Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

Had a dental cleaning this week. Super staff that works for Dr. Tao and excellent services all around. Beautiful new premises! I have referred many friends and family members and all have chosen him as their dentist. Kind and caring!

I always enjoy coming to this dentist, which i know is weird. Alice, Amanda, and Cassie are always super nice when i walk in. The hygenists and Dr Tao are always friendly and listen to any concerns I've had in the past. Never thought i would enjoy going to a dentist, but this dentist office is the reason i take much better care of my teeth!

Honestl, everybody hates the dentist but the people there are awesome. They're really attentive, caring and funny. It was quick and painless. I was in and out of there within 15 minutes. Thank you for making an unpleasant experience pleasant. Cassie, the blonde in the front desk has your back, trust me. I do hate the dentist, but if I had to go back I wouldn't go anywhere else.

I love Carnelian Dental! I have been using this dentist for close to thirty years! Dr Tao has assembled a staff of specialist and state of the art equipment to serve every clients needs. His new building is beautiful and the office staff will make you feel safe and loved! Thank you everyone, my implant will be a work of art when I'm done!

Dr. Tao and the Carnelian Dental staff have been great through all our dental care needs. My family have been going there for years. With humble beginnings in their corner office, they have now grown into their brand new modern office. Well deserved too. They have always been honest and fair with their dental recommendations. Carnelian Family Dentistry is the best!! Dr. Tao, is a great dentist! He's caring, patient and takes his time to explain all procedures. The staff is amazing as well. The ladies are so nice and very pleasant to speak with. Alice has a great personality and is always there to help. The staff is always happy, who wouldn't be, working with Dr. Tao. This quack pulled two teeth that had root canals and wanted to pull a third, and give a fourth tooth a root canal as a preventive measure on my friend, who got a second opinion and over a year later he stills the tooth Dr. Tao would have pulled, and no root canal on the other. If you want to avoid unnecessary pain, and cost stay away from Dr. Tao!

Dr. Tao and all the staff here are incredible. The girls in the front are always so nice and welcoming. A friend recommended this place and spoke so highly of them and I thought, it's just the dentist, how am I going to LIKE it? Turns out, I had the best dental experience of my life. They're very communicable about what they see going on with your teeth and their recommendations. Highly recommend this dentist.
