We understand that Dentistry is an ever-changing field, and we are committed to the pursuit of education in the latest advances in the Dental field so that we can offer to our patients state-of-the-art treatment. Dr. Provenzale is a true leader in the field and maintains credentials at the highest levels. He is one of the select few Dentists to have received formal medical training (i.e. Anesthesiology Residency) and is presently one of only a dozen Board-certified Oral Implantologist (i.e. Implant Dentist) practicing in the entire Chicago area.
Infection control in our office is of the utmost importance to us. To protect our patients and ourselves, we strictly adhere to sterilization and cross-contamination procedures using standards set forth by the ADA (American Dental Association), the CDC (Center for Disease Control), and OSHA (Occupational Safety and Health Administration.)
Additionally, we maintain the highest standard in technology and equipment for unforeseen medical emergencies including Pulse Oximetry, Automated Defibrillator, Endotracheal Tubes, and emergency pharmaceuticals. Dr. Provenzale is certified in ACLS (Advanced Cardiac Life Support.)
Our goal is to provide the best dental care possible while being responsive to the needs of our patients. Building a foundation of trust by treating our patients as special individuals is the very essence of our practice, from the Doctors, our hygienists and throughout our dedicated staff as well. Although we are usually very busy, our policy is to immediately treat any patient in pain. The Doctor will at almost all times be available in the event of a dental emergency, night or day.
Dr. Donald Provenzale, Jr. graduated from Loyola University School of Dentistry, Maywood, IL in 1988 at the top of his class, and subsequently completed residency training in Anesthesiology at Illinois Masonic Medical Center, Chicago, IL, in 1990.
At the time, he was conducting anesthesia research and was given the opportunity to pursue an M.D. degree and ultimately continue training toward becoming a board-certified Anesthesiologist. Due to his love for Dentistry, however, he declined the offer.
Dr. Provenzale is now recognized as aleader in the field, and is one of only 500 Dentists worldwide to be honoredwith the distinction of board-certification in the emerging dental specialty ofOral Implantology.
He is an active member of the American Dental Association, American Academy of Implant Dentistry, and regularly volunteers for both the Illinois Foundation of Dentistry for the Handicapped and Donated Dental Services.

I just had two implants completed by Dr. Provenzale. It was a wonderful experience compared to previous experiences i have had. Dr. Provenzale is board certified, well educated, very professional and so courteous! His staff is also well trained and just as courteous as he is. I had experiences with periodontist doing implants and I did a lot of research to find Dr. P. and can say it was well worth it! I would recommend Dr. Provenzale for general dentistry but especially for implant surgery.

been going here for about 5 yrs now always have a pleasant experience staff is friendly and and professional had 2 implants done here one by dr donald jr and each time I came here implant visit was easy no discomfort no pain the Dr Provenzale Jr was very professional his office called me the next day to make sure was feeling great .I would definitely get a implant again from both doctors. Dr Karen Kowalesik did my implant today very easy procedure no pain she a very professional doctor both implants came out great highly reccomend this office

Dr. Karen Kowalesik was very professional, informative and compassionate. I came in her office and had 2 surgical extraction done as well as 2 socket bonegrafts on lower molars.Also I had a CT scan done at the office. I was very impressed that they had a CT scan in their office and I didnt have to go anywhere else.The staff called the following day to check in on me. I would definitely recommend Dr. Kowalesik.

today i received m implants. i was surprised how quickly it took. when i looked in the mirror i again was surprised how beautiful they looked. The entire process was with very little pain and now i am sorry i did not do it sooner. dr. provenzale jr is a great dentist and i am pleased and i was able to get him to do my implants

Over the years when I have said I have the best dentist ever and someone replies they do and we compare-- it is Donald Provenzale, Jr! More credentials, and certifications than anyone. Do you know what a difference that makes? Many look for a oral surgeon not realizing many of them are not certified in implants or in many other things that Dr. P has been doing since the beginning. And you will love everyone in his practice. .

Had implants done over four years ago. Needed to have surgery correcting bone lose in areas were teeth were missing.Had two seperate sets of implants consisting of two and three teeth each put in. All was performed extremely professionaly and with very little discomfort at all. Has been a joy ever since.

