at this point. Great! Then I'm walked back out to the counter to make ANOTHER appointment. I had really high hopes after my first visit and was excited to begin my oral journey and getting my life together. But it's starting to become one bad experience after the next and it can't even get acknowledged. Should I have gone somewhere else? At this point, yes. But I wanted to give them the chance to correct what they messed up, thinking it was just one bad apple. I'm also new to the area and unfamiliar with other options, plus I work two jobs and it's hard to even find the time.

Called to see if I could get my dad seen with an emergency situation. I gave insurance he has but advised if they did not take it I was willing to pay CASH. I was denied an appointment and told if I would have not given my insurance type she would have let me pay cash. Needless to say they were LITTLE to NONE of help.

I've been a long term patient- cleanings and dental needs. Dr. Jacobs and his assistant Isha have been especially helpful and professional. If you get squeamish, this practice would be a place to go. The care is gentle and compassionate.

I am so upset right now. I have a grand daughter and she is 9. My daughter took her to choice one dental twice and both times when the appointment was over my daughter called me and both her and my grand daughter were in tears. Not just a little dental phobia tears but more upset than ive ever known. All because of the lack of caring and compassion from the dentist there. My grand daughter was crying so bad she couldnt even talk. She finally calmed down enough to tell me this lady dentist a short white lady was so mean to her and hateful and scared her so badly that she wants to go home and hide under the covers and her mom and said please between her tears to never take her there again. Im going to find out her name and report her to the American dental association and better business bureaus and anyone else i can. What kind of person are u to scare a child that way. I dont know how u can lay your head down tonight and be ok with how bad u have frightened this child. SHAME ON U CHOICE ONE DENTAL

Like every other chain they want as much as they can get out of you. The lady who started my cleaning was very nice, that's the 2nd star. A cavity showed up on my x-rays so I asked them to take care of it. I was told I would have to make an appointment for another day even though I was there for a check up... What's the point of going there if they're not going to do whats necessary? They just want to bill for a second appointment. The young lady up front lied and said they need to have my social security number to Bill to insurance, That's a blatant lie. She threatened me saying I had to pay cash if I didn't want to give it to her. I have not given my social security number at multiple doctors and Dentists and they managed to bill. The dentist who I saw there last time who was absolutely fantastic has retired. I will never go back I don't recommend it to anyone. As to your reply, you will never need someone's complete social security number to bill for insurance. To say otherwise is a lie. To demand that of a customer is an invasion of privacy and disgusting. Whether your other patients are knowledgeable is not my concern. I find your receptionists actions offensive. So you're saying there's no other choice one dental that you are affiliated with? Lie again please. Since we're continuing the discussion, I would like to talk about the patient in the other room who was upset because they were in pain while the clinician was saying, "you shouldn't be in pain, that can't hurt". That's the worst bedside manner I've ever heard.

I was sent to pick up paperwork for tax purposes for mom. She spoke to Kim in the office. Kim told her that the papers would be ready at the front desk. On my arrival, I was told that they didn't know about it and Kim was at lunch. No problem with that, my problem was Shannon. I asked how long would it be to get the paperwork and she shut the door in my face. I could hear them laughing in the back about her behavior. That's just rude and unacceptable. Someone needs to talk to her about customer service and manners.

The Dental Hygienists and the doctors are great!! I would highly recommend Choice one to anyone who needs any dental work done! I do have some concerns about Shanon who works in the front. She is very rude and bitter to her coworkers.

Shanon truly loves her job and her patients. We worked together for a total of 22 years. I still have patients that ask about her because they love her. She has always been a hard worker and a team player!
