Below is our New Patient Package that you will need to complete on your first visit. You can expedite matters by downloading the package, completing it at home and handing it to our staff when you arrive for your initial visit to our office.
Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff. On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.

We accept many insurance plans including Delta Dental PPO/Premier, Cigna, Metlife PDP+, Dental Health Alliance, Connection Dental, United Health Care PPO and Aetna as well as most other major PPO's. Throughout the year, our dentists and staff travel to provide desperately needed dental care to under-served nations throughout the world. In many cases, we do this in conjunction with charitable organizations that operate every day to provide needed goods and services to these nations and communities. My experience with dentists in the past have not been great. Because of that I haven't been to a dentist in quite some time. I had my first visit at this office today and I can say that I will be a long time customer. The office staff are so kind and welcoming, the dental hygienist was very sweet and provided so much information as she was cleaning my teeth and both dentists came and introduced themselves to me. It was a very comfortable and friendly environment. I highly recommend them!

I have always had a healthy dislike for going to the dentist because my childhood DDS had hands the size of an NFL Lineman and the sensitivity of a prison guard. Dr. LaCouture has effectively overcome my deep rooted fears with some amazing care and outcomes over the years. Which included driving in to open the office at midnight to replace my front tooth which was knocked out in a basketball game an hour earlier. IMO you can not find better dental care, a better dentist, or a better atmosphere, if you need to go. Thank you to Dr. LaCouture and staff!

My experience at Burning Tree Family Dentistry went great! Walked in with dental appointment anxiety and between the staff and Dr. Cary was put at ease with their friendliness and professionalism. Dr. Cary is awesome! Talked to me about what he was going to work on at that appointment and before I knew it, 3 teeth were repaired. Dr. Cary is calm and gentle. Next appointment, a whole lot less "anxiety"!
