While Dr. Hazen is perfectly good, avoid his office staff at all costs. I'm only editing the rate to 1 star because there is no option for zero star. I highly recommend you find another office that treats people honestly & politely ASAP.

My husband had his first appointment with Dr. Hazen today. What a nice man and an amazing dentist. The Dr. took his time to explain everything in detail regarding what needed to be done. His entire staff was friendly and extremely professional. I highly recommend Dr. Hazen!!

Absolutely Marvelous... I have many crowns in my mouth and Dr. Hazen is top notch and I. Love the staff my Dental Hygienist is also absolutely terrific. So glad I have someone who really cares..

Absolutely Marvelous... I have many crowns in my mouth and Dr. Hazen is top notch and I. Love the staff my Dental Hygienist is also absolutely terrific. So glad I have someone who really cares..

Wonderful, caring office from the staff to Dr, Hazen. Have been going there for twenty years. Always on time with appointments. Gotta go there.

The dentist himself, Hazen, is quite good and knowledgeable However, Im giving 2 stars because the receptionist was quite nasty. I called to schedule an appointment and she acted as if she didn't want to book me, not to mention that she was very presumptuous over my dental work I had done by a different dentist. She is not a dentist, so Im not sure why she even was saying that I didnt need anything done, especially if my tooth was STILL bothering me. And she never called me back, after she said she would. Will find a different dental office because of that unpleasant experience.

Not all heros wear capes, some wear white coats, or even John 5 Tshirts...Dr.Hazen is by far the best dentist around. I broke my tooth late on good friday, I called Dr.Hazen and received an immediate call back. He managed to rally around to get an assistant to come into work on Easter Saturday, and had my tooth fixed temporarily for the weekend....if only everyone had this type of commitment to their work ! 5 stars is not enough for Dr.H and his assistant, Kelly! Thankyou again ;)

The staff at Hazen Family Dentistry is amazing. They are patient, kind, and understanding and do everything possible to give you the healthiest smile. I have had multiple issues with my teeth over the years and am VERY fearful of the dentist. Through back and forth with insurance issues I made the decision to pay out of pocket for Hazen instead of going with a corporate dentist that my insurance provides. Today, after tireless issues with another dentist, I called Hazen back to talk about making this decision. Tabitha was amazing, she was kind, thoughtful, and calmed me down in time of stress. When you are a fearful person, she knew EXACTLY what to do to ease my fears. It's because of the quality work and customer service they provide that I will continue to go here. Thank you!

Dr. Hazen is a gem within Ormond Beach and a true Dentist that cares for his patients. Unlike some other area providers, he takes the time to get to know his patents as does his wonderful staff. Classic Rock music and him being a proud Florida Gator alum makes it even better. GO GATORS!

Excellent dentist, personable, and takes pride in his work. I'm moving for my job and am sad to know I will not be able to see Dr. Hazen in Clearwater, FL. Finding another Dentist that performs the quality and detailed work will be next to impossible. Thanks for all you have done in the years I have been seeing you.
