For Elders and their Care Partners, the Harbor Health Elder Service Plan (PACE) provides a personalized healthcare program, developed by our geriatric medical care professionals, guided by an individualized care plan developed to meet the participants individuals needs.

As a PACE Participant your healthcare is coordinated by a team of caring professionals at the PACE Day Center, transportation included, or when needed in your home. PACE provides care for you 24 hours a day, seven days a week and 365 days a year.

The PACE model of health care began over 30 years ago in California when the San Francisco community saw the pressing needs of families whose elders needed long-term care services. From these beginnings, the PACE Model has become a nationally known program providing a full range of care to older adults over age 55 with chronic care needs while allowing them to remain in their own homes and communities for as long as possible.

Harbor Health Services Inc. is a registered 501(c)(3) non-profit public health agency with a mission to help individuals achieve their full potential through access to local, affordable services that promote health. Harbor provides medical, behavioral health, dental, and support services to more than 34,000 patients in Boston, the South Shore, and Cape Cod.

In addition, Harbor Health operates two Programs of All-Inclusive Care for the Elderly, a program that allows frail elders to live in their homes and communities, as well as the Dorchester South/South Boston Women, Infants, and Children (WIC) Nutrition Program.

Our Elder Service Plan has two medical and day centers in Mattapan and Brockton and cares for more than 450 participants. Our WIC program provides services at Harbors Dorchester locations, as well as at other community health center and mobile locations. WIC serves more than 3,000 women and their children each year.

Harbor Health is a Federally Qualified Health Center and is a deemed facility under the Federal Tort Claims Act (FTCA). The FTCA provides medical malpractice coverage to all deemed facilities and its staff.

Dr. Jack Geiger and Dr. Count Gibson founded the first community health centers in the nation in one urban and one rural community. Dr. Geigers vision sought to confront abject poverty and medical disenfranchisement. He believed a health center would not only provide much needed medical relief and social services, but also a path toward community empowerment and direct citizen action.

Located on an isolated and impoverished peninsula in Boston, Massachusetts, Columbia Point Health Center, now known as Geiger Gibson Community Health Center, became the first operating community health center in the nation when it opened in 1965.

When the late Senator Edward M. Kennedy visited the health center in August 1966, he saw how the lack of medical care gravely impacts a neighborhood and the promising solution of Columbia Point Health Center. The visit had a profound effect on Kennedys thinking, as he explained years later: I have been a strong supporter of community health centers since the inspiring day I visited the one that started it all

Harbor Health Services was formed as a 501 (c)(3) organization in 1985 when the Columbia Point and Neponset Health Center came together in pursuit of their steadfast commitment to community health. Columbia Point was officially renamed Geiger Gibson Community Health Center in honor of the men who started the community health center movement.

Today, Harbor Health continues to respond to current and emerging health needs in communities not only in Boston, but also on the South Shore and Cape Cod. More than 34,000 patients a year receive care at Harbors five community health centers.

Harbors community health centers offer kind, caring, and affordable medical, dental, behavioral health, pharmacy, and social support services. Harbor Health also operates two Program of All- Inclusive Care for the Elderly in Mattapan and Brockton, providing comprehensive healthcare, transportation and social services for more than 450 frail elders who continue to live with dignity and independence in the community.

Its not that he was rude or anything, but his office doesn't have a spittoon, so you have spit in a cup he holds when you rinse. Also, he cancelled our first appointment because of a snow storm, which is fine and understandable, but then charged me for it. As such, I was charged twice (a very small amount) for the same appointment, the one that was cancelled and the one I actually attended. When I asked about this, I was told there is a two week period before I would hear anything about the complaint, so I just paid them and moved on. So DDS Carlos is nice enough, but I'll be looking elsewhere in the future for dental services.
