Franciscan ACO, Inc., provides coordinated, comprehensive care across hospitals, physician practices and other healthcare providers to enhance the healthcare experience of patients under our care. Since 2011, Franciscan ACO, Inc., (formerly Franciscan Alliance ACO) has developed innovative care models. Currently participating in the Medicare Shared Savings Program (MSSP), Franciscan ACO, Inc. has expanded to also include Accountable Care Organization partnerships with Medicare Advantage and commercially insured patient populations.

Within each of these programs attributed patients, known as beneficiaries, receive the added benefit of a more coordinated care experience. This includes coordination of preventive health services and care coordination support for persons with chronic health conditions, such as diabetes and congestive heart failure. Care coordination services are provided through the ACO. Better yet, beneficiaries maintain the ability to see any doctor or healthcare provider of their choice within their insurance plan, as well as the full benefits associated with their covered insurance benefits.

The selfless acts of St. Francis of Assisi a man who left behind all his worldly possessions to follow Christ's mission and care for those in need set forth the guiding values under which Franciscan Health began. In 1875, a group of Catholic Sisters from Olpe, Germany answered a call to bring St. Francis ministries of health and education to the Midwest United States. Here, they opened the doors of our system's first hospital what is now Franciscan Health Lafayette.

Founded by Mother Maria Theresia Bonzel, the congregation later became known as the Sisters of St. Francis of Perpetual Adoration. The Sisters expanded the health system with more hospitals, schools, orphanages and homes for the elderly. And nearly a century after the first hospital opened its doors, the system was officially founded as the Sisters of St. Francis Health Services in 1974.

Our system recognized the evolving changes in the health care landscape and the need to reinforce the shared capabilities across our hospitals and other healthcare facilities. We adopted a new name Franciscan Health to strengthen our presence as a large, comprehensive healthcare system. In 2012, our health system was selected to participate in a Pioneer Accountable Care Organizations (ACOs) initiative by the U.S. Department of Health and Human Services. The ACO model is designed to provide more coordinated care for Medicare beneficiaries, ultimately resulting in cost savings for patients.

    I was extremely nervous as I'd had several painful dental procedures in the last few months thru regular dentists. I was sweating & shaking, literally. Dr. Hine does a great job putting you at ease, administering numbing medication and removing the nerve. I thought I'd need pain meds after but have had very little discomfort. My jaw isn't even very sore!

    Dr. Charles Hine and his staff are very kind, thorough, efficient and truly one of a kind. They make scheduling appointments a breeze, their office is conveniently located, and their facility is very clean. I'm a neat freak and complete germaphobe and felt completely confident that everything was clean and safe. My injections were pain free, and my root canal was completed quickly. I would recommend anyone needing endodontic treatment to see Dr. Hine and staff!

    I'm so thankful I went to see Dr. Hines- injection was great and work he did was top notch. I am a dental professional and he was able to address an issue I'd been having for awhile that no one had been able to identify. I finally have relief after 5 years! Highly recommend!
