Infection Control Procedures ensure your safety, and all procedures at our office exceed even the most stringent guidelines established by the Center for Disease Control (CDC) and the Occupational Safety and Health Agency (OSHA).
All instruments and hand pieces are autoclave steam sterilized after every use and disposable items are utilized wherever appropriate. Patients and staff wear protective safety glasses during treatment. Disposable gloves are worn while treating patients and are never used on more than one patient. Staff members wear masks during the procedures to protect our patients.
All areas that are touched during the procedures are covered with a plastic wrap and changed after each patient so as to eliminate the potential of cross contamination. All surfaces, chairs, lights and units are thoroughly disinfected with a chemical solution formulated to kill infectious bacteria, spores, and viruses before and after every patient use.
At Forestview Family Dentistry, our team members undergo extensive training in sterilization procedures and participate in continuing education courses to keep up-to-date on the most advanced techniques in the industry.
Dr. Andrew Kollath is originally from De Pere, WI. He attended St. Norbert College for his undergraduate degree and received his Doctor of Dental Surgery degree from Marquette University School of Dentistry where he graduated Summa Cum Laude.
While Dr. Kollath enjoys all aspects of dentistry, he is especially interested in cosmetic dentistry including crowns, bridges and implants. He also enjoys endodontics and general restorative dentistry on both adults and children. He is affiliated with the American Dental Association and the Wisconsin Dental Association.
Outside the office, Dr. Kollath enjoys watching and playing sports including football, baseball, golf and basketball. He is a Packers, Brewers, Bucks and Wild fan. He also likes to travel, especially to the national parks where he can camp and hike. He enjoys other activities like kayaking, water sports, running and weightlifting. He is also the proud uncle to a new nephew!

Dr. Endsley grew up in Eagan, MN and attended St. Olaf College for his undergraduate studies. He then went on to earn his DDS from the University of Illinois at Chicago after receiving the Health Professions Scholarship from the US Army. Dr. Endsley served 6 years in the Army, and during that time completed a rigorous 2-year Advanced Education in General Dentistry residency, earning a Masters degree in Oral Biology from the Uniformed Services University of the Health Sciences in the process. During his final year in the military, Dr. Endsley also served as an adjunct assistant professor for the USUHS Postgraduate Dental College, serving as a clinical instructor for the Langley AFB 1-yr AEGD residency program. Currently, Dr. Endsley serves as an adjunct assistant professor at the University of Minnesota College of Dentistry.Dr. Endsley is one of the few dentists to be both a Diplomate with the American Board of General Dentistry, and a Fellow with the Academy of General Dentistry. He maintains memberships with both of those organizations, as well as the American Dental Association.Dr. Endsley is thrilled to be back in his home state with his wife, Alex, and daughter, Emmalyn, and while no longer on active duty, he plans to continue his service with the MN National Guard.

I recently visited Dr. Koivisto for my first root canal treatment. Dr. Koivisto was extremely informative and answered all of my questions. I could not be happier with my treatment - it was not painful or uncomfortable. Dr. Koivisto and his assistant were very accommodating during the whole process. It was actually more tolerable than any filling I have received! I would 100% recommend Dr. Koivisto and his staff for any dental treatment needed!!

April is amazing!!!! I was obviously nervous going into the dentist but she made it a great experience! I would recommend this place to anyone. Great staff, great hours. Just great everything!

Awesome staff, awesome dentists! Dr. Kyle Smith made me feel comfortable and important as a patient. It was clear that making my teeth feel better was a priority to him. Dr. Smith takes an approach where patient input is heard and utilized. He is a very good communicator. I felt no pain throughout my visit, even though I had a deep cavity close to the nerve. Previous experiences with other dental clinics in the area have been sub-par. I'm really happy that I found Cedar Island Dental, it is where I'm going to take the rest of my family when they need dental care.

Dr. Smith was amazing!!! This was my first visit to C.I.D. and I was so impressed. Novocain shots. I didn't even feel them! I have been to several DDS and I've never experienced having three fillings and a cleaning with no pain at all! I would recommend seeing Dr Smith to anyone. I haven't had any pain since the work and it has been 5 days since I had the work done. The staff made me completely comfortable and we're not in a hurry to rush me out the door in any way. Five stars is an understatement 10 stars for Dr Smith and his team.

Best dental office ever! Brought my toddler aged daughter in with me to my appointment to show her what it was all about. They treated her like she was getting a cleaning too! It was above and beyond from the hygienist to the dentist! Highly recommend!