Awesome Dr.Donald Provenzale, Jr. introduced serious need for care of an infection and tooth replacement! The game plan was on what to do! The total procedure created by Dr. Donald Provenzale, was the surgical exactraction and right procedures and quickly cured the underlying infection, created the bone graph needed to support the current modern methods needed of IMPLANTS! I wanted to have with no pain with this implant plan and as promised me and Dr. Donald Provenzale, DELIVERED FULL REPLACEMENT OF MY TOOTH and avoided an alternate bridge! I most highly recommend and refer you to seek out Dr. Donald Provenzale, for your every tooth problem and Dental implants need!

I have been a patient of Dr Provenzale Sr for 48 years. The care I have received has been excellent. In 2017 I had 4 implants done by Dr provenzale jr to replace a large bridge and the outcome was great. My smile is so much better and my ability to enjoy food and life was much improved! The implant procedure was painless I was very happy will all the care that was provided to me at Chewswisely Implant Center at Dr Provenzale JR office also the staff called me the next day to make sure I was doing great .I would do implants in a heart beat with again Dr Provenzale Jr. HIGHLY Recommend Dr Provenzale Jr for anybody needing a implant or a second opinion.He does all surgical procedures in his office at his implant center office he does everything from start to finish for the implant procedure.His staff is excellent and very professional . I will only get implants at Dr Provenzale Jr chewswisely implant center

Dr Provenzale has done 3 of my implants. I was so happy that I had no pain. He did the extraction, bone graft and implant and crown. It made it so easy that I was able to do the whole process with him. It made everything so much easier. Dr P has the ability to put all your anxiety at ease. When the process was done I was so happy to have them. I was so upset that I had to have my teeth pulled but he explained my options and the end result is beautiful! I will definitely be referring anyone that needs a dentist. The staff answered all my questions and even called to check on me. They're the best!!!

Dr Provenzale and his office staff is tremendous to work with. From scheduling and billing to getting implants, their level of knowledge and service is outstanding. It is especially helpful to have everything involved with getting implants completed in 1 office by Dr. Provenzale.

Dr. Provenzale is the BEST. I have been terrified of dentists all my life and have had several bad painful experiences over the years. Now in my sixties and had to have implants 4 to be exact and crowns to the remaining front teeth. Went to a new dentist close to home and she was very nice and she sent me to an oral surgeon who did the implant posts. It was a very painful experience. I expected to have pain but not to the extent that I did. It was the worse 10 days. I wanted a second opinion because after 5 months still without getting my implant crowns, I was told I will need 2 more implant posts because the dentist didn't think my 4 front teeth would hold up to crowns. I was beside myself thinking of repeating the pain. So my dear friend told me about ProDental and Dr. Provenzale. I made an appointment and saw him and Michelle. After reviewing my x-rays and taking some new ones, he said there is no reason I would need more implant posts. He was willing to finish the job for me and promised me no pain. He explained everything and answered all my questions. The fact that he was trained in anesthesia early in his career made a big difference to me. Well, I just finished getting my permanent implants crowns and front teeth crowned. They look beautiful, I feel great and it truly was PAINLESS!! I recommend Dr. Provenzale to all my friends and family. I am so glad I found him.

I had 3 Implants done at Pro Dental by Dr. Provenzale Jr. I was amazed on how well they went . It is a multi step process I need extraction and bonegraft done as well its all done at his office I was glad to know that everything was done by him with very little discomfort and they went very smooth and no pain. The implants work great he's board certified oral implantologist and I would highly recommend getting them, if done by Dr. Provenzale. It has made eating easy with no problems and they look great in my smile. If you do need implants as recommended by your dentist you should get them done by Dr. Provenzale at chewswisley implant center.

I have a cleft lip and came to Dr. Provenzale for implants. He also did a tissue graft for cosmetic purposes. Everything he has done has exceeded expectations! Him and everyone in the office are amazing and I'm so glad we were able to find Dr. Provenzale for this process!

I had two implants and Dr. Provenzale Jr and his team were great. It was so convenient to have the bone graft and extraction done and also implants done by Dr. Provenzale Jr at the same location. I had no pain after implants procedure and the office called me to check up on me after implant surgery..I have been coming to this practice for 10 years .I would recommend Dr Provenzale Jr to anybody that looking for a Board certified Implantologist that does everything at the same location which was great his staff is great very professional I look forward going to the dentist they make you feel like family .
